DIFFUSE HEMANGIOMA-LYMPHANGIOMA IN THE BOWEL AND MESENTERIUM OF A PATIENT WITH DIAGNOSIS OF FETAL ASCITES

Aim:Hemangiomas are benign lesions, commonly seen in childhood, which can grow considerably in the post-natal period. When they are located in the intestines, they can cause symptoms such as rectal bleeding and intussusception. We present a case with widespread hemangioma and lymphangioma in the mesenterium, who presented with fetal ascites, a condition that is rarely encountered.

Case:A 6 month-old girl with isolated fetal ascites, had been followed by other centers with the diagnosis of prematurity. In the patient with massive ascites, magnetic resonance imaging (MRI) had revealed a mesenteric cyst. After the re-evaluation of the MRI in our center, in which the findings were not found to be compatible with mesenteric cyst, exploratory lapatoromy was performed. During the laparotomy, the ascites of about 700 ml was drained. Throughout the whole the intestinal wall, the stomach wall and in the mesenterium, hemangioma-lymphangioma-like lesions were found, the largest of which was 1 cm. The most intense area of the lesions, including the ileocecal valve, the cecum and appendix, in a 30-centimeter long ileal segment and a 10-centimeter long colonic segment were resected together with the mesenterium, and an end-to-end anastomosis was performed. The samples of lesions in the omentum and in the stomach wall were sent for histopathological examination. Having no additional complaints, the patient was discharged on the 5th postoperative day.

Discussion:Hemangioma and lymphangioma can be located in all organs. The symptoms can vary depending on the location. The intestinal lymphangioma may have a diffuse involvement. The differential diagnosis can be difficult due to the massive ascites. The treatment is surgical excision if possible.

 

FETAL ASİT TANISI İLE TAKİP EDİLEN HASTADA BAĞIRSAK VE MEZODA YAYGIN HEMANJİOM-LENFANJİOM

Amaç:Hemanjiomlar çocukluk çağında sık görülen, doğum sonrası dönemde oldukça büyüyebilen selim lezyonlardır. Bağırsaklarda bulunduklarında invajinasyon ve rektal kanama gibi belirtilere yol açabilirler. Nadiren karşılaşılan bir durum olan fetal asit ile başvuran bağırsaklarda ve mezoda yaygın hemanjiom ve lenfanjiomlu olgu sunulmaktadır.

 

Olgu:6 aylık kız, izole fetal asit, premaürite tanıları ile dış merkezde takip edilmiş. Masif asiti olan hastanın yapılan manyetik rezonans görüntülemesinde (MRG) mezenter kisti saptanmış. Çekilen MRG merkezimizde tekrar değerlendirilmesi üzerine bulguların mezenter kisti ile uyumlu olmadığı düşünülerek eksploratris lapatoromi yapıldı. Laparotomi sırasında karın içinden yaklaşık 700 ml asit sıvısı boşaltıldı. Tüm bağırsak duvarında, mide duvarında ve mezoda, en büyüğü 1 cm olan hemanjiom-lenfanjiom benzeri lezyon saptandı. Lezyonların en yoğun olduğu alan olan ileoçekal valvi, çekum ve apendiksi içerecek şekilde 30 cmlik bir ileal segment ve 10 cmlik kolon segmenti mezosu ile beraber rezeke edildi, sonrasında uç-uca anastomoz yapıldı. Omentum ve mide duvarındaki lezyonlardan örnek gönderildi. Ameliyat sonrası dönemde ek yakınması olmaması üzerine 5. gün sorunsuz taburcu edildi.

 

Tartışma:Hemanjiom ve lenfanjiomlar bütün organlarda yerleşebilmektedir. Yerleşim yerine göre belirtiler değişmektedir. İntestinal lenfanjiom yaygın tutulum gösterebilmektedir. Masif asit nedeni ile radyolojik olarak ayırıcı tanı zorlaşmaktadır. Tedavi, mümkün ise cerrahi eksizyondur.
