Introduction: Hydatid cyst caused by Echinococcus granulosus is an endemic disease in Turkey. The most common organs involved by such cysts are liver and lung. Here is  presented a case of a primary hydatid cyst with unusual location in retrovesical region.

Case: A 12-year-old boy was admitted with the complaints of severe lower abdominal pain and vomitting. Physical examination revealed abdominal tenderness on right lower quadrant. US imaging suggested a cyst with septa about 9cm in diameter in retrovesical region, the suggestive of hematoma. Patient was operated duo to plastrone appendicitis. During the surgery, a cystic retrovesical mass of about 10cm in diameter was found and excised totally. A disease of hydatid cyst was diagnosed after histologic investigation.

Conclusion: In cases admitted with the complaint of abdominal pain and diagnosed with a retrovesical cyst in US imaging, the existence of hydatid cysts should be considered in such cases in endemic region.

 Giriş: Kist hidatik Echinococcus granulosus'un neden olduğu, Türkiyede endemik bir hastalıktır. En yaygın organ tutulumu karaciğer ve akciğerde görülür. Biz burada primer kist hidatiğin alışılmadık bir lokalizasyonda, retrovezikal bölgede olabileceğini sunuyoruz.

Olgu: 12 yaşında erkek hasta alt batında şiddetli ağrı, kusma şikayeti ile getirildi. Fizik muayenede sağ alt kadranda hassasiyet mevcuttu. USG'de retrovezikal septalı 9cm çapında kistik kitle (hematom?) olarak rapor edildi. Olgu plastrone apandisit öntanısı ile opere edildi. Operasyonda retrovezikal yaklaşık 10cm çapındaki kistik kitle total olarak çıkarıldı. Patolojik inceleme sonucu kist hidatik tanısı koyuldu.

Sonuç: Endemik bölgelerde karın ağrısı şikayeti ile gelen ve USG' de retrovezikal kist tespit edilen olgularda, atipik lokalizisyonda kist hidatik olabileceği düşünülmelidir.
