Congenital Midureteric Stricture - Case Report

INTRODUCTION: The most common causes of prenatal hydronephrosis are vesicoureteral reflux and ureteropelvic junction obstruction. Ureterovesical junction is another frequent localization for ureteric disruption. Midureteric strictures are hardly ever seen in the literature. In this study, we report a congenital midureteric obstruction case who is diagnosed and treated in our clinic.

CASE: Increase in hydronephrosis and renal function regression were detected in postnatal ultrasonographic and scintigraphic evaluation of our patient in follow-up due to the diagnosis of prenatal left hydronephrosis at 28th gestational week. Case has been considered as ureteropelvic junction obstruction and operation was planned at postnatal 5th month. The exploration revealed a stenotic segment at 2cm distal to the ureteropelvic junction which couldn’t be catheterised with a 3Fr ureteric stent. Following our intraoperative diagnosis of total obstruction, excision and ureteroureterostomy was performed. Pathological examination revealed a lumen structure accompanied with proliferation of vasculer structures and thickening of the muscular layer in that field.

Ureteral stent was removed at postoperative 1st month. Ultrasonography at 3rd month revealed remission of antero-posterior pelvic diameter to 10 mm from the preoperative value of 25 mm. The patient is still in our follow-up.

DISCUSSION AND CONCLUSION: In ureteric stricture which causes antenatal hydronephrosis, we  should consider that the lesion can be at any segment of the ureter. Midureteric strictures can be treated successfully with excision and ureteroureterostomy.

Konjenital Midüreterik Darlık - Olgu Sunumu

GİRİŞ: Antenatal hidronefrozların en sık sebepleri vezikoüreteral reflü ve üreteropelvik bileşke darlığıdır. Üreter darlıklarının bir diğer sık lokalizasyonu üreterovezikal bileşke olup mid-üreterik konjenital darlıklara literatürde oldukça nadir rastlanmaktadır. Bu çalışmada kliniğimizde tanı ve tedavisi uygulanan bir midüreterik darlık olgusu sunulmuştur.

OLGU: Antenatal 28. haftada saptanan sol hidronefroz nedeniyle takipte olan olgunun, postnatal dönemde yapılan ultrasonografik ve sintigrafik değerlendirmesinde hidronefrozda ilerleme ve renal fonksiyonda azalma saptanmıştır. Olgu postnatal 5. ayda üreteropelvik bileşke darlığı öntanısıyla kliniğimizde opere edilmiştir. Yapılan eksplorasyonda üreteropelvik bileşkenin 2 cm distalinde 3 Fr stentin geçişine izin vermeyen segment saptanmış ve tam obstrüksiyon düşünülerek bu segment eksize edilip üreteroüreterostomi uygulanmıştır. Patolojik incelemede lümen yapısının mevcut olduğu ancak aynı alanda vasküler yapıların prolifere olduğu kas tabakasının kalınlaştığı tespit edilmiştir.

Operasyondan 1 ay sonra üreteral stenti çekilen olgunun 3.ay kontrolünde preoperatif dönemde 25mm’ye kadar ulaşan renal pelvis antero-posterior çapın 10mm’ye düzeldiği izlenmiştir. Olgu halen takibimizdedir.

TARTIŞMA ve SONUÇ: Antenatal hidronefroza sebep olabilecek üreter darlıklarında lezyonun üreterin her segmentinde olabileceği akılda tutulmalıdır. Mid-üreterik darlıklar patolojik segmentin eksizyonu ve üreteroüreterostomi ile başarılı bir şekilde tedavi edilebilmektedir.
