Case Report: Nursing care in a patient who has erosion around gastrostomy.

Introduction: We aimed to consider nursing care and successful management of posthospital period in a patient who has serious erosion around gastrosttomy

Case report: A 9-years-old boy accepted to intensive care unit (ICU)(he had trauma when he was 5 years) has loss of consciousness and sense at low extremity, tracheostomy, and percutan endoscopic gastrostomy (PEG). He has been hospitalised to pediatric diseases ICU with complaints of fever (40°), abdominal pain, distension, 185 pulse/minute, intercostal retraction, intensive urine with bad smell, hyperemie, drenage and skin erosion around PEG. Pediatric Surgery consultation was wanted because of increased distantion and gastric remnant, erosion around gastrostomy, problem of enteral feeding, and prediagnose of intraabdominal abcess. Non-invasive care protochol was performed by pediatric surgery team after physical examination and radiologic tests. The cause of distantion was septic ileus and immobilisation. Firstly enteral feeding was stopped, secondly parenteral feeding was applied and gastrostomy tube was directed to free drenage. Bowel sounds, gastric drenage, and defecation were monitorised. Meanwhile enema was ordered. Enteral feeding was begun with 10 ml/hour in 7th day of follow-up. But, enteral feeding was again stopped because of drenage of around of gastrostomy. Treatment was performed to maintain gastric motility. Care of around of gastrostomy (gastric tube irrigation, irrigation of around of gastrostomy with saline, physical debridman, local rifampicin application, drying with light and dry oxygen, antibiotic, epithelisan and zinc cream, wound wrap with silver) was performed every day. Duration of care was 40 minutes per day; skin entegrity of circumference of gastrostomy turned to normal constitution at the of 21 days. Feeding was accomplished with the control of remnant; good tolerance was observed.

Conclusion: It is important to decide whether surgical endication is necessary or not. However, good management of gastrostomy care and successful care period in house prevent unavoidable problems.

Olgu Sunumu: Gastrostomi çevresinde erezyonu olan bir hastanın hemşirelik bakımı

Giriş/Amaç: Gastrostomi çevresi ciddi erezyona uğramış olgunun, hemşirelik bakımını paylaşmayı ve hastane sonrası bakım sürecinin başarılı bir şekilde yönetilmesine dikkat çekmeyi amaçladık.

Olgu: 9 yaşındaki erkek olgunun; 5 yaşında geçirdiği araç dışı trafik kazası sonrasında bilinç kaybı, alt ekstremitelerde his kaybı, solunum yetmezliği sebebiyle çocuk hastalıkları yoğun bakım ünitesinde (YBÜ) takip edildiği ve trakeostomili-perkutan endoskopik gastrostomili (PEG) olarak taburcu edildiği öğrenildi. 11.02.2018 de 40° ateş, karın ağrısı, distansiyon, 185/dk nabız, interkostal çekilme, koyu renkli ve kötü kokulu idrar, koyu trakeostomi akıntısı, PEG çevresinde hiperemi, akıntı ve cilt erezyonu şikâyetleriyle çocuk acile başvuran hasta, takip ve tedavi amacıyla çocuk hastalıkları YBÜ’ne yatırılmıştır. Distansiyon, artmış gastrik rezidü, gastrostomi çevresinde erezyon, enteral beslenme problemi, intraabdominal abse ön tanılarıyla olgunun çocuk cerrahisi konsültasyonu istendi. Çocuk cerrahisi ekibi tarafından, anamnez, fizik muayene, radyolojik inceleme yapıldı ve non-invaziv bakım protokolü oluşturuldu. Distansiyon sebebi, sepsisle ilişkili ileus ve immobilizasyon olarak düşünüldü. Öncelikle enteral beslenme sonlandırıldı, parenteral beslenme başlandı ve gastrostomi tüpü serbest drenaja alındı. Barsak sesleri, gastrik drenaj, gayta çıkışı izlendi ve lavman önerildi. Takibin 7. Gününde enteral beslenme 10 ml/saat başlandı. Ancak gastrik rezidü ve gastrostomi çevresi sızıntının artması sebebiyle enteral beslenme tekrar stoplandı. Gastrik motiliteye yönelik tedavi başlandı. Gastrostomi çevresi bakımı (sondanın yıkanması, serum fizyolojik ile gastrostomi çevresini yıkama, fiziksel debridman, lokal rifampisin uygulama, ışık kaynağı ve kuru oksijen ile kurutma, antibiyotik, epitelizan ve iyileşme sürecine göre çinko içerikli krem/pomat ve gümüş içerikli emici özelliğe sahip yara örtüsü ile kapalı pansuman) günlük yapıldı. Bakım süreci 40 dk/gün şeklindeydi; 21 günü tamamladığında gastrostomi çevresi cilt bütünlüğü tamamen normale döndü. Rezidü kontrolüyle beslenmeye başlandı ve tolerasyonun çok iyi olduğu görüldü.

Sonuç: Gastrostomi komplikasyonlarında, öncelikle cerrahi endikasyon olup olmadığına karar vermek önemlidir. Bununla birlikte, gastrostomi bakımının iyi yönetilmesi; kaçınılmaz sorunları önler ve evdeki bakım sürecinin başarılı bir şekilde sürdürülebilmesini sağlar.
