The relationship between ultrasonographic findings and body mass index in diagnosis of appendicitis

Background/Aim: The diagnosis of appendicitis is made on ultrasound when appendix with a diameter of 6-7 cm is seen. However, whether this diagnostic criteria vary with body mass index (BMI) or not,  remains unknown. This report evaluates the effect of BMI to ultrasonographic findings retrospectively in cases with a presumptive diagnosis of appendicitis.  

Material and Method: Between January 2010 and April 2011, 49 patients evaluated with a presumptive diagnosis of appendicitis (Group I) and 80 patients had an appendectomy with a diagnosis of acute or perforated appendicitis (Group II)  are included in this study. Age, gender, BMI, white blood cell count, diagnosis and ultrasonographic findings (diameter of appendix, response to compression) of the cases are evaluated retrospectively. 

Results: Female to male ratio was 28:21 and 29:51 in Group I and II, respectively. The median age was 11 years in Group I (iqi: 8-13) and also 11 years in Group II (iqi:9-13). When considering BMI, it was normal 63.1%, low 26.3% and high 10.5% of cases in group I. BMI was normal 54%, low 32.4% and high 13.5% of cases in group II. The median value of the diameter of appendix measured by US was 5 (iqi:3-6) in group I. There was no appendix in 44.6%, the diameter of appendix was lower than 7 mm in 46.8% and higher than 8 mm in 8.5% of cases with US. There was no patient with a 7-8 mm diameter of appendix. There was a response to compression in 75%, reduced response in 12.5% and there was no response in 12.5% of cases with US probe. The median value of the diameter of appendix measured by US was 9.5 (iqi:7.75-11.25) in group II. In this group there was no appendix in 19%, the diameter of appendix was lower than 7 mm in 9%, it was 7-8 mm in 20.6% and higher than 8 mm in 50.7% of cases with US. There was no response to compression in 82.3%, reduced response in 15.6% and there was a response in 1.9% of cases with US probe. When comparing the two groups there was a significant increase in white blood cell count and the diameter of appendix in group II (Mann Whitney U test, p<0.05). The effect of the diagnosis and BMI to the diameter of appendix measured by US is analyzed with Two-Way ANOVA test. It is demonstrated that there is an effect of appendicitis to the diameter of appendix (F(1,41)=21.831, p<0.001). There was no effect of BMI to the diameter of appendix (F(2,41)=0.157, p=0.856). There was also no interaction between diagnosis and BMI (F(2,41)=0.165, p=0.848). None of the parameters have an effect on appendicitis separately and there was no effect of BMI to the ultrasonographic findings with multivariate regression analysis. 

Conclusion: Increased diameter of appendix measured by US and responseless to compression by US probe are supportive findings in the diagnosis of appendicitis, although there is no effect of BMI to the diameter of appendix.

 

Apandisit tanısında ultrasonografik bulguların vücut kitle indeksi ile ilişkisi

Giriş/Amaç: Ultrasonografi (US)’de apendiks çapının 6-7 mm den fazla olması apandisit lehine bir bulgu olarak kabul görmektedir.  Ancak bu tanısal kriterin  vücut kitle indeksine (VKİ) göre değişkenlik gösterip göstermediği bilinmemektedir. Apandisit ön tanısıyla US yapılan olgularda VKİ’nin ultrasonografik bulgulara etkisini değerlendirmek üzere geriye dönük bir çalışma yapılmıştır.

Gereç ve Yöntem: Çalışmamızda, Ocak 2010- Nisan 2011 döneminde apandisit ön tanısı ile takip edilen (grup I) 49 hasta ile akut veya perfore apandisit nedeniyle apandektomi yapılan (grup II) 80 olgu dahil edilmiştir. Olguların yaş, cinsiyet, VKİ, beyaz küre sayıları, tanıları, ultrasonografi bulguları (apandiks çapı, kompresyona yanıt) geriye dönük olarak değerlendirildi.

Bulgular: Grup I ve II’de kız erkek oranı sırasıyla 28:21 ve 29:51’di. Yaş ortancaları Grup I’de 11 (çaa:8-13), Grup II’de 11 (çaa:9-13) yıldı. VKI oranlarına bakıldığında Grup I’de olguların VKİ’lerinin %63.1 normal, %26.3 düşük, %10.5 yüksek olarak belirlendi. Grup II’de ise %54 normal, %32.4 düşük, %13.5 yüksek VKİ saptandı. Grup ‘de US’de saptanan apendiks çaplarının ortanca değeri 5 (çaa:3-6 )olarak bulundu.  US de hastaların %44.6 sının apendiksi izlenemedi, %46.8 hastanın apendiks çapı 7mm den küçük, %8.5’ininki de 8mm den büyüktü. Apendiks çapı 7-8mm arasında olan hasta yoktu. US probu ile kompesyona yanıt alınan apendiks oranı %75, kompresyona yanıtı azalmış olanlar %12.5, yanıt alınamayanlar ise %12.5 olarak bulundu. Grup II’de US ile saptanan apendiks çaplarının ortanca değeri 9.5mm (çaa:7.75-11.25)’di. Bu grupta US ile apendiks izlenemeyen hastaların oranı %19, apendiks çapı 7mm altında olanlar %9, 7-8mm arasında olanlar %20.6,  8mm den büyük apendiks çapı ölçülenler ise %50.7 oranındaydı. Hastaların %82.3’ünün apendiksi kompresyona yanıtsız, %15.6’sının kompresyona yanıtı azalmış, %1.9’unda ise yanıt vardı.  Gruplar karşılaştırıldığında beyaz küre sayısı ve US’de saptanan apandiks çapı grup II’de anlamlı oranda yüksek bulundu (Mann Whitney U test, p<0.05).  İki yönlü ANOVA testi ile US ile saptanan apendiks çapına tanının ve VKİ’nin etkileri analiz edildi. Yapılan analizde, apandisit varlığının apendiks çapına etkisinin olduğu ortaya konulmuştur (F (1,41)= 21.831, p<0.001). VKİ’nin apendiks çapına etkisinin olmadığı görülmüştür (F (2,41)=0.157, p=0.856). Tanı ile VKİ arasında da etkileşim saptanmamıştır (F (2,41)=0.165, p=0.848). Multivariate regresyon analizinde, hiçbir parametrenin tek başına apandisit olma durumunu etkilemediği ve VKi’nin ultrasonografik bulgulara etki etmediği izlendi.  

Sonuç: US ile saptanan apendiks çapında artış ve US probu ile kompresyona yanıt alınamamasının apandisit lehine tanıyı destekleyici bulgular olduğu; ancak VKİ’nin, US ile saptanan apendiks çapını etkilemediği düşünülmektedir.
