PENILE SOLITARY FIBROUS TUMOR

Introduction: Solitary fibrous tumor (SFT) is a rare tumor that originates most commonly from the pleura. Solitary fibrous tumor of penis has not been reported in English literature to date.

Aim: A child with penile solitary fibrous tumor is presented who was diagnosed hemangioma at first and he did not benefit from the treatment of propranolol and intralesional steroid injection and then his tumor was excised.

Case report: A 1,5 year-old boy was referred to our clinic for penile swelling. There was mass extending from glans tip to the radix of penis. The diagnosis of biopsy, which was taken from the mass at other clinic ten months ago, was epitheloid hemangioma. Propranolol was given for three months but the mass did not regress. Intralesional steroid injected because of not decreasing of lesion. Complete resection of the tumor was performed because it was not regressed in spite of both medical treatments (5x4x3 cm). Histological and immunohistochemical examinations established the diagnosis of solitary fibrous tumor compatibly with mesenchymal tumor. Chemotherapy was given because there were tumor cells at excision margin. The patient is well for a year.

Conclusion: The definitive diagnosis of solitary fibrous tumor is made only by histopathologically. If there is tumor at the excision margin, adjuvant chemotherapy and/or radiotherapy are supposed even though the histological diagnosis was not malign definitely. A prolonged follow-up is necessary because there is recurrence risk even in the late period.

 

 

PENİSTE SOLİTER FİBRÖZ TÜMÖR

Giriş: Soliter fibröz tümör (SFT) en çok plevradan gelişen nadir görülen bir tümördür. Peniste soliter fibröz tümör tanısı İngilizce literatürde şimdiye kadar bildirilmemiştir.

Amaç: Başlangıçta hemanjiom tanısı alan, propranolol ve intralezyonal steroid enjeksiyonundan yararlanmayınca eksizyon uygulanan penil soliter fibröz tümörlü çocuk sunulmuştur.

Olgu sunumu: 1,5 yaşındaki erkek çocuk peniste şişlik yakınması ile başvurdu. Muayenede, penis ucundan penis köküne kadar uzanan kitle saptandı. On ay önce başka bir merkezde kitleden alınan biyopsinin tanısı epiteloid hemanjiom idi. Bu tanı ile hastaya üç ay propranolol tedavisi verildi. Kitlede küçülme olmadığı için intralezyonal steroid uygulandı. Kitle  yine küçülmeyince cerrahi olarak total çıkarıldı (5x4x3 cm). Kitlenin histolojik ve immünohistokimyasal incelemeleri sonucunda soliter fibröz tümör ile uyumlu mezenkimal tümör tanısı konuldu. Cerrahi sınırda tümör bulunması nedeniyle hastaya kemoterapi başlandı. Bir yıldır takip edilmekte olan hasta halen iyi durumda.

Sonuç: Soliter fibröz tümörün kesin tanısı ancak histopatolojik inceleme ile konabilmektedir. Histolojik olarak kesin malign denmese bile, cerrahi sınırda tümör olan soliter fibröz tümörler için ek olarak kemoterapi ve/veya radyoterapi de önerilir. Geç dönemde bile yineleme riski olduğundan, bu tür hastaların uzun süreli takibi gereklidir.
