LAPAROSCOPIC GASTROSTOMY

 

 

Aim: The aim is to review the results of children who were undergone laparoscopic Nissen fundoplication together with gastrostomy because of gastroesophageal reflux disease and malnutrition.

Patients and Method: A retrospective chart review of 52 patients who were undergone laparoscopic gastrostomy was performed were reviewed. Demographic data of the patients, associated diseases, operation time, duration of feeding postoperatively, duration of hospital stay, presence of intraoperative and postoperative complications and recurrence rates were evaluated.

Results: Twenty-seven of cases were boy and 25 were girl. The median age of patients was 3 years (20days-16years). The median body weight was determined as 11 kg (2-35kg). Ninety-four percent of patients had neurological problem (cerebral palsy (n=19), hypoxic ischemic encephalopathy (n=8), hydrocephalus (n=5), arthrogryposis multiplex (n=2), other (n=15)). U-stitch technique was used for gastrostomy in twenty patients and trocar site technique in the rest thirty-two patients. Mean operative time together with fundoplication was 65 minutes (29-150minutes). Mean feeding time postoperatively was 1day (1-3day). Mean length of hospital stay was 5 days (2-19days). In the first week postoperatively peritonitis occurred in two patients because of dislocation of gastrostomy and they were undergone laparotomy and revision of gastrostomy.

Conclusion: Laparoscopic gastrostomy and simultaneous fundoplication is the gold standard treatment especially in neurologically impaired patients who have malnutrition and need enteral feeding for a long period since the recovery is fast and feeding is early postoperatively. Laparoscopic gastrostomy is a safe procedure that can be preferred in patients when laparoscopic fundoplication is simultaneously required and percutaneous endoscopic gastrostomy cannot be performed. Trocar site gastrostomy is accepted as a safer method than U-stitch technique although their difference is statistically insignificant.  

 

LAPAROSKOPİK GASTROSTOMİ UYGULAMALARI

 

Amaç:Beslenme bozukluğu nedeniyle laparoskopik gastrostomi açılan, beraberinde gastroözofageal reflü nedeniyle Nissen fundoplikasyonu yapılan çocukların sonuçlarının ortaya konması amaçlandı.

Hastalar ve Yöntem:Anabilim dalında laparoskopik gastrostomi açılan 52 hastanın dosyaları geriye dönük olarak gözden geçirildi. Hastaların demografik verileri, yandaş hastalıkları, ameliyat süresi, ameliyat sonrası beslenmeye başlama zamanı, hastanede kalış süreleri, ameliyat sırasında ve sonrasındaki karmaşaları değerlendirildi.

Bulgular:Olguların 27’si erkek, 25’i kızdı. Hastaların ortanca yaşı 3yaş (20gün- 16yaş)’tı. Ortanca vücut ağırlığı 11kg (2- 35kg) olarak saptandı. Nörolojik problemliler (serebral palsi (n=19), hipoksik iskemik ensefalopati (n=8), hidrosefali (n=5), artrogripozis multiplex (n=2), diğer (n=15) %94 oranında idi. Yirmi hastada gastrostomi u- stitch (u dikişi) tekniği ile açılırken geriye kalan otuz iki hastada trokar site (trokar yeri) olarak açıldı. Ortalama ameliyat süresi fundoplikasyonla birlikte 65dk (29-150dk)’ydı. Ortalama ameliyat sonrası beslenmeye başlama zamanı 1gün (1-3gün)’dü. Ortalama hastanede kalış süresi 5gün (2-19gün)’dü. Ameliyat sonrası iki hastada birinci hafta içerisinde gastrostominin yerinden çıkmasına bağlı peritonit gelişmesi üzerine laparotomi ve gastrostomi yenilenmesi yapıldı.  

Sonuç: Malnutrisyon ve uzun dönem enteral beslenme ihtiyacı olan özellikle nörolojik problemli hastalarda kısa hastanede kalış süresi, hızlı beslenmeye başlanması nedeni ile eş zamanlı laparoskopik gastrostomi ve fundoplikasyon açılması altın standarttır. Beraberinde fundoplikasyon yapılması gereken, bu nedenle perkütan endoskopik gastrostomi uygulanamayan olgularda tercih edilebilecek güvenli bir yöntemdir. Trokar yeri gastrostomi, U-dikişi tekniğine göre istatistiksel olarak anlamlı fark saptanmamış olmakla birlikte daha güvenli kabul edilebilir.
