A NOVEL MUTATİON İN MNX1 GENE İN A CURRARİNO SYNDROME PATİENT WİTH ANORECTAL MALFORMATİON: A CASE REPORT

Currarino syndrome (CS) (OMIM#176450) is an autosomal dominantly inherited disorder. Its prevalence has been estimated to be 1–9:100,000. CS has been associated with the motor neuron and pancreas homeobox1 (MNX1) gene mapped on chromosome 7q36.3. Mutations of the MNX1 gene had been reported in almost all familial cases with CS, and in approximately 30% of cases displaying the sporadic form of CS.

In this case, we found a heterozygous novel mutation [c.402del (p.Gly136Alafs*86)] in MNX1 gene in a 3 generation family with CS. While, some of the members of the family have been manifesting all of the CS findings, only mild symptoms were detected in parents who inherit the mutation because of variable expression of CS.

The risk of malignant transformation of presacral teratomas has been estimated at around 1% of CS cases. So, it has been suggested to perform mutation analysis both in CS patients and asymptomatic family members in order to give a better genetic counseling.

ANOREKTAL MALFORMASYONLU CURRARİNO SENDROMLU BİR HASTADA MNX1 GENİNDE YENİ BİR MUTASYON: OLGU SUNUMU

Currarino Sendromu (CS) (OMIM#176450), otozomal dominant olarak kalıtılan bir hastalıktır. CS’nin sıklığının 1–9:100,000 arasında olduğu bildirilmiştir. CS ile ilgili gen, 7q36.3 bölgesinde lokalize olan motor nöron ve pankreas homeobox1 (MNX1) genidir. MNX1 genindeki mutasyonlar ailesel CS vakalarının tümünde, sporadik olgularda ise ortalama %30 oranında bildirilmiştir.

Burada sunulan 3 jenerasyonlu CS’lu bir aileye yapılan mutasyon analizinde MNX1 geninde heterozigot mutasyon [c.402del (p.Gly136Alafs*86)] saptanmıştır. Ailenin kimi bireylerinde sendromun tüm bulguları görülürken mutasyonu aktaran ebeveynlerde değişken ekspressivite nedeniyle daha hafif bulgular olduğu gözlenmiştir.

CS olgularında presakral teratomların malign transformasyon riskinin %1 oranında olduğu tahmin edilmektedir. Bu nedenle, CS hasta bireylerle birlikte asemptomatik aile bireylerinin de taranması doğru bir genetik danışma verebilmek için gereklidir.
