ISOLATED İMPERFORATE ANUS IN NEWBORN TWİNS: A RARE CASE REPORT

Introduction:Anorectal malformation (ARM) is a congenital disease that seen of 1 in 2500-5000 live births and has familial occurrence. There is additional anomaly 50-60 % of the cases with ARM. . Isolated perforated anus in newborn twins is a rare situation. In this report, twin newborns with imperforated anus are presented.

Case:A 32 years old mother delivered a male twin at the third gestation normally by vaginal route  and one of the male twins is 2200 gr and the other is 2300 gr. It was diagnosed pearl sign at the examination of the perineum of the twins referred to our clinics after noticed that they have no anus. However, it was not noticed fistula at the perineal area of the twins. It was not noticed additional anomalia at the end of the other systems and radiologic examinations. Anoplasti was applied to patients with low type (infralevator) ARM diagnosed in their invertograms after 18 hours later from delivery. The patients without any complication were discharged after 3 days later after oparetion.

Conclusion:It was reported only 5 twin newborn with isolated ARM at the literature until now. All patients were monozygot that have anal stenosis in 3 twins, imperforated anus in 2 twins. . It was not noted male twin babies with imperforated anus without fistulae such cases represented at the English literature. The incidence of ARM’s is increasing among brothers except for twins. Many factors have been implicated from the etiology of the ARM until now. becouse  incidence of diseases such as Down Syndrome in the family is much more,  genetic factors in etyopathogenesis of this disease is thought to be effective. . We thought that effective genetic factors at developing of ARM were better understood reporting twin babies with imperforated anus as this report.

İKİZ YENİDOĞANLARDA İZOLE İMPERFORE ANÜS: NADİR OLGU SUNUMU

Giriş:Anorektal malformasyon (ARM) 2500 ile 5000 canlı doğumda bir görülen ailesel geçiş gösterebilen doğumsal bir hastalıktır. ARM bulunan olguların %50-60’ında ek anomali vardır.  İkiz yenidoğanlarda izole imperfore anüs görülmesi çok nadir bir durumdur. Bu rapor da imperfore anüsü bulunan ikiz yenidoğanlar sunulmaktadır.

Olgu: 32 yaşındaki annenin 3. Gebeliğinden olan normal vaginal yolla doğan erkek cinsiyetteki ikizlerden 1.si 2200 gram, 2. İse 2300 gram idi. Anüslerinin olmadığının farkına varılmasıyla kliniğimize sevkedilen ikizlerin perineal muayenesinde inci işareti (pearl sign) tespit edildi. Bununla beraber hastaların perineal bölgelerinde fistüle rastlanmadı. Diğer sistem muayenelerinin ve radyolojik tetkiklerin sonucunda hastalarda ek anomaliye rastlanmadı. Doğumdan sonra 18. saate çekilen invertogramlarında alçak tip ARM tanısı konulan hastalara anoplasti uygulandı. Herhangi bir komplikasyon görülmeyen hastalar ameliyat sonrası 3. günde taburcu edildi.   

Sonuç: Literatürde şimdiye kadar izole ARM’si olan sadece 5 ikiz yenidoğan rapor edilmiştir. Tüm bu hastalar monozigot olup, 3 ikizde anal stenoz, 2 ikizde ise imperfore anüs bulunmaktadır. . İngilizce literatürde bu yazıda sunulan olgular gibi fistül olmaksızın imperfore anüsü bulunan erkek ikiz bebeklere rastlamadık. ARM’lerin ikiz bebekler dışında da kardeşler arasında da görülme sıklığı artmaktadır. Şimdiye kadar ARM etiylolojisinde birçok faktör suçlanmıştır. Down Sendromu gibi hastalıklarda aile içerisinde görülme sıklığının fazla olması, genetik faktörlerin bu hastalığın etyopatogenezinde etkili olduğunu düşündürmektedir. Bu raporda olduğu gibi imperfore anüsü bulunan ikiz bebeklerin rapor edilmesiyle ARM’nin gelişiminde etkili olan genetik faktörlerin daha iyi anlaşılacağı kanaatindeyiz.
