THE INCREASING TENDENCY OF PILONIDAL SINUS IN ADOLESCENT

Aim:

Pilonidal sinus disease (PSD) is a painful condition mostly occurs in the intergluteal region and is commoly seen in young adults. PSD in pediatric patients is not a rare occurrence. It is predicted that, asymptomatic pilonidal disease remains undiagnosed in children and they are never infected. Many operative and conservative treatments have been described for the management of PSD.

 

Material Method:

Between 2010 January and 2012 January, 9 adolescents with pilonidal sinus were treated in our pediatric surgery department; 6 girls and 3 boys with a mean age of 14 (10-17) years. 4 of them was infected and presented with painful swelling in the gluteal region. Abscesses were incised and drained then treatedwith oral antibiotics. Body mass index (BMI) was measured for all patients.General anesthesia was used in all surgeries. The operative position used was prone. Palpation and methylene blue injection are always used to estimate the borders of diseased tissue. We performed total excision of the sinus and primary midline closure in all patients.

 

Results:

Among the 6 female patients, 1 were obese (BMI > 30 kg/m2), 3 were overweight (BMI > 25 but < 30 kg/m2) and 2 had a healthy weight (BMI < 25 kg/m2).Among the 3 male patients, 1 were obese (BMI > 30 kg/m2), 2 were overweight (BMI > 25 but < 30 kg/m2). All patients were given intravenous antibiotics and stayed in bed for 3 days after operation. The sutures were removed on day 14. Shaving the natal cleft and area twice monthly after surgery for 1 year was strongly recommended. 4 patients recovered uneventfully but 2 girls had abscess and swelling in the operation region and treated daily with local antibiotics for 2 weeks.

 

Conclusion:

Obesity, sitting for a long time and tight wearing especially in girls are risk factors for PSD. The borders of the surgical wound would depend on the size of the pilonidal sinus cavity and associated sinus tract and should not be extended beyond methylene blue stained tissue.

 

Adolesan Çağda Pilonidal Sinüs hastalığı İnsidansı Giderek Artmakta mı?

Amaç:

Pilonidal Sinüs Hastalığı (PSH) yaygın olarak genç erişkinlerde gözlenen ve genellikle intergluteal bölgede yer alan ağrılı bir durumdur. PSH’ın aslında çocukluk çağında görülmesi nadir değildir. Bulgu vermeyen PSH çocukluk çağında enfekte olmadıkları için teşhis edilemedikleri tahmin edilmektedir. Bu yaygın hastalığın tedavisinde birçok medikal ve cerrahi tedavi yöntemi tanımlanmıştır

 

Materyal Metod:

Ocak 2010 ile Mayıs 2012 arasında 12 adet pilonidal sinüs tanılı adolesan kliniğimizde tedavi edildi. Ortanca yaşı 14 (10-17) olan 7 kız ve 5 erkeğin, 7’sinde enfeksiyona bağlı intergluteal bölgede şiddetli ağrı ve akıntı şikayetleri mevcuttu. Abse boşaltıldı ve antibiyotik tedavisine başlandı. Tüm olguların vücut kitle endeksleri (VKİ) ölçüldü. Cerrahi işlem genel anestezi altında yüzüstü pozisyonda iken enfekte doku sınırları elle muayene ve metilen mavisi enjeksiyonu yardımı ile tespit edilemeye çalışıldıktan sonra tüm olgularda sinüsün total eksizyonu ve primer tamir uygulanmıştır.

 

Sonuçlar:

Kız hastalardan 1’i obez (VKİ> 30 kg/m2), 5’i fazla kilolu (VKI > 25 ama < 30 kg/m2) ve 2’si normal kilodaydı. Beş erkek olgunun 2’si obez ve 3’ü fazla kiloluydu. Tüm hastalara operasyon sonrası antibiyotik tedavisi ve 3 gün yatak istirahati uygulanmıştır. İki hafta sonra dikişler alınmıştır. Bir yıl süreyle natal yarık ve bölgenin ayda iki kez tıraş edilmesi şiddetle önerilmiştir. Dokuz olgu takiplerde sorunsuz iken 3 kız olguda gelişen yara bölgesinde akıntı iki haftalık günlük pansuman ve lokal antibiyotik ile tedavi edilmiştir.

 

Tartışma:

Obezite, uzun süre oturma ve özellikle kızlarda dar giyinme tarzı PSH için önemli risk faktörleridir. Cerrahi sınır pilonidal sinüs boşluğu ve yolunu kapsamalı ve metilen mavisi ile boyanan bölge dışına uzanmamalıdır. Operasyon sonrası özellikle lazer epilasyon önerilmelidir. Çocuklarda obezite ve uzun süre bilgisayar başında oturmanın hastalık için risk oluşturduğunu düşünmekteyiz.
