TRANSANAL ENDORECTAL PULL-THROUGH FOR HIRSCHSPRUNG’S DISEASE: EXPERIENCE WITH 44 PATIENTS

 

Aim:Transanal endorectal pull-through (TEPT) is the latest development in treatment of Hirschsprung’s disease (HD). The aim of this study was to evaluate the outcome of TEPT in HD for demographic, clinical finding, diagnosis and results and follow-up after TEPT.  

Patients and methods:A retrospective study of 44 patients who underwent TEPT between June 2001 and December 2010 was conducted. These patients were evaluated with regard to age, sex, preoperative investigations, length of the aganglionic segment, intraoperative and postoperative complications and result of follow-up.

Results: Thirty-eight patients were boys and 6 were girls. The patients’ ages ranged from 14 days to 8 years (mean, 8 months). Sixteen patients were operated in first 2 months of their life. The most common symptoms were abdominal distention and vomiting. Delayed meconium passage (>24h) was found in 32 (72%) patients. The primer diagnosis in 35 patients was barium enema and anorectal manometry finding. Rectal biopsy was performed in only 9 (20%) patients before the TEPT.  Preoperative barium enema and anorectal manometry performed in 43 patients. Barium enema showed transitional zone in 38 (88%) patients. Barium retansiyon, more than 24 hours, was found 39 patients. An anorectal manometry revealed lack of rectoanal inhibitory reflex in 37 (86%) patients. Hirschsprung’s-associated enterocolitis (HAEC) was seen preoperatively in 11 patients. Seven of the patients had a history of previous laparotomy. The average length of resected bowel was 25.5 cm (range, 10 to 50 cm). Two patients required laparotomy because of achieve ganglionic segment.  The mean time of start feeding was 1.8 days. Average follow-up was 26 months. Postoperative complications included transient perianal excoriation in 12 patients, HAEC in 10, anastomotic stricture in 3, encopresis in 3, recurrent constipation in 2, rectal prolapse in 1 and rectovestibuler fistula in 1. Medication was needed more than 2 weeks in 8 patients. 

Conclusions: The transanal endorectal pull- through is a feasible and safe procedure in children with rectosigmoid HD. The clinical outcome is satisfactory. The hazards and morbidities associated with laparotomy and colostomy may be avoided with a TEPT technique in HD confined to the recto-sigmoid region. The outcome has been favorable, but long-term follow-up is necessary for full assessment of those patients.

HİRSCHSPRUNG HASTALIĞINDA TRANSANAL ENDOREKTAL PULL-THROUGH: 44 HASTALIK DENEYİM

 

Amaç: Trananal endorektal  pull-through (TEPT), Hirschsprung hastalığının (HH) tedavisinde ulaşılan son noktadır. Bu çalışmanın amacı HH tanısıyla TEPT yapılan olguların demografik verilerini, klinik bulgularını, tanı şekli ve takip sonuçlarını irdelemektir.

Hastalar ve Metot: Haziran 2001 ile Aralık 2010 tarihleri arasında HH tanısıyla TEPT yapılan olguların bilgileri geriye dönük olarak incelendi. Bu olguların yaşı, cinsiyeti, preoperatif tanı şekli, aganglionik segmentlerinin uzunluğu, intraoperatif ve postoperatif komplikasyonları ve takip sonuçları kaydedildi.

Bulgular: Çalışma grubunda 38 erkek ve 6 kız vardı. Olguların yaş aralığı 14 gün ile 8 yıl arasında değişmekteydi (ortalama, 8 ay). 16 hasta yaşamının ilk 2 ayda opere edildi. En sık başvuru şikâyeti karın şişliği ve kusma idi. Gecikmiş mekonyum çıkışı (>24 saat) 32 (%72) hastada vardı. 35 hastanın tanısında sadece baryumlu kolon grafisi ve anorektal manometri kullanıldı. Yalnızca 9 (%20) hastaya preoperatif rektal biyopsi yapıldı. Preoperatif baryumlu kolon grafisi ve anorektal manometri 43 hastaya yapıldı. Baryum enema ile transizyonel zon 38 (%88) hastada gösterildi. 24 saati geçen baryum retansiyonu 39 hastada vardı. Anorektal manometride 37 (%86) hastada rektoanal inhibitör refleks yoktu. Hirschsprungla ilişkili enterokolit (HİEK) preoperatif dönemde 11 hastada görüldü. Hastaların 7’sinde geçirilmiş laparatomi öyküsü vardı. Ortalama barsak rezeksiyon uzunluğu 25,5 cm idi (10-50 cm). 2 hastada ganglionik segmente ulaşabilmek için laparatomi yapıldı. Beslenme zamanı ortalama 1,8 gündü. Olgulara ortalama 26 ay takip edildi. Postoperatif komplikasyonlar hastaların 12’sinde geçici perianal ekskoriasyon, 10’unda HİEK, 3’ünde anastomoz striktürü, 3’ünde enkoprezis, 2’sinde tekrarlayıcı konstipasyon, 1’inde rektal prolapsus ve 1’inde rektovestibüler fistül  şeklinde idi. 2 haftayı geçen medikasyon 8 olguda görüldü.

Sonuç: Transanal endorektal pull-through rektosigmoid HH’de kullanılan güvenli ve uygulanabilir bir yöntemdir. Klinik sonuçları tatminkardır. Bu teknikte kolostomi ve laparatomi ile ilişkili morbiditeler görülmez. Sonuçlar oldukça tatminkardır, yinede bu olgularda uzun dönem sonuçları takip edilmelidir.
