Colostomy in Children

 

Aim:In this study; mortality, morbidity and risk factors were investigated in children with colostomy.

Methods: A total of 96 patients with colostomy were retrospectively analyzed between 1996-2012 in our clinic.

Results:57 patients (60%) were male, and 39 (40%) were female. The major indications for colostomy were anorectal malformations (51 patients), Hirschsprung's disease, (23 patients), trauma (6 patients), volvulus (4 patients) and 12 other diseases. The age of cases varied between one day and 13 years. 52 of them (54%) were smaller than one month, 24 were (25%) in the range of one month and one years, 20 were (19%) higher than one year. The types of colostomy in 53 cases(55%) were separated and modified separated, 27 (28%) were loop and 16 (16%) were Hartmann, respectively. The side of colostomy was sigmoid in 61 (63%) cases, transverse colon in 29 (30%) and caecum in 6 (6%) patients. 38 complications occurred in 35 (36%) patients, postoperatively. Complications of peristomal dermatitis 21 (55%), mucosal bleeding 4 (10%), prolapse 4 (10%), stomal stenosis and/or obstruction 5 (12.5%), evisceration 3 (7%), and stomal ischemia 1 (3%) patients were observed. A stomal revision was required for 7 patients. Overall, 10 patients died whilen one of them had releated without colostomy.

Conclusions: Major congenital abnormalities were the most common cause of death in neonates with colostomy. Congenital anomalies are the first rank among diseases in children requiring colostomy and the most common cause of mortality. Accordance with the increased experience in the prevention of complications of colostomy nursing care services, improvement of care and stomal care are important parameters. Short time of colostomy closure will reduce the potential complications.

Çocuklarda kolostomi

 

Amaç: Bu çalışmada kolostomili çocuklarda mortalite, morbidite ve risk faktörleri araştırıldı.

Metot: Kliniğimizde 1996-2012 yılları arasında kolostomi yapılan 96 hasta geriye dönük olarak analiz edildi.

Bulgular: Hastaların57’si (% 60)erkek, 39’u(%40)kız idi. Kolostomi endikasyonu anorektal malformasyonlar 51(%53), Hirschsprung hastalığı 23(%24), travma 6, volvulus 4 ve 12 olguda diğer hastalıklardı. Yaşları bir gün ile 13 yaş arasındadeğişen olguların 52’si(%54) bir aydan küçük, 24’ü(%25) bir ay-bir yaş aralığında, 20’si(%19) ise bir yaşından büyüktü. Kolostomi tipleri 53’ünde (%55) separe ve modifiye separe, 27’sinde (%28)loop, 16’sında (%16) ise Hartman şeklindeydi. Kolostominin yeri 61(%63) olguda sigmoid, 29(%30) olguda transvers kolon, 6(%6) olguda çekumda idi. Postop 35 (%36)olguda 38komplikasyon görüldü. Komplikasyonlardan peristomal dermatit 21(%55), stomal kanama 4(%10), prolapsus 4(%10), stomal stenoz veyaobstrüksiyon 5(%12.5), evisserasyon 3(%7), ve stomal beslenme bozukluğu 1(%3) hastada görüldü. Revizyon 7(%7) hastaya gerekli oldu. Kolostomili olguların 10’u exitus oldu, hiç birinde primer neden kolostomi değildi. Hepsi yenidoğan döneminde olan ölümlerin en sık nedeni majör konjenital anomalilerdi.

Sonuç: Çocuklarda kolostomi gerektiren hastalıklar arasında konjenital anomaliler ilk sıradadır ve mortaliteninde  en sık nedenidir. Kolostomi komplikasyonlarının önlenmesinde deneyimin artması ile uygun kolostomi açılması ve hemşirelik bakım hizmetleri ile stomal bakımın artması önemli tedbirlerdir. Kolostomi kapama süresinin kısa tutulması olası komplikasyonları azaltacaktır.
