INCIDENCE AND ETIOLOGY OF UNDESCENDED TESTIS IN CHILDREN WITH CEREBRAL PALSY

Aim of the study: Cerebral palsy (CP) is the most common cause in neurologically impaired children. Very few data on their testicular problems can be found in literature. Both parents and physicians are less interested in genital examination. We aimed to present the incidence and etiology of undescended testis (UT) in these patients.

Methods: With approval of local ethical committee, CP patients (1-15 years) treated in pediatric neurology clinics in a certain period were enrolled in the study. All patients were examined neurolagically and genitally, blood samples were collected for measurment of follicule stimulating hormon, luteinizing hormon, free and total testosteron hormon levels. Patients with and without UT were divided in 2 groups. Patients with history of hormonal therapy, inguinoscrotal surgery and diseases other than CP and UT were excluded. Results were statistically evaluated.

Results: UT was detected in 24 (40%, Group 2) among 60 CP patients that tetraparesic type consisted the majority (61.6%). Mean ages for Group 1 and Group 2 were 6.3 and 5.8 years, respectively. Three testes (2.5%) were atrophic, 5 (4.2%) were nonpalpable. Eleven patients had unilateral, 13 had bilateral UT. Comparison of groups’ hormon levels was statistically insignificant.

Conclusion: UT is very common among CP patients. Hormonal anomalies are insufficient to clarify the etiology. This may indicate other neurological causes such as increased muscular tonus in these patients. Further studies are needed.

SEREBRAL PALSİLİ ÇOCUKLARDA İNMEMİŞ TESTİS SIKLIĞI VE ETİYOLOJİSİ

Amaç: Çocuklarda en sık görülen nörolojik defisit nedeni olan serebral palsi (SP) hastalarında testis problemleriyle ilgili çok az çalışma mevcuttur. Bu hastaların izlemlerinde özellikle genital muayene önem sıralamasında geri planda kalmaktadır. Bu çalışmada SP’li çocuklardaki inmemiş testis (İT) sıklığı ve etiyolojisinin ortaya konması amaçlanmıştır.

Yöntem: Lokal etik kurul onayı alınarak Çocuk Nörolojisi polikliniklerinde belirli bir dönem ayaktan tedavi edilen 1-15 yaş arası SP olgusu çalışmaya dahil edildi. Tüm hastalar standart nörolojik ve ürogenital muayeneye tabi tutuldu, follikül stimüle edici hormon, lüteinizan hormon, serbest ve total testosteron hormon düzeyleri için kan örnekleri alındı. Olgular İT saptanmayan ve saptanan olarak 2 gruba ayrıldı. Hormon tedavisi almış, inguinoskrotal operasyon uygulanmış, SP VE İT dışında ek hastalığı olanlar çalışmaya dahil edilmedi. Sonuçlar istatiksel olarak değerlendirildi.

Bulgular: Çoğunluğu (%61,6) tetraparezik tip SP olan 60 olgunun 24’ünde (%40, Grup 2) İT mevcuttu. Yaş ortalamaları sırasıyla Grup 1 ve Grup 2 için 6,3 ve 5,8 idi. Testislerin 3’ü (%2,5) atrofik, 5’i (%4,2) nonpalpabl idi. Olguların 11’inde unilateral, 13’ünde ise bilateral İT mevcuttu. Hormon değerlerinin gruplara göre karşılaştırılmasında istatistiksel fark saptanmadı. 

Sonuç: SP’li çocuklarda inmemiş testis çok sık görülmektedir. Bu hastalardaki hormonal anormallikler bu durumu açıklamakta yetersiz kalmaktadır. Etiyolojide hastalığın doğası gereği artmış kas tonusu gibi olası başka nedenler rol oynuyor olabilir. Bu konu ile ilgili ileri çalışmalara ihtiyaç vardır.
