The Coincidence of Appendicitis and Blunt Abdominal Trauma- The Chicken or the Egg Dilemma

 

Introduction: Appendicitis occurring after blunt abdominal trauma is a rare condition which debates still exist whether this is an incidental finding or result of a cause-effect relationship.  While the underlying mechanism is still unclear, the possibility of a coincidence between these two distinct pathologies endangers a delayed diagnosis and perforation of the appendix. 

Case:  A 6-year-old boy presented to the ED with a 24-hour history of mild abdominal pain, discomfort and nausea starting about 8 hours after a punch in his abdomen by his class mate. He had vomitted once without bile, and his appetite diminished surprising the parents. The abdominal examination was significant for moderate distension and tenderness to palpation and guarding throughout the abdomen, greatest at the right lower quadrant. The examination findings and USG lead us to the  abdominal computed tomography

(CT) scan which displayed moderate free fluid in the pelvis with no solid organ injury. The appendix was inflammed and there were fecaliths in the lumen. Although the patient’s vital signs remained normal, the CT scan findings as well as the patient’s uncomfortable appearance and significant tenderness on abdominal examination lead us to plan surgery instead of observing the patient and performing serial examinations. A diagnosis of acute appendicitis and rupture of the organ was made.

Discussion:  Although easier with the aid of modern imaging techniques, we emphasize that, appendicitis has to be ruled out in presence of a history of blunt abdominal trauma with the findings of right lower quadrant pain and tenderness. The aim of this paper is not to demonstrate rather a direct relationship between trauma and appendicitis or prove the causative factors but to maintain a high level index of suspicion that there are cases with obscured appendicitis behind the curtaining history of blunt abdominal trauma. Keeping the possibility in mind, timely diagnostic evaluation and suitable imaging will prevent the delay in diagnosis, which will reduce the complications.

Apandisit ve Künt Batın Travması Birlikteliği-Yumurta mı Tavuk mu?

 

Giriş: Travma sonrası oluşan apandisit nadir görülen bir durumdur ve tesadüfî bir birliktelik veya bir neden-sonuç ilişkisinin sonucu olup olmadığı konusunda tartışmalar devam etmektedir. Altta yatan mekanizma hala belirsiz olsa da, bu iki farklı patoloji arasındaki ilişki, gözden kaçtığında apendiksin gecikmiş tanısı ve perforasyonu ile sonuçlanır.

Olgu: 6 yaşında erkek, sınıf arkadaşı tarafından karnına atılan bir yumruktan 8 saat sonra başlayan karın ağrısı, rahatsızlık hissi ve bulantı şikayetleri ile olaydan yaklaşık 24 saat sonra başvurdu. Bir kez safrasız kusma öyküsü ile birlikte aile iştahının azaldığını ifade etmekteydi. Karın muayenesinde palpasyonda sağ alt kadranda daha belirgin olmak üzere tüm kadranlarda hassasiyet saptandı. Muayene bulguları ve USG sonucunda abdominal bilgisayarlı tomografi planlandı. BT de solid organ yaralanması gözlenmedi ve ılımlı serbest sıvı,  apendiks lümeninde fekalit ve inflamasyon bulguları rapor edildi. Hastanın vital bulgularının normal seyretmesine karşın, BT bulguları yanı sıra fizik muayene ve hastanın rahatsız görünümü, izlem ve seri incelemeler gerçekleştirmek yerine cerrahi karara yönlendirdi. Akut apandisit ve rüptür tanısı kondu.

Tartışma: Modern görüntüleme tekniklerinin yardımıyla kolaylaşmış olsa da, künt karın travma öyküsü ve sağ alt kadranda ağrı ve hassasiyet bulguları birlikteliğinde apandisitin ekarte edilmelisi gerekliliğini vurgulamaktayız. Bu yazının amacı, travma ve apandisit arasında doğrudan bir ilişki göstermek veya etken faktörler kanıtlamak değil, künt karın travmasının perdelemesi altında gizlenmiş apandisit olabileceğini ve şüpheli davranmak gerekliliğini irdelemektir. Olasılığın akılda tutulması, zamanında tanısal yaklaşımlar, değerlendirme ve uygun görüntüleme yöntemleri, tanıda gecikmeyi önleyecek ve komplikasyonları azaltacaktır.
