INTERMUSCULAR HYDATID CYST: A CASE REPORT

Primary musculoskeletal and childhood Echinococcus infection without other organ involvement is a very rare condition. Here, we report a case of a 7-year-old boy with primary intermuscular hydatid cyst in the right thigh.

CASE:

A 7-year old boy who was living in rural area was admitted with a complaint of painful swelling in the right thigh with the increasing size for the last three months that caused difficulty in walking. There were no histories of trauma, surgery or illnesses. Laboratory studies showed eosinophilia (15.4%) and leucocytosis (15,300 / mm3). Elisa test was positive for Echinococcus. Ultrasonography (USG) showed 27x44 mm cystic lesion with a thick capsule containing extensive internal septation localized anterio-medial position in the right thigh.

            Magnetic resonance imaging (MRI) study revealed 28x44 mm cystic lesion (Hydatid cyst?)  Preoperatively, 10 mg / kg albendazole was given for 10 days. The cyst was detected between adductor rectus femoris and iliopsoas muscles during operation with under general anesthesia.  The cystic mass was totally removed without destroying the cyst wall (Figure 2) and hypertonic saline was applied into the cavity.

            After excision, mass was opened and germinative membrane and rock water was found in it.(Figure 3). Any problem did not occur in the postoperative period.  The patient was discharged with  10 mg / kg/dayalbendazolegiven for 3 months. On oneyearfollow-up, his clinical examination, laboratory and radiogical tests do not show any echinococcosis recurrence to date.

RESULT: In conclusion hydatidcystshould not beignored in the endemicareas in patients with cystic masses in any part of the body and with suspicion approach radiologicaland serologicalstudies for hydatidcystshould be done before biopsy. When the diagnosis of hydatid cyst was made the cystic masses should totally be removed without drainage.

INTERMÜSKÜLER KİST HİDATİK: OLGU SUNUMU

Diğer organ tutulumu olmadan primer kas-iskelet yerleşimli ve çocukluk çağı ekinokok enfeksiyonu oldukça nadir bir durumdur. Biz burada 7 yaşında bir erkek çocukta sağ uylukta  primer kas-iskelet yerleşimli kist hidatik vakasını sunduk.

OLGU SUNUMU

            Kırsal kesimde yaşıyan 7 yaşında erkek çucuk saü uylukta son 3 ayda boyutunda artma olan ve yürüme güçlüğüne yol açan ağrılı bir şişlik ile getirildi herhangi bir travma, cerrahi veya hastalık üyküsü yoktu laboratuar çalışmalarında eozinofili ve lükositoz güsterildi. Ekinokok açısından Eliza testi pozitif idi Ultrasonografide  sağ uyluk anteriomedial yerleşimli içnde yaygın septasyonlar içeren kalın kapsüllü 27x 44 mm boyutlarında kistik lezyon gösterildi. Manyetik rezonans görüntülemede sağ uylukta 28x44mm boyutlarında, kistik lezyon (kist hidatik) tespit edildi. Operasyon öncesi 10 gün boyunca 10mg/kg albendazol verildi. Genel anestezi altında yapılan opersyon esnasında kist addüktör rektus femoris ve ileopsoas kasları arasında kist tespit edildi. Kistik kitle kist duvarına herhangi bir zarar verilmeden tamamen çıkarıldı(resim 2) ve kavite hipertonik salin ile yıkandı. Eksizyon sonrası kitle açıldı ve germinatif membrane ve kaya suyu tespit edildi(Resim 3) ameliyat sonrası periotta herhangi bir problem görülmedi hasta 10mg/kg gün albendazol 3 ay süreyle verilmek üzere taburcu edildi.Bir yılık bir takip sonrası klinik ve laboratuar ve radyolojik bulguları herhangi bir ekinokok rekürrensini göstermiyrdu.

Sonuç olarak endemik bölgelerde vücudun herhangi bir yerinde kistik kitle ile başvuran hastalarda kist hidatik unutulmamalıdır ve şüpheci bir yaklaşımla biopsi yapılmadan önce kist hidatik açısından radyolojik ve serolojik çalışmalar yapılmalıdır. Kist hidatik tanısı konduğundada kistik kitle drene edilmeden tamamen çıkarılmalıdır.
