Delayed diagnosis of huge neuroblastoma presenting with rupture and bleeding following a minor abdominal trauma

Aim: Neuroblastoma is one of the most common solid tumors in children. However, tumor rupture and intraperitoneal bleeding are very rare occurrences; it often develops following major traumas. This study presents a late-diagnosed case presenting with intraperitoneal and retroperitoneal hemorrhage due to ruptured neuroblastoma after minor trauma.

Case: A 7.5 year-old female patient admitted to hospital with complaints of abdominal pain after a minor blunt abdominal trauma 5 months ago. She underwent splenectomy upon diagnosis of spleen injury-splenic artery rupture at the regional hospital. Re-hospitalized one month ago due to convulsions and hypertension, the patient’s screening results revealed left hydronephrosis and hematoma and underwent hematoma drainage at the same hospital. Then she was referred to our hospital because of bleeding and declining hemoglobin values. Recurrent hematoma in control screenings were evaluated at the radiology clinic. Angiography was performed to exclude splenic artery bleeding. Embolization was performed upon suspected left 12th intercostal artery bleeding. Her follow-up results showed a hemoglobin value of  7,6 g/dL, hematocrit value of 22%, and a thrombocytes value reduced to 20.000 K/mm³ and unstable hemodynamic condition, after which the patient underwent laparotomy. A giant retroperitoneal mass that has pushed the stomach to right, the colon to anterior, small intestines to down and anterior, extending from posterior stomach to the pelvis was seen in the patient. Gastrocolic ligament was opened and 200cc hematoma was aspirated. The frozen examination of the biopsy specimen taken from the anterior wall of the mass revealed malignant tumor, the operation was terminated after bleeding control was achieved. The patient was followed on mechanical ventilation in the postoperative intensive care unit, and she was referred to the oncology clinic. Chemotherapy was started on the same day. The patient was separated from mechanical ventilator on 7th postoperative day. The histopathological examination revealed neuroblastoma. The patient continues to receive chemotherapy.

Conclusion: Solid tumors and secondary hemorrhagic conditions are very rare. However, tumor rupture and bleeding should be considered in cases where the anamnesis and clinical picture is inconsistent in mostly retroperitoneal bleedings.

Rüptür ve kanama ile seyreden ve geç tanı alan retroperitoneal nöroblastom olgusu

Giriş: Nöroblastom çocuklarda en sık görülen solid tümörlerden biridir. Bununla birlikte tümör rüptürü ve intraperitoneal kanama oldukça nadir görülür. Sıklıkla major travmalar sonrasında gelişir. Çalışmamızda minör travma sonrası gelişen nöroblastom rüptürüne bağlı intraperitoneal ve retroperitoneal hemoraji ile seyreden ve geç tanı alan olgu sunulmuştur. Olgu:7.5 yaşında kız hastaya 5 ay önce minör künt batın travması sonrası karın ağrısı şikayeti ile başvurdukları bölge hastanesinde dalak yaralanması - splenik arter kopması tanısı ile splenektomi yapılmış. 1 ay önce hipertansiyon nedeni ile tekrar hastaneye yatırılan hastanın tetkiklerinde sol hidronefroz ve hematom tespit edilmiş.  Aynı hastanede hematom drenajı yapılan hastanın hemoglobin değerlerinde düşme olması, görüntülemelerinde hematomun tekrarlaması üzerine hastanemize sevk edildi. Görüntüleme tetkikleri radyoloji bölümünde değerlendirildi. Splenik arter kanaması olabileceği belirtildi. Anjiografi yapıldı. Sol 12. İntercostal arter kanaması şüphesi ile embolizasyon yapıldı. Takiplerinde hemoglobinin 7,6 g/dL'e, hematokritin %22'e, trombositin de 20.000 K/mm³’e gerilemesi ve hemodinamin instabil olmaması üzerine laparotomi yapıldı. Hastada, mideyi sağa, kolonu anteriora ince barsakları aşağı ve anteriora itmiş, mide arkasından pelvise kadar uzanan retroperitoneal dev kitle görüldü. Gastrokolik bağ açılarak 200 cc hematom boşaltıldı. Kitleden alınan biyopsinin frozen incelemesi yuvarlak hücreli malign tümör olarak bildirilmesi üzerine kanama kontrolü yapılarak ameliyat sonlandırıldı. Hasta onkoloji bölümü ile konsülte edildi. Aynı gün kemoterapi başlandı. Kesin patoloji sonucu nöroblastom olarak rapor edilen hastanın kemoterapisine devam edilmektedir. Sonuç: Solid tümörler ve bunlara ikincil hemorajik tablolar nadir görülür. Bununla birlikte ağırlıklı olarak retroperitoneal yerleşimli kanamalarda, anamnez ve klinik tablonun uyumsuz olduğu olgularda tümör rüptürü ve kanaması akla getirilmelidir.
