Spontaneously recovered duodenal perforation: Case Report

 

Aim:  Discussion of a duodenal perforation case with a delayed diagnosis and subsequently recovered  spontaneously.

Case: 12 year old male patient with abdominal pain for days without a history of trauma has been followed for defense and sensitivity on the right upper quadrant. On the continuity of the pain, computerized tomography scans with intravenous contrast were taken  and free fluid was observed in the right paraduodenal region, while solid organs were healthy.  Nasogastric tube (NGT) was applied to the patient with normal levels of transaminase and amylase and  follow up was resumed. 300-500 cc of daily drainage fluid including bile didnot alter within the days. On the intolerance of nutrition, upper gastrointestinal tract (GIT) imaging was taken after seven days and contrast media did not reach the distal stomach. Following these findings, the patient was referred to our clinic with pre-diagnosis of duodenal perforation secondary to duodenal obstruction. On the USG examination in the clinic, paraduodenal fluid was lessened. Duodenal stricture or extravasation has not been observed on the upper GIT passage graph. NGT was clamped on the second day due to normal serum gastrin level.  No abdominal pain, vomiting and distension were observed and the patient was nourished orally following the removal of NGT. On the control USG after five days, no free fluid has been observed and the patient was discharged. The case is event-free at the fifth month of follow-up.

Conclusion:  Duodenal perforations not associated with trauma are the rarely observed pathologies of children and cause mortality or morbidity with the lack of early diagnose and treatment. The presented case has been observed at the outer clinic due to delayed diagnosis. Despite the fact that, treatment for duodenal perforations requires surgical intervention, follow-up decision was made due to application on the eleventh day of symptoms, regression of findings and reduced intraabdominal fluid. No complication has been observed on the follow-up.

Spontan iyileşen duodenum perforasyonu:Olgu Sunumu

 

Amaç: Geç tanı konulması nedeniyle takip edilen ve spontan iyileşen  duodenum perforasyonlu   olgunun tartışılması.

Olgu:     Travma öyküsü olmadan 3 gündür devam eden karın ağrısı yakınmasıyla dış merkeze başvuran 12 yaşında erkek olgu sağ üst kadranda hassasiyet ve defans nedeniyle gözleme alınmış.  Ağrısı gerilemeyen hastaya aynı gün çekilen IV kontrastlı BT’de sağ paraduodenal alanda serbest sıvı görülmüş, solid organlar salim izlenmiş. Transaminazları ve amilaz düzeyleri normal olan olguda NG drenajına alınarak takibe devam edilmiş.  Günlük 300-500 cc safralı drenajda günler içinde değişiklik olmamış.  Beslenme girişimlerini tolere etmeyen olguya  7 gün sonra Üst GİS pasaj görüntülemesi yapılmış ve kontrast maddenin midenin distaline geçmediği görülmüş. Bunun üzerine duodenal perforasyona sekonder duodenal obstrüksiyon ön tanısıyla kliniğimize yönlendirilmiş. Olgunun kliniğimizde yapılan US incelemesinde paraduodenal sıvıda azalma görüldü. Üst GİS pasaj grafisinde duodenumda darlık ya da ekstravazasyon saptanmadı. Gastrin düzeyi normal düzeylerde gelen olguda ikinci gün NG klampe edildi. Karın ağrısı, kusma, distansiyon olmaması üzerine NG sonda alınarak oral beslendi.5 gün sonraki kontrol US incelemesinde serbest sıvı görülmeyen olgu taburcu edildi. Olay sonrası beşinci ayda olgu sorunsuzdur.

Sonuç: Travma dışı duodenum perforasyonları çocuklarda nadir görülen ,erken dönemde tanı konulup tadavi edilmezse  mortal ve morbid seyreden patolojilerdir. Sunulan olguda geç tanı konulması nedeniyle dış merkezde  istemeden takip edilmiştir. Duodenum perforasyonlarının tedavisi cerrahi olmasına karşın; olgu kliniğimize başvurduğunda 11 gün geçmesi semptomlarının gerilemesi ve karın içi sıvıda azalma görülmesi nedeniyle takibe devam edilmiş herhangi bir komplikasyon izlenmemiştir.
