Comparison the efficiency of Ligasure with monopolar devices for division the mesoappendix in laparoscopic appendectomy

 

Background and aim: Various methods are present in division of mesoappendix in laparoscopic appendectomy. In our study we compared the efficiency of monopolar devices with electrothermal bipolar vessel sealing system (Ligasure) in division of mesoappendix.

Patient and methods: The medical records of 168 consecutive children between March 2011 and June 2012 were retrospectively investigated whom were appendectomized by three-trocar technique. Mesoappendix were divided in first group by Ligasure and by monopolar devices (Monopolar scissors and Maryland dissector) ın second group. Patient demographics, appendicitis forms, appendix localizations, operation time, early and late complications, peritoneal drenage rates, and conversion to open surgery rates were evaluated and differences between the two groups were statistically investigated.

Results: Mean age was 10.58 (4-16 years). Mesoappendix was divided in 80 children by Ligasure and by monopolar devices in 88 children. Pathologies were phlegmonous, gangrenous, perforated and plastron appendicitis; 97, 23, 38, 2 respectively. Appendix localizations were normal, retrocecal subserous and retrocecal subserous and subhepatic, 138, 18 and 12 respectively. 4 children converted to open surgery. Early complications did not occurred in both groups while 2 abdominal abscesses, 1 ileus and trocar site infection occurred in 4 children.There were no statistically differences between the two groups regarding the early and late complications, conversion to open surgery, operation time and peritoneal drainage.

Conclusion: Ligasure and ultrasonic cutters were known to be safe and fast devices for division of mesoappendix in laparoscopic appendectomy but they increase the costs. Similar periods of operation time and safety can be accomplished by increased laparoscopy experience.

 

Laparoskopik apendektomide mezoapendiksin serbestlenmesinde Ligasure ve monopolar aletlerin etkinliklerinin değerlendirilmesi

 

Önbilgi ve Amaç: Laparoskopik apendektomide mezoapendiksi serbestlemek için birden fazla yöntem vardır. Çalışmamızda mezoapendiksi serbestlemede monopolar aletler ile elektrotermal bipolar damar mühürleme cihazı (Ligasure) etkinliği karşılaştırıldı.

Hasta ve Yöntemler: Mart 2011 ile Haziran 2012 arası 3 trokar yöntemi ile appendektomi yapılan 168 (100 erkek, 68 kız) çocuğun dosya bilgileri incelendi. Mezoapendiks birinci grupta Ligasure ile ikinci grupta ise monopolar aletler (Monopolar makas, Maryland disektor) ile serbestlendi. Çalışmada olguların demografik bilgileri, cerrahi bulguları, apendiksin lokalizasyonu, operasyon süreleri, erken ve geç komplikasyonlar, periton drenajı, açığa cerrahiye geçme oranları değerlendirildi ve gruplar arasında farklılıklar araştırıldı.

Bulgular: Olgularda yaş ortalaması 10.58 (4-16 yıl). Mezoapendiks 80 çocukta ligasure ile 88 çocukta ise monopolar  aletler ile serbestlendi. 97 flegmonöz, 23 gangrenöz, 38 perfore, 2 plastrone apandisit, 8 olguda ise apendiküler patoloji saptanmadı. 138 olguda apendiks doğal yerleşimde, 18 retroçekal subseröz, 12 retroçekal subseröz subhepatik yerleşimli idi. 4 olguda açık cerrahiye geçildi. Gruplarda erken dönemde komplikasyon olmaz iken 2 çocukta karın içi apse, 1 inde ileus, 1 inde trokar yerinde enfeksiyon görüldü. Her iki grupta erken ve geç cerrahi komplikasyon gelişimi, açık cerrahiye geçme oranları, cerrahi süre, periton drenaj gereksinimi açısından istatiksel anlamlılık saptanmadı.

Sonuç: Laparoskopik apendektomide mezoapendiksin serbestlenmesinde ligasure ve ultrasonik kesici aletler cerrahi süreyi kısalttığı ve güvenilirliği  arttırdığı bilinmekle beraber en önemli dezavantajları artan maliyet oranlarıdır. Bununla beraber artan laparoskopi deneyimi ile monopolar aletlerle de benzer cerrahi süre ve güvenlik sağlanabilir.
