Case Report of an Ischioheteropagus with Immature Teratoma, Covered Cloacal Exstrophy, Omphalocele, Hemibladder, Hemiuterus, and Intestinal Duplication

 Introduction:Conjoined twin is a very rare pathology live birth. Parasitic twin which is a special type of  conjoined twins is exceptionally rare. We present an unusual case of ischioheteropagus with an immature teratoma, covered cloacal exstrophy, omphalocele, hemibladder, hemiuterus and intestinal duplication.

Case:A 1-day-old baby girl came to our hospital born through a 40-week.   Birth weight of the fetus was 3,150 g including both autosite and parasite, had 2 parasitic limbs (arm and leg) attached to the pelvic girdle of the perineum. A wholly developed labium majus and minus was observed between the right limb and the parasitic leg. Also in the same area, adjacent to the bottom of the labia, an anus was observed opening out as a fistule. Semi-developed labium majus and minus were observed between the left limb and the other parasitic extremity. A covered cloacal extrophy with dimensions as 6x5cm was observed to originate from the undeveloped half of the labium majus and minus. The colon was opening out with an ostium. The baby had a giant hypogastric omphalocele. The omphalocele contained a duplicated small bowel (5 cm in diameter)and large and small bowels, and a fascial defect with approximately 10 cm in diameter. A satellite solid mass with 3 cm in diameter was observed above the umblical cord . Cystic like mass lesion similar with the doppler ultrasonographic images was observed in the magentic resonance imaging with T2 majority starting from the sacrococcygeal level extending to retroperitoneum of intrapelvik region with dimensions of 7,5x7x4,5cm and multiloculated bilobate contours.The tumor (immature Teratoma)were totally excised. The duplication cyst was resectioned and anastomosed. With conserving the vascularisation of the both parasitic extremities, the omphaloselwas repaired with a pediculed flep.

Results: The defect of the abdominal wall has been repaired with fleps have been procured from the tissues of the parasitic fetus. The management of such a case with multiple pathologies should be multidisciplinary. Radiological methods play a major role in discriminating of pathology from normal anatomy of patients with complex anomalies.

İmmatür Teratom, Örtülü Cloacal Ekstrofi, Omfalosel, Hemibladder, Hemiuterus ve İntestinal Duplikasyon ile Birliktelik Gösteren İschioheteropagus Olgu Sunumu

 Giriş:Yapışık ikiz nadir bir patolojidir. Yapışık ikizlerin özel bir şekli olan parazitik ikiz ise çok daha enderdir. Bu sunumda, olgunlaşmamış teratom ile cloacal exstrofi, omphalosel, hemibladder, hemiuterus ve intestinal duplikasyon eşilik eden ischioheteropagus olgusu sunulmuştur.

Olgu: 1 günlük kız bebek hastanemizde 40 hafta geldi. Doğum ağırlığı 3150 gr olup pelvik bölgeye yapışık 2 ekstremiteden (kol ve bacaktan) oluşan bir parazit yapışık ikiz mevcuttu. Sağ bacak ile parazitik ekstermite arasında tamamen gelişmiş bir labium  majus gözlenmiştir. Aynı bölgede, fistülize bir anüs açıklığı mevcuttu. Sol bacak ile parazitik ekstermite arasında tamamen gelişmemiş yarım labium  majus gözlenmiştir. Tamamen gelişmemiş  labium  majus altından başlayan  6x5cm bir kapalı cloacal extrophy gözlendi. Kolon bir açıklık ile dışarıya açılmıştı. Cloacal extrophy bu açıklığı üstünü örtmekteydi. Bebekte 10 cm çapında dev bir hypogastric omfalosel vardı. Omfalosel kesesi içinde 5 cm çapında bir intestinal duplikasyon kisti görülmekteydi. Göbek üstünde 3 cm çapında uydu kitle mevcuttu.Doppler ulturasunografi ve manyetik rezolans görüntüleme sakrokoksigial düzeyden başlayıp retroperitoneal uzanım gösteren pelvis içinede 7,5 x7x4, 5cm boyutlarında ve multiloküle bilobüle kistik kitle gözlendi.Bu tümör (olgunlaşmamış Teratoma) tamamen çıkartıldı. İntestina duplikasyon kisti barsak rezeksiyon anastomos ile çıkartıldı.  Her iki parazit ekstremitelerin vasküleritesi korunarak pediküllü flep oluşturuldu; omfalosel bu flep ile onarıldı.

Sonuç:  Dev omfalosel onarımı için parazitik fetustan ait dokulardan flepler çıkartılarak karın duvarı defekti tamiri yapılmıştır. Olgunun birçok patolojiye sahip olması nedeniyle olgunun tedavi yönetimde multidisipliner yaklaşım olmalıdır. Bu denli yoğun anomalisi olan hastalar normal anatomi ve patolojinin ayırt edilmesinde radyoloji cerrahi tekniğe açısından yol göstericidir.
