ASSESMENT OF THE EFFECTIVENESS OF N-ACETYLCYSTEINE WITH HISTOPATHOLOGICAL AND BIOCHEMICAL PARAMETERS AT THE EARLY AND LATE PERIODS OF OESOPHAGEAL CORROSIVE BURNS

 

Aim:In this study, our aim was to show the earlier and late histopathological changes, effects on glutathione (GSH) level and oxidative stress of N- acetylcysteine (NAC) – which is an antioxidant- on the rats who had corroded esophagus.

Material/Method: 49 female Wistar Albino rats were used. Modified Gehanno and Guedon models were used to make corroded esophagus. Rats were divided into 3 main groups. Group 1: SHAM, group 2: esophagitis with no treatment, group 3: esophagitis with the therapy of NAC. Each group was divided into 2 subgroups (a) and (b). In the group with therapy, NAC was given after 2 hours of corrosion and the treatment was given to both subgroups for 10 days. Group (a) were sacrificed on day 11, group (b) were sacrificed on day 21. Esophageal specimen and blood samples which were collected from those rats, were studied histopathologically and biochemically. MDA, which is an outcome of lipid peroxidation, was studied on tissue samples and plasma. GSH, which is an antioxidant molecule, was studied on tissue and blood samples.

Findings:Histopathological studies show that the rats, which were sacrificed on the 11th day of the caustic damage, the damage was milder in those treated with NAC than those with esophagitis. In those rats which NAC was given metabolite levels were lower than those without treatment but glutathione levels were higher. The levels were not divergent in those collected on the 21st day.

Discussion: In this experimental study, significant effects of NAC treatment were shown histopathologically. Studies with different dosages and the time of NAC treatment are necessary. NAC can be a promising treatment in corroded esophagus cases.

KOROZİF ÖZOFAGUS YANIKLARINDA N-ASETİL SİSTEİN ETKİNLİĞİNİN ERKEN VE GEÇ DÖNEMDE HİSTOPATOLOJİK VE BİYOKİMYASAL DEĞİŞİKLİKLERLE DEĞERLENDİRİLMESİ

 

Amaç:Korozif madde yutulmasına bağlı gelişen özofajitlerin patogenezinde oksidatif stresin önemli rol oynadığı ileri sürülmektedir. Bu çalışmada, bir antioksidan olan N-asetil sisteinin (NAC) korozif özofagus yanık modeli oluşturulan ratlarda, erken ve geç dönemde meydana getirdiği histopatolojik değişiklikler ile glutatyon (GSH) düzeyi ve oksidatif stres üzerine etkilerinin araştırılması amaçlanmıştır.

Gereç ve Yöntem:Bu çalışmada 49 adet dişi Wistar Albino rat kullanılmıştır. Özofajit modeli Gehanno ve Guedon modelinin modifikasyonu örnek alınarak oluşturuldu. Çalışmada kullanılan ratlar 3 ana gruba ayrıldı: Grup I; SHAM, grup II; özofajit oluşturulup tedavi verilmeyen, grup III; özofajit oluşturulduktan sonra NAC tedavisi verilen rat grupları olarak belirlendi. Her grup kendi içinde (a) ve (b) olmak üzere iki alt gruba ayrıldı. Tedavi gruplarında NAC tedavisine yanıktan 2 saat sonra başlandı ve her iki alt grupta 10 gün boyunca tedaviye devam edildi. (a) grupları 11. gün, (b) grupları 21. gün sakrifiye edildi. Ratlardan alınan tam kan örneği ile distal özofagus spesmenleri histopatolojik ve biyokimyasal olarak değerlendirildi. Lipit peroksidasyon ürünü olan malondialdehit (MDA) düzeyleri doku ve plazmadan, antioksidan molekül olan glutatyon (GSH) düzeyleri doku ve tam kandan çalışıldı.

Bulgular: Kostik hasarın ardından 11. gün sakrifiye edilen ratlarda yapılan histopatolojik değerlendirmede, NAC tedavisi verilen ratlardaki hasarlanma özofajit oluşturulan ratlara göre daha hafif değerlendirilmiştir. NAC tedavisi verilen ratlarda metabolit düzeyleri özofajit oluşturulup tedavi verilmeyen ratlara göre düşükken, glutatyon düzeyleri daha yüksek ölçülmüştür. Yirmibirinci gün bakılan düzeylerinde gruplar arasında fark bulunamamıştır.

Tartışma: NAC tedavisinin korozif özofagus hasarında kullanıldığı süre içerisinde anlamlı etkisi olduğu histopatolojik olarak gösterilmiştir. Farklı dozlarda ve daha uzun sürelerde NAC uygulamasının kostik özofajit modellerinde etkilerini gösteren yeni çalışmalar gerekmektedir ve NAC korozif özofajit tedavisinde umut verici bir ilaç olabilir.
