PROTECTIVE EFFECT OF KEFIR PROPHYLAXIS IN EXPERIMENTAL NECROTISING ENTEROCOLITIS

In recent years, probiotics are shown to be effective in prevention of NEC of premature infants in clinical researches. Kefir is a fermented dairy product embedded within polysaccharide and protein matrix, formed by bacterial and fungal activity. In this study we aimed to evaluate whether kefir prophylaxis can prevent NEC as a combination of probiotics or not.

Methods: Wistar-Albino rat pups were randomized into 4 groups on the 1st day of their lives. Pups in Group 1, 2 and 3 were taken away from their mothers and kept in an incubator for adjusted environment in means of temperature and humidity, throughout the study period. Each of these pups were fed with 4x0,2-0,3 ml of preterm formula prepared according to the study protocol. Group 1 (n=8) received preterm formula+kefir, Group 2 (n=8) received preterm formula+ Bifidobacterium Lactis, Group 3 (n=8) received preterm formula. Control group (Group 4) (n=6) were kept with their mother. Hypoxia-ischemia + cold injury was performed to create experimental NEC model, to all pups twice a day. All were sacrificed on the 4th day of the study. Tissues obtained were evaluated by pathological and biochemical tests.

Results: Tissue examples were examined with TUNNEL method for examining apopitotic cell count in 1000 consecutive cells. Apopitotic cell count was 7,7±2,9 and 7,6±2,3 in Group 1 and 2, respectively (p>0,05), which was lower than 8,8±3,5 of Group 3. As awaited, it was significantly lower in control group 2,5 ± 1,3 (p=0,001). Among biochemical parameters evaluated; TNF-α level was lowest in Kefir group (668,9 pg/ml), while PAF level was lowest in Probiotic group (1348,5 pg/ml). Total oxidant and antioxidant status of tissues were evaluated with TOS and TAS, but there was no significant difference among groups.

Discussion: Kefir prophylaxis can decrease apopitosis of intestinal epithelium and can result in partial attenuation in NEC tissue indicators, in experimental NEC model. Kefir+pretem formula is as effective as Bifidobacterium+preterm formula. Kefir, which is a traditional probiotic, shows a similar effect with bifidobacterium which has been demonstrated for its protective effects in NEC in both animal and premature infant studies.

DENEYSEL NEKROTİZAN ENTEROKOLİT MODELİNDE KEFİR PROFİLAKSİSİNİN KORUYUCULUĞU

Giriş: Klinik çalışmalarda probiyotiklerin prematüre bebeklerde kullanımının NEK gelişimini önlediği gösterilmiştir. Kefir polisakkarit ve protein matriks içine hapsedilmiş, laktik asit bakterileri ve mayaların aktiviteleri ile oluşturulan fermente edilmiş bir süt ürünüdür. Bu çalışmada kefirin NEK’i engelleyici bir probiyotik kombinasyonu olarak kullanılıp kullanılamayacağını araştırmayı amaçladık.

Metod: Dört gruba ayrılan 0-24 saatlik Wistar-Albino ratlardan Grup 1, 2 ve 3’deki yavrular anne yanından ayrılarak ısı ve nemi ayarlanmış küvöz içine alındı ve herbiri 4x0,2-0,3 ml/gün hacimlerle, çalışma protokolüne uygun olarak hazırlanan mamalarla; Grup1 (n=8) prematür formula  + kefir, Grup2 (n=8) prematür formula + Bifidobacterium Lactis ile, Grup3 (n=8) prematür formula ile beslendi. Grup 4 (Kontrol) (n=6) ise annelerinin yanından ayrılmadı. Tüm gruplara günde iki defa hipoksi-iskemi + soğuk hasarı uygulanarak deneysel NEK modeli oluşturuldu. Bütün hayvanlar çalışmaya dahil edildikten sonra 4.gün sakrifiye edildi. Alınan doku örnekleri patolojik ve biyokimyasal yöntemlerle değerlendirildi.

Sonuçlar: Elde edilen doku örneklerinin TUNNEL yöntemi ile bakılan 1000 ardışık epitel hücresindeki apopitoz sayıları Grup 1 ve 2’nin sırası ile  7,7±2,9 ve 7,6±2,3 idi (p>0,05). Her iki grubun apopitoz değeri Grup 3’ün 8,8±3,5 değerinden daha düşüktü, ancak istatistiksel farklılığa ulaşmamıştı. Beklendiği gibi sadece AS ile beslenen grubun apopitoz indeksi 2,5 ± 1,3 ile tüm gruplara göre anlamlı olarak daha düşüktü (p=0,001). Biyokimyasal olarak değerlendirilen TNF-α düzeyi 668,9 pg/ml ile en düşük kefir grubunda, PAF düzeyi ise 1348,5 pg/ml ile probiotik grubunda elde edilmiştir. Dokulardaki toplam oksidan ve antioksidan aktiviteler TOS ve TAS ile değerlendirilmiş, gruplar arasındaki fark istatistiksel anlamlılığa ulaşmamıştır.

Tartışma: Deneysel NEK modelinde, kefir profilaksisi, tek başına prematüre mamasının neden olduğu intestinal epiteldeki apopitozu, NEK’in doku göstergelerini kısmen azaltmakta Kefir+prematüre maması, daha önce defalarca başarısı gösterilmiş bifidobakter+prematüre maması kadar etkindir. Geleneksel bir probiyotik olan kefir, literatürde NEK’ten koruyucu özelliği hem hayvan hem de prematüre bebeklerde gösterilmiş olan bifidobakteriye benzer etkinlik göstermektedir.
