Hereditary Duodenal Atresia

Duodenal obstruction is one of the most common anomalies in newborns and infants. Although the etiology is not clear in duedonal atresia (DA), Thandler explained the etiology as a result of a problem in luminal recanalization period in 1902. Hereditary duodenal atresia is commonly present with associated anomalies such as Down’s syndrome, malrotation, congenital heart disease, other intestinal obstruction, urinary tract malformations, annular pancreas and anorectal pathologies. These associated anomalies, premature and low birth weight are responsible for the high mortality rate. Hereditary form of duodenal atresia was seen rarely and reported with the autosomal recessive inheritance.

 

Case:

Here we presented a twin and a cousin with duodenal atresia findings in one family. The parents of the affected cases had no consanguinity but married with each other (two sisters married with two brothers). They were originated from the same region of Turkey (eastern part). The twin had also additional embryological defects. One case had Meckel' s diverticulum, jejunal atresia and gallbladder agenesis and the other case had Meckel’ s diverticulum and malrotation. In pedigree analyses, a male cousin was observed with duodenal atresia history. Multiple abortion history was noticed in this family.

 

Discussion:

Having hereditary form of duodenal atresia with other related genetic abnormalities in a non-hereditary pattern could be explained with the same possible molecular gene alterations, shared by these two families. So we suppose that the possible genes affected in our cases may inherit to the affected cases without any consanguinity.

Herediter Duodenal Atrezi

Giriş:

Duodenal obstrüksiyon yenidoğan ve infantlarda sık görülen anomalilerden birisidir. Duodenal atrezinin (DA) etyolojisi her ne kadar açık olmasa da 1902 yılında Thandler etyolojiyi luminal rekanalizasyon periyodunda oluşan bir problem sonucu ortaya çıktığını açıklamıştır. Herediter DA sıklıkla Down sendromu, malrotasyon, konjenital kalp hastalıkları, diğer intestinal obstrüksiyonlar, üriner sistem malformasyonları, anüler pankreas ve anorektal patolojiler gibi anomaliler ile birliktelik gösterir. Ek anomaliler, prematürite ve düşük doğum ağırlığı yüksek mortalite oranından sorumludur. Duodenal atrezinin herediter formu oldukça nadir olup, otozomal resessif kalıtım ile ilgili olduğu bildirilmiştir.

 

Olgu:

Her iki ikiz eşinde ve bunların kuzeninde duodenal atrezi bulguları olan bir aile kliniğimize başvurdu. Etkilenen olguların ailelerinde herhangi bir kan bağı bulunmamakla birlikte iki kız kardeşin iki erkek kardeş ile evliliği söz konusudur. Aileler Türkiye’nin aynı bölgesinden orijin almaktadır (Doğu kesimi). İkizlerin ayrıca ek embriyolojik anomalileri mevcuttu. Olgulardan birinde Meckel Divertikülü, jejunal atrezi ve safra kesesi agenezisi mevcutken, diğerinde Meckel Divertikülü ve malrotasyon olduğu izlenmiştir. Soyağacı incelemesinde erkek kuzende de duodenal atrezi öyküsü olduğu gözlenmiştir. Bu ailede multiple düşük öyküsü de dikkate değer bulunmuştur.

 

Tartışma:

 

Herediter olmayan bir paternde duodenal atrezinin ek genetik anomalilerle birlikte herediter formunun gözlenmesi, her iki ailenin aynı moleküler genetik bozukluğa sahip olması ile açıklanabilir. Bu nedenle olgularımızda etkilenmiş olan genlerin kan bağı olmaksızın etkilendiği düşünülmektedir
