Our 10 Year Experience With Urethra Ruptures: Clinical Series Consisting Of 12 Cases

Aims:To investigate the patients monitored and treated with Urethral Rupture (UR) and evaluate the treatment results as well as the factors contributing to it.

Methods:We retrospectively investigated records of patients that received treatment at our clinic for UR between 2001 and 2011. The patients were evaluated in terms of age, gender, cause of injury, type of illness, accompanying pathology, treatment type, length of stay, complications and mortality.

 

Results:12 patients with a diagnosis of UR were admitted to our clinic within the 10-year period. The mean age of patients was 91 months (ranging from1 to 156 months). All of the patients were males. UR was caused by a traffic accident in 8 patients (61%), falling off a great height in 2 patients (15%), gunshot in 1 patient (7.6%) and in 1 patient where complication developed after a surgery at an external center (7.6%). All of the patients were evaluated with retrograde urethrogramThe complete to incomplete rupture ratio was 4/8. An insertion of a foley catheter was attempted in three complete rupture patients during the time of admission at the external center. Isolated UR was present in the 2 patients who fell off a great height.  Treatment included insertion of a suprapubic urinary diversion with cystostomy  catheter and observation in 3 patients (25%), urgent open urethroplasty in 4 patients (33%) and early urethral realignment as well as insertion of a cystostomy  catheter catheter in 5 patients (41%). The mean length of stay was 26 days (ranging from 10 to 42 days). Perioperative exitus in one of the patients was due to hypovolemia. Postoperative follow-up was completed via sistoscopy and urethrography work up. After a 2-year follow-up period, 4 of the patients (33%) presented with no problems, 6 (50%) of the patients presented with urethral stenosis, 1 patient (8.3%) with perineal fistule and 1 patient (8.3%) with testicle necrosis. Permanent incontinence developed in 2 of the patients among the ones with complete rupture.

 

Discussion:  There is a high complication risk even with the most ideal treatment approaches for UR. In our series, a complication occurred in 66% of the patients, stricture being the most common one. Whereas  suprapubic urinary diversion with cystostomy  catheter  forms the most important step of the treatment, timing of operation should be determined through an evaluation of causes for complete/incomplete rupture accompanied by pelvic fracture and hematoma.   

10 Yıllık Üretra Rüptürleri Deneyimimiz: 12 Vakalık Klinik Seri

Amaç:Üretra rüptürü (ÜR) nedeni ile takip ve tedavi edilen hastaları incelemek ve tedavi sonuçlarını ve etki eden faktörleri değerlendirmek.

Metod:  2001-2011 yılları arasında üretra rüptürü (ÜR) nedeni ile kliniğimizde tedavi gören  hastaların kayıtları geriye dönük olarak incelendi. Hastalar yaş, cinisyet, oluş nedeni, hastalığın tipi, eşlik eden patoloji, tedavi şekli, yatış süresi, komplikasyonlar ve mortalite yönü ile araştırıldı.

Sonuçlar:10 yıllık süre içerisine 12 hasta kliniğimize ÜR tanısı ile yatırıldı. Hastaların yaş ortalaması 91 ay (1-156) idi. Hastaların  tamamı (%100) erkekti. ÜR 8 hastada(%61) motorlu araç  kazası, 2 hastada (%15) yüksekten düşme, 1 hastada (%7.6) ateşli silah yaralanması, 1 hastada (%7.6) dış merkezde  cerrahi sonrası gelişen komplikasyona bağlıydı. Hastaların tamamı retrograd üretrografi ile değerlendirildi. Komplet, inkomplet rüptür oranı 4/8 idi. Komplet rüptür gelişen 3 (%25) hastada dış merkezde müracat anında foley sonda takılmaya çalışılmıştı. Yüksekten düşen  2 hastada  izole ÜR mevcuttu. Tedavi; 3hastada (%25) sadece suprapubik  sistostomi tatbiki ve   izlem şeklindeyken 4’ünde(%33)  acil açık üretroplasti  ve 5’inde (%41) erken üretral realignment beraberinde suprapubik sistostomi kateter tatbiki yapıldı. Ortalama yatış süresi 26 gün (10-42) dü. Bir hasta hipovolemi nedeni ile peroperatif exitus oldu. Postoperatif izlem sistoskopi, üretgrafi tetkikleri ile yapıldı.  2 yıllık takip süresi sonunda 4 hasta (%33) sorunsuz, 6 hastada (%50) üretra darlığı, 1 hastada (%8.3) perineal fistül, 1 hastada (%8.3) testis nekrozu belirlendi. Komplet rüptür olan hastalardan 2’sinde kalıcı inkontinans oluşmuştu.

 

Tartışma:ÜR’leri ideal tedavi yaklaşımları ile bile yüksek komplikasyon gelişmektedir. Serimizde %66 hastada komplikasyon gelişirken en sık  sitriktür tespit edilmiştir. Sistofix kateter  tedavinin en önemli aşamasını oluştururken rüptürün komplet/inkomplet olması eşlik eden pelvik fraktür ve hematom  olması gibi nedenler değerlendirilerek operasyon zamanlaması belirlenmelidir.
