THE EFFECTS OF HEAT AND OXYGEN SUPPLY ON SURGICAL SITE INFECTIONS IN CHILDREN.

 

 

 

Aim:This study aims examination of effect of the heat and oxygen supply given to patients during and after surgery, who underwent surgery for acute or perforated appendicitis in childhood, on surgical site infection (SSI).

 Material and Method: Prospective study was performed in 80 patients. Patients were divided into four groups. (Table 1)

 All patients were heated using the heat bed during surgery. The additional heat is supplied for 6 hours using a heat bed at Groups B and D at postoperative period, no additional heat were provided to groups A and C. Only 40% oxygen were given to Groups A and B during the operation. 100% oxygen was given to Groups C and D during the surgery and 80% oxygen was given with mask for 6 hours in the postoperative period. Periodically monitoring of oxygen saturation by pulse oximetry, capillary blood gas evaluation, monitoring of fever, white blood cell count, C-reactive protein (CRP) and blood glucose levels were measured.

 Results: CAE was detected in total of 11 patients (13.8%). Statistically significant difference was observed at an advanced level between the groups in the incidence of surgical site infection (p<0,01). 31.8% of Group A (n = 7) and 20% of Group C (n = 4)  has infection, but no infection was observed in any cases in Groups B and D. Infection rate in group A was significantly higher than Group B and Group D (respectively, p=0,011 and p=0,009). There was no significant difference between group A and group C. Infection rates for Group C was significantly higher that rates of infection of Groups B and D. (respectively, p=0,045 and p=0,035). There was no statistically significant difference in terms of oxygen saturation, blood gas values, CRP or blood sugar values between CAE determined group and CAE nonobserved group. Values obtained by the measurement of white blood cells on day 2 in Group C were significantly higher than group D (p=0,049). The incidence of high fever (> 37.5 ° C) was significantly higher in group developed CAE than group having no CAE development in first 48 hours (p<0,01).

Conclusion: No CAE was found in patient groups protected from hypothermia by providing supply of additional heat, however CAE was developed in groups having no heat supply. There is no effect of oxygen supply given during and after surgery on CAE. The value difference of white blood cells obtained from the study has possitive effect on infection marker of heat supply independent from oxygen supply. These results indicate that hypothermia in children is an important factor for the development of CAE and suggest that continuing to give heat supply after surgery will reduce infection rates in patients.

 

ÇOCUKLARDA ISI VE OKSİJEN DESTEĞİNİN CERRAHİ ALAN ENFEKSİYONU ÜZERİNE ETKİLERİ.

 

Amaç: Çocukluk yaş grubunda akut veya perfore apandisit nedeniyle ameliyat edilen hastalarda ameliyat sırasında ve sonrasında verilen ısı ve oksijen desteğinin cerrahi alan enfeksiyonu (CAE)  üzerine olan olası etkisinin araştırılması amaçlanmıştır.

Gereç ve Yöntem: Çalışma ileriye dönük olarak ve 80 hastada gerçekleştirildi. Hastalar dört gruba ayrıldı. (Tablo 1)

Ameliyat süresince tüm hastalar ısı yatağı kullanılarak ısıtıldı. Ameliyat sonrası dönemde B ve D gruplarına ısı yatağı kullanılarak 6 saat süreyle ek ısı desteği sağlanırken, A ve C gruplarına ek ısı desteği verilmedi. A ve B gruplarına yalnızca ameliyat süresince olmak üzere % 40 oranında oksijen verildi. C ve D gruplarına ise ameliyat süresince % 100 oksijen ve ameliyat sonrası dönemde maske ile 6 saat süreyle % 80 oksijen verildi. Belirli aralıklarla nabız oksimetresi ile oksijen satürasyonu izlemi, kapiller kan gazı değerlendirmesi, ateş izlemi, kan beyaz küre, C-reaktif protein (CRP) ve kan glukoz düzeyleri ölçümü yapıldı.

Bulgular: Toplam 11 hastada ( % 13,8) CAE tespit edildi. Gruplar arasında cerrahi alanda enfeksiyon görülme oranları arasında istatistiksel olarak ileri düzeyde anlamlı farklılık saptandı (p<0,01). A Grubu’nun %31,8’inde (n=7) ve C Grubu’nun % 20’sinde (n=4) enfeksiyon görülürken, B ve D Grupları’nda hiçbir olguda enfeksiyon görülmedi. A grubunun enfeksiyon oranı Grup B ve Grup D’den anlamlı düzeyde yüksek olarak saptandı (sırasıyla, p=0,011 ve p=0,009). A grubu ile C grubu arasında anlamlı fark yoktu. Grup C’nin enfeksiyon oranları ise Grup B ve Grup D’nin enfeksiyon oranlarından anlamlı düzeyde yüksek olarak saptandı. (sırasıyla, p=0,045 ve p=0,035). CAE tespit edilen grupla CAE gözlenmeyen grup arasında oksijen satürasyonu, kan gazı değerleri, CRP veya kan şekeri değerleri açısından istatiksel olarak anlamlı fark saptanmadı. Kan beyaz küre değerlerinin 2. gün ölçümünde Grup C’de elde edilen değerler Grup D ‘ye göre anlamlı şekilde yüksekti (p=0,049). CAE gelişen grupta CAE gelişmeyen gruba göre ilk 48 saatte yüksek ateş (>37.5 °C) görülme sıklığı anlamlı olarak yüksekti (p<0,01).

Sonuç: Ek ısı desteği sağlanarak hipotermiden korunan hasta gruplarında CAE tespit edilmezken, ısı desteği sağlanmayan gruplarda CAE gelişmiştir. Ameliyat süresince ve sonrasında verilen oksijen desteğinin ise CAE üzerine olan herhangi bir etkisi gösterilememiştir. Çalışmada elde edilen kan beyaz küre değeri farklılığı da oksijen desteğinden bağımsız olarak, ısı desteğinin bu enfeksiyon belirteci üzerindeki olumlu etkisi olduğunu desteklemiştir. Bu sonuçlar çocuklarda hipoterminin CAE gelişimi için önemli bir etken olduğunu göstermektedir ve hastalara ameliyat sonrasında ısı desteği verilmeye devam edilmesinin enfeksiyon oranlarını düşüreceğini düşündürmektedir.
