NEONATAL SMALL BOWEL OBSTRUCTION CAUSED BY CONGENITAL MESOCOLIC HERNIA: CASE REPORT

 

Aim of the study:

We aimed to present one case with congenital transmesocolic hernia which is a rare cause of neonatal intestinal obstruction.

 

Methods:

Retrospective analysis of one patient with congenital mesocolic hernia was performed. Diagnosis was confirmed by intraoperative findings. Data including age, reason of hospitalization, type of operation and postoperative results were reviewed.

 

Results:

Infant was hospitalized to neonatal intensive care unit due to abdominal distension and rectal bleeding.

At direct radiogram, bowell gasses were seen at the median of the abdomen and no gas was seen at the pelvis. In the second day, baby underwent operation because of an increase at abdominal distension and recal bleeding. Most of the small intestine were seen as pressed in the hernia sac at the meso of decending colon during operation. The circulation of the bowels were impaired and seen ischemic. After reduction of the intestine, volvulus was seen. Volvulus was corrected and ischemia disappeared. Also, malrotation was noted with mesocolic hernia. At first, hernia sac at meso was resected meticulously to avoid an injury to vessels. Defect at meso repaired. Ultimately, operation was terminated by appendectomy and widening the meso of small bowel. Postooperative first week feding was started and third week discharged. Follow- up is uneventfull after 6 months.

 

Discussion:

Transmesocolic hernia is seen very rarely. Generally, it is acquired after an operation with a defect at the meso. In literature, there are only few studies about congenital mesocolic hernia in children but it must be keep in mind at neonatal intestinal obstructions. The decision of operation couldn’t be postponed due to risk of volvulus which can be seen concomitantly.

YENİDOĞAN DÖNEMİNDE MEZOKOLİK HERNİYE BAĞLI İNTESTİNAL OBSTRÜKSİYON: OLGU SUNUMU

 

Amaç:

Yenidoğan dönemi intestinal obstrüksiyonunun nadir bir nedeni olan transmezokolik hernili bir olgunun sunulması amaçlanmıştır.

 

Yöntem:

Transmezokolik hernili bir olgunun kayıtları incelenmiştir. Tanı operasyon sırasında konulmuştur. Hastanın operasyon yaşı, yatış sebebi, yapılan operasyon ve postoperatif sonuçlar değerlendirilmiştir.

 

Bulgular:

Miad doğum sonrası batın distansiyonu ve rektal kanaması olan olgu yenidoğan yoğun bakıma yatırılmıştır. Çekilen ayakta direk batın grafisinde barsak gazlarının ortada toplandığı ve distale geçiş olmadığı gözlenmiştir. Batın distansiyonu ve rektal kanamanın artması üzerine 2. gün acilen operasyona alınmıştır. Operasyonda yapılan eksplorasyonda ileal ansların tamamına yakınının sağ kolon mezosundaki herni kesesi içinde sıkışmış olduğu gözlendi. Barsakların dolaşımı bozulmuştu. İntestinal anslar redükte edildiğinde volvule oldukları izlendi. Anslar devolvüle edildi. Dolaşımı düzeldi. Mezokolik herni ile birlikte malrotasyon da mevcuttu. Öncelikle mezodaki herni kesesi mezo damarlarına dikkat edilerek eksize edildi. Mezodaki defekt onarıldı. Sonrasında intestinal ansların mezosu genişletilip apendektomi yapılarak operasyon sonlandırıldı. Operasyon sonrası 1. hafta beslenmeye başlandı. 3. hafta taburcu edildi. Postoperatif 6. ayda sorunsuz olarak izlemine devam edilmektedir.

         

Sonuç:

Transmezokolik herni oldukça nadir görülmektedir. Genellikle geçirilmiş batın cerrahisi sonrası mezodaki defekte bağlı edinsel olarak gelişmektedir. Literatürde konjenital mezokolik herniye ait çok az sayıda vaka bildirilmiştir. Yenidoğan dönemi intestinal obstrüksiyon nedenleri arasında nadir de olsa akılda tutulmalıdır. Beraberinde volvulus da görülebileceği için operasyon kararında geç kalınmamalıdır.
