Heat Dispersion Comparison of Bipolar Electro-surgical Devices-II: Histopathological Study

Aim: To examine the heat dispersion of bipolar electro-surgical devices by histopathological evaluation of the rat tissues.

Material and Methods: Twelve Wistar-Albino rats were randomly allocated into two Groups (B, L). Following midline vertical laparotomy, bipolar electrocautery in Group B and LigaSure vessel sealing system in Group L were applied on esophagus, liver and small intestine for 5 seconds. The tissues were excised containing the damaged region in the middle and kept for histopathological examination. After fixation and dehydration, the tissues were embedded in paraffin wax and sections with 5 micrometer thickness were collected on slides. The tissues were examined by Olympus BX-50 microscope, and the tissue damage as mm was evaluated in 4X10 magnification with ocular micrometer. Olympus DP-20 camera was used for tissue photographs. The values were presented as mean ± standard deviation and Mann Whithney U test was used statistical analyses. P was significant at < 0.05.

Results: Tissue damage in liver (1.2±0.09 mm vs. 2.37±0.09 mm), small intestine (1.26±0.1 mm vs. 2.62±0.1 mm) and esophagus (0.8±0.2 vs. 2.47±0.1) were significantly lower in Group L (p<0.001, p<0.001, p<0.001).

Discussion: Electrosurgical devices are routinely used in dissection and hemostasis. We compared two bipolar devices with respect to their heat damage on the tissues. Our results indicate that utilization of LigaSure vessel sealing device on liver, small intestine and esophagus is more secure than the bipolar electrocautery on the experimental setting.

 

 

 

Bipolar Elekro-Cerrahi El Aletlerinin Rat Dokularında Isı Yayılım Kıyaslanması-II: Histopatolojik Çalışma

Amaç: Cerrahi disseksiyon ve hemostaz sırasında kullanılan bipolar koter ve LigaSure cihazlarının sıçan dokularındaki ısı hasarının histopatolojik yöntemlerle gösterilerek karşılaştırılması.

Yöntem: Oniki adet dişi erişkin Wistar-Albino sıçan rastgele olarak iki gruba ayrıldı (Grup B, Grup L). B Grubunda bipolar koter ve L Grubunda LigaSure cihazı sırasıyla özofagus, karaciğer ve incebağırsak üzerinde 5 saniye tutuldu. Dokular hasar yeri tam ortaya gelecek şekilde eksize edildi ve histopatolojik inceleme için tesbit edildi. Dehidratasyonu takiben dokular parafin kesitlere gömülerek 5 mikrometre kalınlığında kesitler alındı. Kesitlere hematoksilen-eozin boyası yapılarak Olympus BX-50 mikroskop ile incelendi, doku hasarı her doku için 2 kadrandan mm 4X10 kez büyütmede oküler mikrometresi ile ölçüldü ve Olympus DP-20 kamera ile fotoğrafları çekildi. Değerler ortalama ± standart sapma olarak gösteridi ve Mann Whitney U testi ile değerlendirildi. P < 0,05 anlamlı kabul edildi.

Sonuçlar: Doku hasarı uzunluğu, karaciğer (1,2±0,09 mm vs. 2,37±0,09 mm), incebağırsak (1,26±0,1 mm vs. 2,62±0,1 mm) ve özofagusta (0,8±0,2 vs. 2,47±0,1) L Grubunda anlamlı olarak düşüktü (p<0,001, p<0,001, p<0,001).  

Tartışma: Elektro-cerrahi cihazlar cerrahinin birçok alanında disseksiyon ve hemostaz amaçlı kullanılmaktadır. Çalışmamızda bipolar prensip ile çalışan cihazlar karşılaştırılmıştır. LigaSure cihazının karaciğer, ince bağırsak ve özofagusta daha az doku hasarı yapıcı etkisi olması bu organların cerrahisinde bipolar kotere göre daha güvenli kullanılabileceğini göstermektedir.
