Spigelian Hernia accompanying Cryptorchidism: Cause or Consequence? Unidentified syndrome? Review of Literature with 4 cases

Introduction/Aim:

Spigelian hernia is rare in childhood. Fifty case have been reported to date. Among the boys with spigelian hernia 75 % have also criptorchidism. Four cases with both spigelian hernia and criptorchidism hes been reviewed.

Material/Method : Clinical characteristics of operated cases in which both spigelian hernia and criptorchidism present between years of 2002-2010  were examined retrospectively. 

Findings:Individuals were 6 months-, 1 years-, 2years- and 5 years-old. Patients were reported to have a lateral abdominal swelling congenitally. Two cases admitted to hospital for swelling and the other two for criptorchidism. Hernia was at right in two case, and at left in the other two cases.  Patients were operated for both spigelian hernia and criptorchidism. In all cases testes were in hernial sac, gubernacula were not seen and there was not inguinal ducts. Hernia repair and orchypexia was applied. 

Results: Also known as Lateral ventral hernia, spigelian hernia develops throughout semilunar line which lie from cartilage of 9th rib to pubic tubercle on aponuerosis of transversal abdominal muscle. In adults, it results from inraabdominal pressure and trauma while it is congenital in children. In children, high frequency of spigelian hernia  simultaneous with criptorchidism the question if criptorchidism is a cause or consequence, or a syndrome of an unidentified simultaneousness is in discussion. As in the cases reported here, due to the absence of gubernaculum inguinal ducts and also because testes were in the hernial sac, the results supports the idea which indicates that they are a simultaneous syndrome.

 

Spigelian Herni ile İnmemiş Testis Birlikteliği: Sebep mi? Sonuç mu? Tanımlanmayan Sendrom mu? 4 Olgu ile Birlikte Literatürün Gözden Geçirilmesi

Giriş /Amaç

Spigelian herni çocuk yaş grubunda nadirdir.  Şimdiye kadar 50 tan az çocuk olgu rapor edilmiştir. Spigelian herni saptanan erkek çocukların %75’inde inmemiş testis birlikteliği rapor edilmiştir. Spigelian fıtığa inmemiş testisin eşlik ettiği 4 çocuk olgu literatur eşliğinde değerlendirildi.

Gereç/Yöntem : 2002-2010 yılları arasında, inmemiş testisin eşlik ettiği  spigelian fıtık nedeniyle opere edilen  çocuk olguların klinik özellikleri geriye dönük incelendi. 

Bulgular:Hastalar 6 ay, 1 yaş, 2 yaş ve 5 yaşlarındaydı. Hastalarda karın yan taraf şişliklerinin doğuştan olduğu öğrenildi. 2 olgu şişlik, diğer iki olgu da inmemiş testis şikâyeti ile başvurdu. 2 olguda fıtık sağ tarafta, 2 olguda sol taraftaydı. Hastalar spigelian fıtık ve inmemiş testis nedeniyle opere edildi. Bütün olgularda testis fıtık kesesi içindeydi, gubernakulumları görülmedi ve inguinal kanalları yoktu. Olgulara fıtık onarımı ve orşipeksi uygulandı. 

Sonuç: Lateral ventral fıtık olarak ta adlandırılan spigelian fıtık 9.kaburga kıkırdağından pubik çıkıntıya kadar uzanan semilunar çizgi boyunca transvers karın kasının aponörozunda gelişen bir fıtıktır.Erişkinlerde intraabdominal basınç artışı ve travma gibi nedenlerle oluşan herni çocuklarda genellikle konjenitaldir. Çocuklarda inmemiş testis ile SH birlikteliğinin yüksek oranda olması nedeniyle, inmemiş testisin sebep mi yoksa sonuç mu olduğu veya bu birliktelik tanımlanmayan bir sendrom olabileceği tartışılmaktadır. Olgularımızda gubernekulum ve ingüinal kanalının olmaması, testisin kesenin içinde olması, bu birlikteliğin bir sendrom olabileceği yönündeki tezi desteklediğini düşünmekteyiz.
