Indirect inguinal hernia repair with the Mitchell Banks technique in children; 15-year experience

 

Aim: The aim of this study was to compare the Mitchell Banks technique and the classic high ligation technique used in pediatric hernias of all age groups.

Material and Method: The charts and outpatients records of children aged 0-16 years who had been treated surgically for indirect inguinal hernia between January 1997 and June 2012 at the Aydın and Denizli State Hospitals, Pediatric Surgery Departments were retrospectively evaluated. Male patients over 1 year of age who had been operated for inguinal hernia were divided into 2 groups as those who had undergone high ligation without external oblique fascia opening (Mitchell Banks technique) and with external oblique fascia opening. The patients were called for follow up 1 week, 1 month, 3 months and 6 months after the surgery. Follow ups continued every 6 months for 2 years. The patients' demographic features and postoperative early and late phase complications were evaluated.

Results: Of the 8513 patients operated on for inguinal hernia, 3509 were excluded as they were under 1 year of age and 1744 because they were female. Of the remaining 3995 patients, 2388 (59.7%) had undergone the Mitchell Banks technique and 1607 (40.3%) the classic high ligation technique. The median age and age range of the patients were 5.1± 0.19 (1-12) years in the first group and 4.6± 1.15 (1-16) years in the second group. The early and late phase complication rates in the two groups was 2.3% and 2.9% in the first and second group respectively. The most common early stage complications were scrotal edema and hematoma in both groups while the least common complication was the late stage complication of iatrogenic undescended testis.

Result: We believe that this technique that is routinely used for children under one year of age can also be safely used in older children. 

Çocuklarda Mitchell Banks tekniği ile indirekt inguinal herni onarımı-15 yıllık deneyim

 

Amaç:  Bu çalışmanın amacı tüm yaş gurubundaki pediatrik hernilerde kullanılan Mitchell Banks tekniği ile klasik yüksek ligasyon tekniklerinin karşılaştırılmasıdır.

Materyal ve Metod: Ocak 1997 ve Haziran 2012 tarihleri arasında Aydın ve Denizli Devlet Hastanesi Çocuk Cerrahisi Kliniklerinde, cerrahi olarak tedavi edilen ve indirekt inguinal hernisi olan 0–16 yaş arası çocukların dosyaları ve poliklinik kayıtları retrospektif olarak incelendi. İnguinal herni nedeniyle opere edilen 1 yaşından büyük erkek hastalar, ekstarnal oblik fasya açılmadan (Mitchell Banks tekniği) ve ekstarnal oblik fasya açılarak yüksek ligasyon uygulananlar olmak üzere 2 gruba ayrıldı. Hastalar operasyondan sonra 1.hafta, 1. ay, 3. ay ve 6. ayda kontrole çağrıldı. Daha sonra kontroller 6 ay aralarla 2 yıl boyunca devam etti.  Hastaların demografik özellikleri ile postoperatif erken ve geç dönem komplikasyonları incelendi.  

Sonuçlar:  İnguinal herni nedeniyle opere edilen 8513 hastanın 3509’u 1 yaşın altında, 1744’üde kız olduğu için çalışmaya dahil edilmedi. Geriye kalan 3995 hastanın 2388’i (%59.7) Mitchell Banks tekniği ile, 1607’si ise (%40.3) klasik yüksek ligasyon tekniği ile opere edildi. Hastaların ortanca yaşı ve yaş aralığı sırasıyla birinci grupta 5.1± 0.19 yıl (1-12) ikinci grupta ise 4.6± 1.15 yıl (1-16) idi. Her iki grupta da erken ve geç dönem komplikasyon görülme oranları sırasıyla birinci grupta %2.3, ikinci grupta %2.9 olarak saptandı. Her iki grupta da en sık gözlenen erken dönem komplikasyonlar skrotal ödem ve hematom iken en az görülen komplikasyonlar ise bir geç dönem komplikasyonu olan iatrojenik inmemiş testisti.  

Sonuç: Bir yaş altı çocuklarda uygulanması rutin olan bu tekniğin büyük çocuklarda da emniyetle kullanılabileceğini düşünmekteyiz.
