Familial Cystic Nephroma Treated with Partial Nephrectomy: a case report

Introduction:

Cystic nephroma (CN), which is also called “multiocular cystic nephroma”, is an uncommon benign cystic neoplasia of the kidney. The reported familial cases associated with CN are extremely rare. A case of a familial CN treated with partial nephrectomy is reported in this article. 

Case:

Abdominal tomography of a 24-month old boy, who was admitted to our hospital complaining of hematuria, indicated a 3x3.5x3.5 cm sized multiple loculated cystic mass originating from inferior pole of the left kidney which was lobulated with septations that had contrast enhancement. Medical history of the patient pointed out that his elder sister was also treated because of cystic nephroma and neuroblastoma. We justified a surgical exploration with the pre-diagnosis of cystic nephroma. The intraoperative frozen section biopsy did not reveal any malignant cell, and we performed heminephrectomy by ligating the inferior branches of the renal vasculature. Postoperative course was uneventful and the patient was discharged on postoperative 5thday. Intravenous pyelogram (IVP) of the patient at postoperative 2ndmonth indicated the residual left kidney was functioning normally. During a 6 month period ultrasonographic follow-up did not demonstrate any recurrence or residual mass.

Dicussion

CN is a benign cystic multiloculated kidney mass. The criteria for the diagnosis of CN suggested by Powel were first revised by Boggs and Kimmelstiel then later Joshi and Beckwith revised. According to these criteria; 1)The lesion is composed of cysts completely different in size and septations dividing them, 2)Cystic mass is clearly distinguished from the kidney parenchyma, 3)The solid component of the tumor is only composed of septations, 4)The septations are consist of fibrous tissue and may include well differentiated renal tubules.   

CN is classically known as a non-familial disease, but a small number of familial cases are reported in the literature. When the medical history of our patient is evaluated, we learned that right nephrectomy and excision of a concomitant surrenal mass were performed on the patient’s elder sister 10 years ago when she was 1 years old, and her renal mass was reported as CN, and the surrenal mass as neuroblastoma. She had an uneventful follow-up. 

Conclusion:

Nephron protective surgery should be preferred for the treatment of CN which is a benign disease, if feasible technically. Due to the presence of familial cases, the siblings may be evaluated with ultrasonography.

 

Parsiyel Nefrektomi ile Tedavi Edilen Familyal Kistik Nefroma: Olgu sunumu

Giriş:

Kistik nefroma (CN) böbreğin nadir görülen benign kistik neoplazisidir. Multiloküler kistik nefroma olarak da adlandırılır. CN ile ilgili bildirilen ailesel vakaların sayısı çok azdır. Bu makalede parsiyel nefrektomi ile tedavi edilen ailesel bir CN vakası sunulmuştur.  

Vaka:

Kanlı idrar yapma nedeniyle başvuran 24 aylık erkek hastanın tomografisi sol böbrek alt polünden köken alan yaklaşık 3 x 3,5 x 3,5 cm boyutlarında lobüle şekilli, septasyonlar içeren, septalarında kontrast tutulumunun izlendiği, multipl loküle kistik kitle lezyonu mevcuttur olarak rapor edildi. Hastanın hikayesinden  ablasının da kistik nefroma+nöroblastom nedeniyle tedavi edildiği öğrenildi. Kistik nefroma ön tanısıyla operasyon planlandı Operasyon sırasında gönderilen frozen biyopside malign hücre görülmemesi üzerine alt kutba giden damarlar bağlanarak heminefrektomi uygulandı. Postoperatif sorunu olmayan hasta 5. Gün taburcu edildi. Postoperatif 2. ayda çekilen IVP de böbreğin normal fonksiyon gösterdiği görüldü. 6 ay boyunca yapılan USG takiplerinde nüks ya da rezidü kitle görülmedi.

Tartışma

CN benign kistik multiloküle börek kitlesidir. CN tanısı için Powel tarafından öne sürülen kriterler önce Boggs ve Kimmelstiel tarafından daha sonra ise Joshi ve Beckwith tarafından revize edilmiştir. Bu kriterlere göre;  1-lezyon tamamiyle farklı boyutlardaki kistlerden ve bunları ayıran septalardan oluşur, 2- Kistik kitle normal böbrek parankiminden belirgin bir şekilde ayrılmıştır, 3- Tümörün tek solid komponenti septalar tarafından oluşturulur, 4- Kistler basık, küboidal epitelle döşelidir, 5- Septalar fibröz dokudan oluşur ve septalar iyi diferensiye renal tübüller içerebilir.

CN ile ilgili klasik bilgi hastalığın non-familyal olduğu yönündedir fakat çok az ayıda da olsa familyal vakalar literatürde bildirilmiştir. Hastamızın hikayesi incelendiğinde, hastanın kız kardeşine 10 yıl önce, 1 yaşındayken böbrek kitlesi nedeniyle sağ nefrektomi uygulandığı, aynı zamanda sürrenal lojda bulunan ayrı bir kitleninde eksize edildiği öğrenildi. Hastanın böbrekteki kitlesi CN, diğer kitleside nöroblastom olarak rapor edilmiş. Hastanın takiplerinde sorun olmamış.

Sonuç:

Benign bir hastalık olan CN da teknik olarak mümkün olan durumlarda nefron koruyucu cerrahi tercih edilmelidir. Ailesel vakaların varlığı nedeniyle hasta çocukların kardeşlerinin ultrasonografi ile değerlendirilmesi düşünülebilir.
