COLONOSCOPY ASSISTED RECTAL ATRESIA REPAIR: CASE REPORT

 

          Introduction:Rectal atresia (RA) is a rare type of anorectal malformation (ARM) and constituting % 1–2 of the ARM. RA is characterized by the presence of the proximal rectum, which ends at the or above the pubococcygeal line, and a well-formed distal anus located in its normal location, which is about 1–3 cm in depth. We want to share our experience with colonoscopy assisted minimal invasive method of rectal atresia repair by transanal approach.

          Case: 2 years old male patient, evaluated post-natal 4th day for the absence defecation and abdominal distension. On physical examination, systemic examination was normal. During anal examination, a normally positioned anus and an approximately 4 cm anal canal ending blindly. Additional anomalies were not detected in further evaluations. With a diagnosis of RA, divergent sigmoid colostomy was performed.

The patient was hospitalized for pullthrough at the age of 2.  Proximal rectal pouch was visiuolized with the aid of a colonoscope that was entered from the distal end of the colostomy. A Veress needle which was entered from the anus, inserted into the proximal rectal atretic pouch with the guide of the colonoscope light. A guide wire that was sent through the veres needle was catched with an endoscopic forceps and taken out from the distal end of the colostomy. Through the distal colostomy, a 16 Fr gastrostomy catheter was placed at the rektoanal anastomotic line with the help of the guide wire. Then rectoanal anostomosis was completed with muco-mucosal sutures. At the post-operative 1th and 2nd months rectoanal anastomosis was dilated under general anesthesia. There was no complication postoperatively.

          Result:RA is a rare type of ARM. The selected cases with short atretic segment can be treated with colonoscopy assisted transanal approach. These patients usually have good postoperative results due to well-developed sphincter complex.

KOLONOSKOPİ YARDIMIYLA REKTAL ATREZİ ONARIMI: OLGU SUNUMU

 

         Giriş:  Rektal atrezi (RA) anorektal malformasyonların (ARM) nadir bir tipidir ve tüm ARM’lerin %1-2’sini oluşturur. RA, pubokoksigeal çizgide veya yukarısında yer alan proksimal rektum ile normal lokalizasyonunda, 1-3 cm derinliğinde, iyi gelişmiş distal anüs ile karakterizedir. Anal kanal ve rektal poş iyi gelişmiş sfinkter kompleksi ile çevrilidir. Farklı yöntemler RA onarımında uygulanmaktadır. Minimal invaziv yöntemle kolonoskopi yardımı ve transanal yaklaşımla rektal atrezi onarımı yapılan olgu deneyimimizi sunmak istiyoruz.

        Olgu: 2 yaşında erkek hasta, post-natal 4. günde gaita çıkışının olmaması ve batın distansiyonu nedeniyle değerlendirildi. Fizik muayenesinde sistemik muayeneleri normaldi. Anal muayenede normal yerleşimli anüsünün olduğu, anal kanalın yaklaşık 4 cm’de kör sonlandığı tespit edildi. Tetkiklerinde ek anomali saptanmadı. Rektal atrezi tanısı ile hastaya sigmoid diverjan kolostomi açıldı. Hasta 2 yaşında düzeltici ameliyatı için yatırıldı. Operasyonda distal kolostomi uçundan girilen kolonoskop yardımı ile proksimal atrezik rektal poş görüntülendi. Anüsten girilen veres iğnesi ilerletilerek proksimal atrezik rektal poşa kolonoskop ışığı klavuzluğunda girildi. Veres iğnesi içerisinden gönderilen kılavuz tel endoskopik forseps ile tutularak distal kolostomi uçundan çıkarıldı. Kılavuz tel rehberliğinde 16 Fr gastrostomi kateteri, distal kolostomi ağzından geçirilerek rektoanal anastomoz hattına bırakıldı. Daha sonra mukomukozal süturlarla anastomoz tamamlandı. Post-op 1. ve 2. ayda genel anestezi altında rektoanal anastomoz dilate edildi. Post-op dönemde herhangi bir komplikasyon olmadı.         

             Sonuç: RA nadir görülen bir ARM tipidir. Atrezik segmentin kısa olduğu seçilmiş olgularda kolonoskopi yardımıyla transanal yaklaşımla tedavi edilebilir. Hastaların iyi gelişmiş sfinkter kompleksine sahip olması nedeniyle ameliyat sonrası sonuçları iyi olmaktadır.
