IS IT POSSIBLE TO ACHIEVE BETTER COSMETIC APPEARANCE IN LAPAROSCOPIC ASSISTED MORGAGNI HERNIA REPAIR?

 Morgagni hernias are  rare and comprise approximately  1% to 6 % of all congenital diaphragmatic hernias in childhood. They are usually asymptomatic  and discovered incidentally.  Once open surgical procedures have been accepted as a gold standart,  recently , minimally  invasive techniques have been described to repair in increasing numbers. There are  various reports  of repair using minimal invasive surgical techniques  with  laparoscopic primary intracorporeal  sutured closure, mesh closure or combination of both. Laparoscopic- asisted repair of Morgagni hernia by taking full thickness of anterior abdominal wall  with extracorporeal knot tying in the subcutaneous tissue is another option. Here we present  our results  with  the  modification in laparoscopic- asisted repair of Morgagni hernia.

Method and cases:

All morgagni hernias admitted to our institution have been operated laparoscopically since July 2009. During study period,  laparoscopic repair  of Morgagni hernia were performed  in seven children aged between 3 months to 13 years. There were three girls and four boys. Intracorporeal   sutured closure was applied  in four children and anterior abdominal wall  repair  under direct vision with extracorporeal knot tying with our modification were performed  in last three cases.

Patients were placed in reverse Trandelenburg position.Three ports  were inserted into the abdominal cavity for the  procedure. No attempt was made to excise  the sac in most  cases. 1-2 mm incisions, small enough to allow the needle  pass through,  were made.   2-0 polypropylene sutures were then passed percutaneously through the abdominal wall into the abdominal cavity. The needles were grasped with a laparoscopic needle holder and  passed through the  folded hernia sac. A 18-gouge branule with a circuit made suture material was inserted . After the circuit suture around the needle was completed,  the needle was removed  via  inside of the canula at the exactly   same location where the needle had been inserted before.  The sutures were tied and   burried in the same place.  Four or six sutures required to complete defect closure. No recurrence was observed in both intra corporeal and extracorporeal closure.

Results:  Although  laparoscopic  repair of Morgagni hernia is valuable,  laparoscopic –assisted repair of Morgagni hernia using the full thickness of anterior abdominal wall with extracorporeal knot tying in the subcotaneous tissue with our modification  was found much  more safe,  easily applicable and  better cosmetic appearance.

LAPAROSKOPİK MORGAGNİ HERNİSİ ONARIMINDA DAHA İYİ BİR KOZMETİK GÖRÜNÜM MÜMKÜN MÜ?

Morgagni hernisi  nadir rastlanan bir hastalıktır. Tüm çocukluk çağı diafragma hernilerinin %1 ile 6 lık bir bölümünü oluşturur. Genellikle asemptomatik olup rastlantısal olarak tanı alır. Açık onarım  altın standart olarak kabul edilmiş iken son zamanlarda gittikçe artan sayıda laparoskopik onarımda bildirilmektedir. Yapılan laparoskopik onarımlar genellikle   primer olarak intrakorporal sütürlerle defektin kapatılması, meş uygulanarak defekt onarımı yada her ikisinin kombinasyonu şeklinde bildirilmiştir. Son yıllarda yapılan modifikasyonlarla laparoskopik onarım tekniği daha da kolay uygulanabilir hale gelmiştir. Çalışmamızda laparoskopik Morgagni hernisi onarımı yaptığımız olgular ve tedavi yöntemimizdeki modifikasyonumuz sunulmuştur.

Yöntem ve olgular:

Kliniğimizde 2009 Temmuz ayından itibaren gelen tüm Morgagni hernileri laparoskopik yöntemle onarıldılar.  Yaşları 3 ay ile 13 yaş arasında değişen toplam 7 çocukta ( 3 kız, 4 erkek )  laparoskopik  onarım  gerçekleştirildi. Dört hastada primer intrakorporal sütürlerle onarım yapılmış iken, son üç hastada  laparoskopi destekli tam kat anterior karın duvarı onarımı eklediğimiz modifikasyonumuz ile uygulanmıştır. Hastalar ters trandelenburg  pozisyonunda  yatırıldı, işlem için üç  port girildi. Defektin karın duvarındaki iz düşümünden, düğüm gömülecek kadar  mikro cilt insizyonları (2-3 mm) yapılarak; buradan karın içerisine 2/0 polipropilen sütür girildi. Kese disseke edilmeden, sütür defektin arka kenarından   geçirildi. Mikro insizyondan sokulan kement aracılığı ile iğne dışarıya alındı ve cilt altında düğümlendi. Defektleri kapatmak için 4 ile 6 dikiş yeterli bulundu. Şu ana kadar olan takiplerinde herhangi bir nüks saptanmadı.

Sonuç:

Morgagni hernisinde laparoskopik onarım başarı ile uygulanabilir bir tekniktir.  Laparoskopi destekli tam kat anterior karın duvarı onarımı  standart yöntemlere göre uygulanması kolay ve başarı oranı yüksek bir yöntemdir. Bu  yöntem aynı zamanda daha iyi kozmetik sonuçlar vermektedir.
