EFFECTS ON WOUND HEALING OF MESENCHYMAL STEM CELLS IN RATS WITH EXPERIMENTALLY INDUCED ESOPHAGEAL CORROSIVE BURNS

AIM:We aimed to evaluate  the effects of the mesenchymal stem cells (MSC) in rats with experimentally induced corrosive esophageal burns with application of several routes.

MATERIAL AND METHODS:Fifty Sprague Dawley rats weighing 228-260 g (mean 244 g) were randomly divided into five equal groups. Group S (sham) underwent only laparotomy. In the remaining  groups a  caustic esophageal burn has beengenerated by  10% sodium hydroxide. Group C (control) received no further medication. The treatment groups, received MSC via intravenous (Group IV), intralesional (Group L) and omental niche (Group ON) routes. After 21 days, the removed distal parts of the esophageal have been evaluated according to stricture formation, microscopic stenosis index, histopathologic score, Vascular Endothelial Growth Factor (VEGF) and Fibroblast Growth Factor (FGF) tissue expression, as well as tissue hydroxyproline levels were studied biochemically.

RESULTS:At the end of the study, the rats in treatment groups did not have any weight loss. In Group IL  and in Group ON no stenosis were observed,  while all rats in  the control group,  75% of those in Group IV had stenosis. In Group ON and Group IL, mean histopathological scores (3 and 2) and mean hydroxyproline (HP) levels (7,02 and 4,14) were significantly lower than the control group (HP=32, score=5) (p <0.05). The VEGF expression was not significant between MSC groups and  control group  (p> 0.05). In the expression of FGF was no significant difference between sham group and Group IL (p> 0.05) was found.

CONCLUSION:Application of MSC may prevent stenosis after experimentally induced corrosive burn of esophagus. Local application of MSC seems more effective than the systemic use.

RATLARDA DENEYSEL OLARAK OLUŞTURULAN ÖZOFAGUS KOROZİV YANIKLARINDA MEZENKİMAL KÖK HÜCRE UYGULAMASININ YARA İYİLEŞMESİ ÜZERİNE ETKİLERİ

AMAÇ:  Bu çalışmada ratlarda deneysel olarak özofagusta oluşturulan koroziv yanık sonrası değişik yollardan uygulanan mezenkimal kök hücrelerin  (MKH) yara iyileşmesi üzerine etkilerini değerlendirmeyi amaçladık.

GEREÇ VE YÖNTEM:Ağırlıkları 228-260 g (ortalama 244 g) 50 adet Spraque Dawley türü dişi rat beş eşit gruba ayrıldı. Grup S’de (Sham) sadece laparatomi yapıldı. Diğer gruplara % 10’luk sodyum hidroksitle özofagusta yanık oluşturuldu. Grup K’da (Kontrol) başka medikasyon verilmedi. Diğer üç grupta; intravenöz (Grup İV), intralezyonel (Grup İL) ve omental niş (Grup ON) yoluyla MKH uygulandı. Yirmibir gün sonra distal  özofagusta makroskobik darlık, mikroskopik stenoz  indeksi,  histopatolojik skor,  Vasküler Endotelyal Büyüme Faktörü (VEGF) ve Fibroblast Büyüme Faktörü (FGF) ekspresyonu ile özofagus dokusunda hidroksiprolin  (HP) düzeyleri incelendi.

BULGULAR:Çalışma sonunda, tedavi uygulanan gruplarda vücut ağırlığı kaybı yoktu. Kontrol grubunda anlamlı tartı kaybı vardı (249g vs 236g). Darlık oranları, kontrol grubunda % 100, Grup İV’de % 75 iken, Grup İL ve Grup ON’de hiçbir ratta darlık gözlenmedi. Lokal MKH uygulanan Grup İL ve Grup ON grupların ortalama histopatolojik skorları (3 ve 2) ve HP (7,02 ve 4,14) düzeyleri kontrol grubundan (HP=32, skor=5) anlamlı olarak düşüktü ( (p<0,05). Kontrol Grubu ve MKH uygulanan gruplar arasında  VEGF ekspresyonun değerlendirilmesinde anlamlı bir fark tespit edilmedi (p>0,05). FGF ekspresyonu açısından Sham grubu ile Grup İL arasında anlamlı fark yoktu (p>0,05).

SONUÇ:MKH uygulamasının, alkali özofagus yanığı sonrası ortaya çıkan özofagus darlığı gelişimini engelleyebilmektedir. Lokal MKH uygulaması etkinliği, sistemik  MKH uygulamasından  daha fazla gibi gözükmektedir.
