Laparoscopic resection and pyeloplasty in a rare cause of ureteropelvic junction obstruction: Ureteral Polyp

Ureteropelvic Junctıon Obstructıon (UPJO) commonly arises from intrinsic and extrinsic pathologies. Endoluminal pathologies such as ureteral polyps are extremely rare cause of UPJO. In this video, we would like to present our approach to the UPJO and hydronephrosis due to a ureteral polyp in a child. Video presentation: A ten year old boy was admitted to our department because of intermittent left abdominal pain. Hydronephrosis and a ureteral polyp with 20 mm diameter close to the ureteropelvic junction was detected by ultrasound. Laparoscopic transperitoneal resection of ureteropelvic segment including ureteral polyp and pyeloureterostomy was performed. Up to the four months of the postoperative follow up, he did not have any significant problem. Conclusion: Minimal invasive approaches could be alternative to the open surgery for the ureteral polyps. Intraluminal endoscopic resection could be preferred, but it has some limitation due to the size and broad base settlement of the polyp. As an alternative procedure, laparoscopy should be considered with the advantage of providing a better surgical exposure and good cosmetic outcome.

Nadir Bir Üreteropelvik Darlık Nedeni Üreteral Polipte Laparoskopik Eksizyon ve Piyeloplasti

Üreteropelvik darlık, ekstrensek ve intrensek nedenlere bağlı gelişir. Üreteral polip gibi endoluminal nedenler çok nadirdir. Bu videoda, üreteral polip nedeniyle üreteropelvik darlık ve dolayısı ile hidronefroz gelişen olgudaki cerrahi yaklaşımımız sunulmaktadır. Video Sunumu: Aralıklı sol yan ağrısı yakınması ile başvuran 10 yaşındaki erkek hastada, ultrason tetkikinde üreteropelvik bölge yerleşimli, yaklaşık 20 mm çapta, üreteral polibe bağlı hidronefroz saptandı. Olguya laparoskopik transperitoneal yaklaşımla, polip içeren üreteropelvik segmentin eksizyonu ve pelviüreterostomi yapıldı. Postoperatif 4. ayında son kontrolü yapılan hastanın takibinde sorun tespit edilmedi. Sonuç: Üreter poliplerinde açık cerrahi girişim haricinde minimal invaziv tedavi alternatifleri de düşünülebilir. Tedavi seçeneklerinden biri intraluminal endoskopik rezeksiyondur.  Ancak lezyonun boyutları ve oturduğu taban alanı endoskopik girişimin uygulanabilirliğini sınırlar. Diğer bir seçenek olan laparoskopik rezeksiyon ise gerek sağladığı iyi görüş alanı, gerekse kozmetik sonuçları ile alternatif bir yöntem olarak akılda tutulmalıdır.
