Complex Fetal Ovarian Cysts Can Cause Problems Even After Regression

Purpose: Fetal ovarian cysts’ size and nature are considered as the most important factors for determining the treatment modality. When to perform an intervention and to which fetal ovarian cysts has not been clarified yet. We herein present our experience with fetal ovarian cysts.

Methods: The prenatal and postnatal hospital records of 38 ovarian cysts were evaluated retrospectively.

Results: Prenatally at the 37 fetuses, 38 fetal ovarian cysts were evaluated retrospectively. There were 27 simple and 11 complex fetal ovarian cysts followed by prenatally and postnatally. None of the simple fetal ovarian cysts underwent in utero torsion. Postnatally, 23 simple ovarian cysts were regressed within 3 months. Four patients with simple ovarian cysts larger than 4 cm were operated. During follow up period, 7 complex ovarian cysts regressed, one complex ovarian cyst complicated with intestinal obstruction underwent surgery, 3 complex ovarian cysts did not regress and they were operated. Three out of 7 complex ovarian cysts that were initially regressed presented with intestinal obstruction and ooferectomy plus adhesiolysis were performed. Surgical treatment was required in 7 out of 11 (64%) complex ovarian cysts.

Conclusion: Complex ovarian cysts lead to problems even after regression in the postnatal period and require operative intervention sooner or later.

Kompleks fetal over kistleri regrese olsalar bile, cerrahi sorunlara sebep olabilirler

Amaç: Fetal over kistlerinin boyutu ve yapısı tedavi yöntemini belirlemede en önemli faktör olarak kabul edilir. Ne zaman ve hangi fetal over kistlerine girişim yapılacağı günümüzde halen tartışılmaktadır. Fetal over kistlerinin tedavisine ilişkin deneyimlerimizi sunmak istiyoruz.

Gereç ve Yöntem: Hastane kayıtlarından geriye dönük olarak 38 fetal over kistinin prenatal ve postnatal sonuçları değerlendirildi.

 Bulgular: Prenatal dönemde 37 fetüsde 38 fetal over kisti değerlendirildi. Prenatal ve postnatal dönemde 27 basit ve 11 kompleks fetal over kisti izlendi. Basit over kistlerinin hiçbiri intrauterin döneme torsiyona uğramadı. Postnatal dönemde, 23 basit kist 3 ay içinde geriledi. 4 cm den daha büyük kisti olan 4 hasta ise opere edildi. İzlem süresince 7 kompleks fetal over kisti geriledi, 1 kompleks fetal over kisti intestinal obstrüksiyona neden olarak komplike olunca opere edildi. Diğer 3 kompleks fetal over kisti ise boyutlarında artış olması nedeniyle opere edildi. Başlangıçta gerilemiş olan 7 kompleks fetal over kistinden 3’ü daha sonra intestinal obstrüksiyona neden olduğundan bu hastalar opere edilerek ooferektomi ve adezyolizis uygulandı. 11 kompleks fetal over kistinin 7’sinde (%64) cerrahi tedavi gerekli oldu.

Sonuç: Kompleks fetal over kistleri geriledikten sonra bile postnatal dönemde probleme neden olarak er ya da geç operasyon gerektirir.
