PERIUMBILICAL MULTIPORT LAPAROSCOPIC APPENDECTOMY:A PRELIMINARY EXPERIENCE USING A NOVEL TECHNIQUE

 

 

Purpose

Single incision laparoscopic surgery (SILS) is especially developed in order to achieve better cosmetic results. In this approach, using the umbilicus for port access is widely accepted. The aim of this study is to evaluate our initial experience with a novel port access technique that we used for laparoscopic appendectomy: Periumbilical Multiport Laparoscopic Appendectomy (PUMLA).

Material and Methods

Between January 2011 and March 2012, a total of 12 appendectomies were performed using a new port access technique . Eleven of these cases had acute appendicitis and 1 patient had Familial Mediterranean Fever (FMF). During these operations, 3 separate curvilinear incisions were created in the periumbilical skin folds and ports were placed in these ways. The 5 mm port was used for a 0 degree telescope. The other 2 were used  to manipulate the laparoscopic  instruments. The appendectomy was performed intracorporeally in 4 cases; however, in 8 cases, the appendix was taken out via trocar after dissecting the meso-appendix and the appendectomy was performed extra-corporeally. Reconstruction was not needed to the umbilicus as there was no anatomical distortion.

Results

Four patients were female; 8 were male. Their ages were between 5-15 years (mean 9.5 y). Operation time was between 20 and 61 minutes (mean 36 min). We did not need to convert to standard laparoscopic appendectomy or place an additional port in any of these patients. The average postoperative length of hospital stay was 10. 2 hours (5-22 h). No complications were observed in the early and late postoperative period. As a result of the separate periumbilical curvilinear incision technique, good cosmetic results were observed.

Conclusion

We found that this new port access technique is feasible, safe, and is associated with good cosmetic results.

PERİUMBLİKAL MULTİPORT LAPAROSKOPİK APENDEKTOMİ:YENİ BİR TEKNİKLE İLGİLİ İLK DENEYİMLERİMİZ

 

Giriş: Single incision laparoskopik cerrahi (SILS) iyi bir kozmetik görünüm sağlamak amacıyla geliştirilmiştir. Bu yaklaşımda doğal orifis olarak göbek kullanımı çok geniş kabul görmüştür. Bu çalışmanın  amacı apendektomide kullandığımız yeni bir port giriş tekniği ile ilgili ilk deneyimlerimizi sunmaktır : Periumbilical Multiport Laparoscopic Appendectomy  (PUMLA).

Gereç ve Yöntemler:Ocak 2011-Mart 2012 arasında kliniğimizde 12 çocuk hastaya yeni bir port giriş tekniğiyle apendektomi yapıldı. Bunlardan 1 ine FMF nedeniyle, diğer 11 ine ise akut apandisit nedeniyle apendektomi yapıldı. Bu ameliyatlarda, periumbilikal cilt kıvrımlarında curvilinear ve aralarında mesafe olan üç insizyon yapıldı ve portlar buralardan yerleştirildi. 5 mm olanı 0 derece teleskop için diğer ikisi ise çalışma enstrümanları için  kullanıldılar. 4 vakada apendektomi intracorporeal yapıldı. 8 vakada ise apendiks, mezosu diseke edildikten sonra trokar giriş yerinden dışarı alınarak apendektomi extracorporeal gerçekleştirildi. Göbeğin şekli bozulmadığından yeniden şekillendirilmesi gerekmedi.

Bulgular:Hastaların 4 ü kız 8 i erkek ve  5-15 yaş (ort.9.5) arasındaydılar. Ameliyat süresi 20 dk ile 61 dk (ort.36 dk) arasında değişmekteydi. Hiçbir hastada standart laparoskopik apendektomiye geçiş gerekmedi. Hastaların ortalama postoperatif yatış süresi 10.2 saatti (5-22). Erken ve geç dönemde herhangi bir komplikasyon görülmedi. İnsizyonlar periumblikal curvilinear olduğundan tüm olgularda iyi bir kozmetik görünüm elde edildi.

Çıkarımlar:Biz bu yeni port giriş tekniğini apendektomide kolaylıkla yapılabilir, güvenli ve kozmetik sonuçları iyi olarak bulduk.
