FIRST CASE OF BRUCK SYNDROME WITH GASTROSCHISIS

Introduction/Aim:Bruck syndrome is a rare disorder featuring the unusual combination of skeletal changes resembling osteogenesis imperfecta with congenital contractures of large joints. According to our knowledge only 24 patients are reported in the English literature. Although the genotypic and phenotypic features of the Bruck syndrome are heterogeneous, the reported case is the first association of Bruck syndrome with gastroschisis.

Case:A female baby with a history of prenatal diagnosis of gastroschisis was delivered by cesarean section because of fetal distress at 36th gestational week with a birth weight of 1350g from a 19-year-old healthy woman in our hospital. Afterwards, patient was transported to our pediatric surgery department. On physical examination she had gastroschisis complicated with jejunal perforation, and contractures at the elbows, hips, and knees and club foot bilaterally. She had a crepitating, tender swelling on the left femur and babygram revealed a left femur fracture. An emergency operation including resection of the 20 cm necrotic perforated jejunum initiating from ligament of Treitzs and primary anastomosis and reduction of the bowel segments to the abdominal cavity were performed. Postoperative third day she had an additional fracture on the left humerus, both the fractures were followed up with splints.   She tolerated oral feeding within the ninth postoperative day. The presence of the congenital joint contractures in addition to multiple bone fractures promoted the diagnosis of Bruck Syndrome and pamidronat treatment began. After reaching a weight of 2000g she was discharged at the postnatal 30th day. Cyclic pamidronat treatment was given to her in a period of 3 months. She is now 7 months of age, gaining weight without any gastrointestinal complaint, and does not have any additional fractures with the ongoing pamidronat treatment.

Conclusion:Although prematurity and low birth weight is common with gastroschisis, additional system anomalies are uncommon. Conversely association of Bruck syndrome with gastrochisis has not been reported until now, thus the reported case is unique.  

GASTROŞİZİSLİ İLK BRUCK SENDROMU OLGUSU

Giriş/Amaç: Bruck sendromu konjenital büyük eklem kontraktürleri ile osteogenezis imperfektaya benzer iskelet değişikliklerinin birlikte görüldüğü nadir bir hastalıktır. Bizim bilgilerimize göre İngilizce literatürde sadece 24 hasta bildirilmiştir. Bruck sendromunun genotipik ve fenotipik özellikleri heterojen olmakla birlikte, olgumuz Bruck sendromu İle gastroşizisin birlikte olduğu ilk olgudur.

Olgu:19 yaşındaki sağlıklı annenin birinci gebeliğinden, 1350 gr ağırlığında fetal distes nedeniyle,36 haftalık olarak, C/S ile hastanemizde doğan gastroşizisli kız bebek bölümümüze transporte edildi. Fizik muayenesinde gastroşizisi ve herniye barsakta perforasyonu olduğu görüldü. Hastanın her iki dirsek, kalça ve dizde kontraktürleri ve iki taraflı clup foot deformitesi vardı. Sol femuru üzerinde krepitasyon veren, hassas bir şişlik saptanan bebeğin çekilen filminde sol femurda kırık saptandı. Operasyonda Treitz ligamentinden başlayan 20 cm’lik nekrotik ve perfore jejunal ans rezeke edilip, primer anastomoz yapıldı, ardından barsak segmentleri abdominal kaviteye redükte edildi. Postoperatif  3. gün sağ humerusda da kırık saptadı ve heriki kırık atelle takip edildi. Posoperatif 9. gününde oral beslenme başlandı. Hastada konjenital eklem kontraktürlerine ek olarak kemik kırıklarının olması nedeniyle Bruck sendromu tanısı konuldu ve pamidronat tedavisi başlandı. 30 günlükken 2000 gr ağırlığına olaşan hasta şifa ile taburcu edildi. Hastaya üçer aylık periyodlarla pamidronat tedavisi verildi. Hastanın 7 aylık takibinde herhangi bir gastrointestinal yakınması olmadı ve pamidronat tedavisi ile ek kemik fraktürü gelişmedi.

Sonuç:Prematürite ve düşük doğum ağırlığı gastroşizis ile birlikte sık görülse de, ek sistem anomalileri nadirdir. Diğer taraftan Bruck sendromu ile gastroşizis birlikteliği şu ana kadar bildirilmemiştir. Bu nedenle bildirilen olgu benzersizdir.
