Duodenal Perforations in Children

 

Aim:To analyse patients those admitted to our clinic due to perforation in duodenum.

Patients and Method :

Ten patients those have been admitted to our clinic with perforation in duodenum between 1990 – 2010 analysed retrospectively.

Results :

Ten patients (7M, 3W) those have been admitted to our clinic between 1990 – 2011 have a mean age of 5.2 years (25 days – 14 years). Two of cases admitted directly to our clinic while 3 of cases refered from other clinics in our Hospital and the rest refered from another hospital. 3 patients had symptoms of stomach ache, 1 had nausea, 2 had vomiting, 1 had abdominal distension, 1 had fatique and 1 had signs of hemorage from upper GI tract. One of cases  was traffic accident, 1 was crash and 1 was beating. Mean time for appşiance to our clinic was 3.3 days ( 1day – 1 week). At first admission one patient had leucocytosis while annother one had leucopenia. In 7 cases there were air under diaphragma while the rest were normal. USG was performed in three cases and in one had fluid accumulation in abdomen. CT was performed in three patients those admitted due to trauma and free air was there in the abdomen. Ulcus in duodenum was the cause in six cases while in 3 the cause was trauma and in 1 case with ichtiosis it was idiopathic. The case with hyperbilluribinemia after near total pancreatectomy due to nesidioblastosis had been explored and perforation in deuedenum diagnosed. Resection in first two parts of duodenum and pilor, choledecojejunostomy, gastrojejunostomy and jejunojejunostomy was performed. Primary repair was performed in rest. In one case with primary repair gastrostomia was performed while in 4 cases omentoplasty were performed. In follow up 7 cases are well while one patient was exitus and two left follow up.

Conclusion :

 

Perforation in duedenum is a rare entity that pediatric surgeons encounter. Because it’s mostly ignored in patients with stomach ache in differential diagnosisin Pediatric Clinics, mean time for admitting patients is long. Primary repair(duodenoraphy ± omentoplasty) is the most used modality and it’s safe enough.

 

Çocuklarda Duodenum Perforasyonları

 

Amaç:Kliniğimizde duodenal perforasyon nedeniyle takip ve tedavisi yapılan olguların incelenmesidir.

Olgular ve Yöntem :

Duodenal perforasyon tanısı ile 1990 – 2010 yılları arasında tedavi edilen 10 olgunun kayıtları geriye dönük olarak incelendi.

Bulgular :

1990 – 2011 yılları arasında kliniğimize başvuran 10 olgunun (7E, 3K) ortalama başvuru yaşı 5,2 yıl (25 gün - 14 yıl) dı. İki olgu primer olarak başvuruda bulunurken, 3 olgu diğer kliniklerden, 5 olgu dış merkezlerden refere edilmişti. Başvuru yakınmaları karın ağrısı (3), bulantı (1), kusma (2), karın distansiyonu (1), halsizlik (1), üst gastrointestinal sistem kanaması(1), yüksekten düşme (1), araç dışı trafik kazası (1) ve darp (1) idi. Ortalama başvuru süresi 3,3 (1 gün – 1 hafta) gündü. İlk başvuru anında iki olguda lökositoz mevcut iken bir olguda ise lökopeni mevcuttu. Ayakta direkt batın grafilerinde 7 olguda diyafragma altı hava mevcutken diğer olguların direkt grafisi normaldi. 3 olguya USG yapılmıştı ve birinde batın içinde sıvı mevcuttu. Travma nedeniyle başvuran 3 olguda batın BT’sinde karın içi serbest hava mevcuttu. 6 olguda perforasyon nedeni duodenal ulcus,  3 olguda travmatik perforasyon, ichtiosisli 2 aylık bir olguda ise idiopatik idi. Bir olguda nesidioblastozis nedeniyle totale yakın pankreatektomi sonrasında hiperbilüribineminin devam etmesi üzerine yapılan laparotomide duodenal perforasyon saptanmış ve hastaya duodenum 1. ve 2. Kıtaya rezeksiyon, pilor rezeksiyonu, koledekojejunostomi, gastrojejunostomi ve jejunojejunostomi uygulandı. Kalan 9 hastada primer tamir yapıldı. Primer tamir yapılan hastaların birinde gastrostomi açılırken 4 hastaya omentoplasti de yapıldı. Takipte 7 olgunun herhangi bir klinik yakınması yokken, bir olgu kaybedilmiş iki olgu takip dışı kalmıştır.

Sonuç :

Duodenal perforasyonlar çocuk cerrahlarının nadir karşılaştıkları cerrahi problemlerden biridir. Bu nedenle karın ağrısının ayırıcı tanısında sıklıkla gözardı edildiğinden özellikle Çocuk Hastalıkları kliniklerinden yönlendirilen olgularımızda ortalama başvuru zamanı oldukça yüksektir.  Tedavide en sık tercih edilen primer onarım (duodenorafi ± omentoplasti), etkin ve güvenli bir cerrahi yöntemdir.
