SYNCHRONOUS BILATERAL MORGAGNI AND LEFT BOCHDALEK HERNIAS: A CASE REPORT

 

SYNCHRONOUS BİLATERAL MORGAGNİ AND LEFT BOCHDALEK HERNİAS

The most part of  Congenital Diaphragmatic Hernias (CDH) is demonstrated in the antenatal period. CDH which demonstrated later than antenatal period ; are difficult to diagnosis and the most of them are misdiagnosis  .The occurrence of bilateral diaphragmatic hernias is a rare conditition but combination of bilateral Morgagni hernias and left Bochdalec hernia is extremely rare. İn this study;  we will present  39 day old infant with bilateral Morgagni hernias and left Bochdalec hernia which was misdiagnosed with aspiration pneumonia. 

THE CASE:

A girl infant with cyanosis was found at home and her family  entered a  nearest hospital. The case had  Cardio pulmoner  arrest (CPR) in hospital and after the CPR the case was hospitalized  in intesive care unit in our hospital. The  case received treatment as aspiration pneumonia . Despite the treatment the clinic of infant was not going well. The radiology department read the infant’s chest x-ray  and thought that was diaphragmatic hernia. Left Bochdalek hernia was seen with thoracic ultrasound. After the consultation of pediatric surgery,  the case was  performed laparotomy. İn exploration there was a left posterolateral defect  and  bilateral parasternal defect . All of the defects  had  hernia sac. A part of left lob of the liver and colon were in the left hemithorax. All of the defects repaired without excision of sac.

CONCLUSİON:

CDH, although rare, may occur in the late period. Diagnosis is sometimes difficult, but the case with sudden respiratory distress, or  not responding to treatment, we have to think ‘congenital diaphragmatic hernia ‘. We have to  be exploration of both the diaphragm parts during the operation.

 

 

BİLATERAL MORGAGNİ HERNİSİ VE SOL BOCHDALEK HERNİSİ BİRLİKTELİĞİ: OLGU SUNUMU

 

 BİLATERAL MORGAGNİ HERNİSİ VE SOL BOCHDALEK HERNİSİ BİRLİKTELİĞİ

Konjenital diafragma hernilerinin (KDH) büyük çoğunluğu antenatal olarak tanı alır. Geç bulgu veren KDH’de sıklıkla tanı güçtür ve genellikle klinik olarak farklı tanı alırlar. KDH’nin bilateral görülmesi nadirdir. Bilateral Morgagni hernisinin, sol Bochadalek hernisi ile birlikteliği ise oldukça nadir görülen bir anomalidir. Biz aspirasyon pnömonisi tanısıyla yatırılıp tedavi başlanan ve sonrasında bilateral Morgagni hernisi ve Bochdalek hernisi saptanan 39 günlük kız  olguyu sunuyoruz.

Olgu:

Evde siyanoze halde bulunan olgumuz, en yakın hastaneye götürülmüş ve hastanede arrest olan olguya  Kardiyopulmoner resusitasyon(CPR) uygulanmış ve CPR’a yanıt veren olgu aspirasyon pnömonisi tanısıyla hastanemiz çocuk yoğun bakım kliniğine yatırılıp antibiyotik tedavisi başlanmış.

Tedaviye  rağmen akciğer bulguları gerilemeyen olgunun X-ray grafisinde radyoloji bölümünce diafragma hernisi olabileceği düşünülmüş. Toraks usg’de sol diafragma hernisi tespit edilen olgu tarafımızca opere edildi. Sol üst kadrandan laparotomi yapıldı. Eksplorasyonda   sol posterolateral defekt ve bilateral  parasternal defekt görüldü. Tüm defektlerde herni kesesi mevcuttu. Karaciğer sol lobun bir kısmının ve kolonun sol hemitoraksa yerleştiği görüldü. Herni kesesi eksize edilmeden defektler onarıldı.

Sonuç:

KDH nadir deolsa geç dönemde ortaya çıkabilir. Tanı bazen güçtür ancak ani solunum sıkıntısı ile gelen  veya tedaviye yanıt vermeyen olgularda nadir bir neden olsa da KDH düşünülmelidir. Operasyon sırasında eksplorasyonda diaframın her iki parçası da değerlendirilmelidir.
