TRAUMATIC RUPTURE OF CHOLEDOCHAL CYST: CASE REPORT

Aim: Traumatic rupture of choledochal cyst (CC) is extremely rare in children and only two cases have been reported. An 11-year-old girl who developed acute abdomen after an accidental fell over an iron bar is reported to discuss the diagnostic features and treatment options of traumatic rupture of CC in children.

Case report: The patient admitted to our emergency department with a history of accidental fell over an iron bar. She had compliant of vomiting and abdominal pain. Physical examination showed tenderness at right upper quadrant. Ultrasonographic evaluation revealed free fluid accumulation and a 110x90 mm cystic mass adjacent to liver and pancreas. The patient underwent computed tomography with a presumptive diagnosis of liver laceration. CT showed ruptured type IV CC and liver laceration. During laparotomy, bile stained hemoperitoneum was detected. Cholecystectomy, total excision of cyst and hepaticojejunostomy were performed. The patient was discharged from the hospital after an uneventful 10 days period.

Conclusion: Traumatic rupture is the rarest presentation of CC in children. Patients can be misdiagnosed as liver hematoma. Traumatic rupture of CC should be kept in mind in patients with free fluid and cystic mass in radiologic evaluation. Total excision of the cyst with hepaticojejunostomy is the choice of treatment.

 

TRAVMATİK KOLEDOK KİST RÜPTÜRÜ: OLGU SUNUMU

Amaç: Travmatik koledok kist (KK) rüptürü çocuklarda çok nadir olup yanlız iki olgu rapor edilmiştir. Demir levha üstüne düşme sonrası akut batın bulguları ile başvuran 11 yaşında kız hasta, KK rüptürünün tanı özellikleri ve tedavi seçenekleri tartışılmak üzere sunulmuştur.

Olgu sunumu: Onbir yaşında kız hasta acil servise demir levha üzerine düşme sonrası kusma ve karın ağrısı şikayeti ile başvurdu. Fizik incelemede sağ üst kadranda hassasiyet mevcuttu. Ultrasonografik değerlendirmede yaygın serbest sıvı ve 110x90 mm boyutlarında, karaciğer ve pankreasa komşu kistik kitle saptandı. Hastaya karaciğer laserasyonu ön tanısıyla yapılan bilgisayarlı tomografide rüptüre tip IV KK ve karaciğer laserasyonu izlendi. Laparatomide safra boyalı hemoperitoneum gözlendi. Kolesistektomi, total kist eksizyonu ve hepatikojejunostomi yapılan hasta postoperatif 10. günde sorunsuz olarak taburcu edilmiştir.

Sonuç: Travmatik KK rüptürü çocuklarda KK’lerinin en nadir klinik bulgusudur. Hastalar yanlışlıkla karaciğer hematomu olarak değerlendirilebilir. Radyolojik incelemede serbest sıvı ve kistik görünüm varlığında rüptüre KK akılda tutulmalıdır.  Bu olgularda tedavi seçeneği total kist eksizyonu ve hepatikojejunostomidir.
