SPONTANEOUS PNEUMOTHORAX AFTER BONE MARROW TRANSPLANTATION

Introduction:

After the rupture of a small air sac on the surface of the lung, spontaneous pneumothorax could be seen. Pulmonary complications are the most common causes of morbidity and mortality in hematopoietic cell transplant recipients and the incidence is nearly 40% to 60%. The complications are more frequent in allergenic recipients, especially those with graft versus host disease (GVHD).

 

Case Report:

A 10-year-old male presented with sudden onset of chest pain after coughing. He has a history of steroid-refractory grade III acute GVHD of the lung that began 3 months after transplantation. He was presented with spontaneous pneumothorax again, 10 months ago and monitored closely without any medical intervention. On physical examination there was a decreased breath sound over the area of the affected left lung. The patient was tachycardic, tachypnic, and hypoxemic. Oxygen therapy was started with medical treatment. The patient is monitored constantly and multiple chest x-rays. Radiographic findings include visualizing the ipsilateral lung margin as a radiolucent line that parallels the chest wall andmediastinal displacement the trachea. Intercostal tube thoracostomy was performed quickly. 5 days later there was no free air in the thorax and he was discharged without any problem. 2 weeks later he presented with the same complaint and treated with gomco and tube thoracostomy.

 

Conclusion:

Once spontaneous pneumothorax diagnosed, with symptoms of tachypne, and hypoxemiatreatment should begin with immediate high flow oxygen to maximize oxygenation and immediate tube thoracostomy should be done as the treatment of choice. Gomco device will be used for more effective treatment with adhesions.

KİT Sonrası Spontan Pnömotoraks Gelişimi: Bir olgu Sunumu

Giriş:

Akciğerin yüzeyinde yer alan küçük bir hava kesesinin rüptüre olması sonrasında spontan pnömotoraks görülebilir. Kemik iliği transplantasyonu sonrası görülen pulmoner komplikasyonlar en sık mortalite ve morbidite sebepleridir ve yaklaşık %40 ile 60 oranında görülür. Komplikasyonlar özellikle graft versus host hastalığı olan alerjik alıcılarda daha sık görülmektedir,

 

Olgu Sunumu:

10 yaşında olgunun ilk başvurusu öksürme sonrası ani başlayan göğüs ağrısı şikayetidir. Transplantasyondan üç ay sonra aciğerlerde gelişen steroide dirençli grade 3 GVHD hikayesi, bronkoskopi sonrası pnömothoraks hikayesi mevcuttur. Kontrol gögüs BT’sinde sağ akciğerde atelektazik alanlar ve yapışıklıklar olduğu, trakeanın sağa yer değiştirdiği görülmüştür. 10 ay önce yine spontan pnömotoraks şikayeti ile başvurmuş ve herhangi bir müdahale yapılmadan yakın takibe alınmıştır. Fizik muayenede etkilenmiş akciğer bölgesinde solunum seslerinde azalma belirlenmiştir. Hasta taşikardik, taşipneik ve hipoksemik idi. Medikal tedavi ve oksijen desteğine başlanmıştır. Hasta tekrarlayan akciğer grafileri ile sürekli takibe altına alınmıştır. Radyolojik incelemede aynı taraf akciğer sınırı radyolusent bir çizgi şeklinde göğüs duvarına paralel olarak izlenmiştir. interkostal tüp torakostomi uygulanmıştır. Pnömothoraks bulguları gerilemeyen olguya negatif basınçlı gomco cihazı bağlanmış ve serbest hava boşaltılmıştır. Beş gün sonra problemsiz taburcu edilmiştir. Yaklaşık 2 hafta sonra öksürük sonrası aynı şikayet ile başvuran olgu tüp torakostomi ve negatif basınçlı gomco cihazı ile tedavi edilmiş, 6 gün sonra sorunsuz taburcu edilmiştir.

 

Tartışma:

GVHD sonrası akciğer dokusu travmaya hassas olabilmektedir bu nedenle aileler bu konuda bilgilendirilmelidir. Taşipne bulguları ile başvuran spontan pnömothoraksın tanısı konur konmaz, hemen hipoksi için yüksek akımlı oksijen tedavisi başlanmalıdır ve tüp torakostomi tedavisi ilk seçenek olarak planlanmalıdır. Yapışıklıklara bağlı olarak tüp torakostomi ile yeterli hava boşalması sağlanamayan olgularda negatif basınçlı gomko cihazı kullanılabilir.
