Repetitive pancretic surgery for persistent hypoglycemic symptoms in two siblings due to congenital hyperinsulinism

İntroduction: Hypoglycemia is the most common metabolic problem in infants with a incidence of 4-6 per 1000 newborns. It is usually transient and easily treated, but in fewer than %1, congenital hiperinsulinism presents an important cause. Early diagnosis and treatment is mandatory to prevent a broad range of complications including neurological damage and life-threatening events. We presented the case of two siblings, previously diagnosed and treated for CHI with surgery, who subsequenty required repetitive pancreatectomies to correct persistent hypoglycemia symptoms.

Case 1: A 20-month-old female children was admitted to our institution with persistent hypoglycemia that was diagnosed as CHI during the work-up for convulsion at the age of 2-months. She had undergone %50-pancreatectomi in an out-center, 11 months before her admission. In our clinic, she underwent subtotal (%98) pancreatectomy, Postoperatively,  she was hyperglysemic only for 1 week. However, 40 days after this procedure, the patient was reoperated for residual pancreatic tissue and a thin rim was left along to the duodenum and pancreoticoduodenal arteries.  The patient was readmitted for seizures and hypoglycemia 2 years after this last operation. Her symptoms were resistant to treatment with medications. After excision of the last pancreatic tissue parts, she developed diabetes and required insulin to control blood glucose.

Case 2: A  8-year old male children was admitted with persistent hypoglycemic symptoms, he was diagnosed as CHI during the work-up for convulsion at the age of 10-months. He had undergone %65 pancreatectomi in an outcenter, 2 years before his admission. A small tissue was left along to the duodenal arcus. After the operation he had hypoglycemic attacks for 4-weeks and then started hypoglicemia. At this time his glucose level is  regulated with an adequate insulin program.

Conclusion: Congenital hyperinsulinism is a condition that is characterized with a variety of genetical disorders in which hypoglycemia is caused by excessive insulin secretion from the pancreatic beta-cells. Diagnosis is usually made by hypoglycemic symptoms. It has a broad clinical spectrum but there are two main forms, diffuse and focal. The distinction between this forms is important because treatments are different. In focal forms, a limited excision is curative and can prevent unnecessary resections, but the diffuse form may require a more extended resection and remains risk for diabetes mellitus. Taking multiple biopsies from different parts of the pancreas during the operation can help determine the form of the pathology. Developing more sensitive imaging modalities can help to differentiate focal and diffuse forms,  new medications can improve the prognosis in intractable cases to surgery.

 

Konjenital hiperinsülinizme bağlı sebat eden hipoglisemik semptomlar nedeniyle tekrar pankreatik cerrahi gerektiren iki kardeş olgusu

Giriş: Hipoglisemi bebeklerin en sık metabolik sorunu olup 1000 doğumda 4-6 sıklığında görülür. Genellikle geçici, kolay düzeltilebilir olup %1 inden azında konjenital hiperinsülinizm (KHI) önemli bir neden olarak karşımıza çıkar. Beyin hasarı dahil yaşam tehdit edici durumlar içeren komplikasyonların önlenmesi için erken tanı ve tedavi zorunludur. Daha önce KHI tanısı alan ve cerrahi uygulanmış, ancak hipoglisemik semptomların devam etmesi sebebiyle tekrarlayan pankreatektomiler gerektiren iki kardeş olgu sunulmuştur.

Olgu 1: 20 aylık kız çocuk medikal tedaviye dirençli hipoglisemi ile getirildi. 2 aylıkken konvulziyon atağı sonrası tanı aldığı ve bir yıl önce dış merkezde %50 olduğu öğrenildi. Hasta operasyona alınarak totale yakın ( %98 ) pankreatektomi yapıldı. Bir haftalık hiperglisemik dönem sonrası yeniden hipoglisemi gelişince 40 gün sonra reopere edilerek, koledok çevresi rezidüler çıkarıldı ve duodenal arkus içinde küçük bir doku bırakıldı.  Hastanın 2 yıl sonra hipoglisemik nöbetlerinin başlaması ve ilaçlarla kontrol edilememesi nedeniyle tekrar opere edilerek duodenum arkusundaki tüm dokular çıkarıldı. Ameliyat sonrası kan şekeri 300mgdL seviyesinde seyreden hasta uygun bir insulin programıyla takip edilmektedir.

Olgu 2: Sekiz yaşında erkek çocuk,  10 aylıkken konvulziyon atağı sonrası tanı aldığı ve dış merkezde yaklaşık % 65 parsiyel panktreatektomi’ den sonra sürekli hipoglisemi atakları olduğu gerekçesiyle hastanemize sevk ile getirildi.Hastaya totale yakın pankreatektomi yapıldı. Duodenal arkus içinde çok az bir doku kaldı. Postoperatif dönemde hipoglisemi atakları oldu . Bir ay sonra ise,  hiperglisemi atakları başladı. Halen insulin ile şekeri  düzenlenmekte.Her iki hastamızda da genetik analiz ve F-floro-L DOPA pozitron emisyon tomografisi ile diffüz-fokal HI ayrımı yapılmadı.

Sonuç: KHI  pankreatik beta hücrelerinin çok çeşitli genetik bozukluklara ikincil olarak gelişen insulin salınımıyla karakterizedir. Yaklaşık 1/50.000 doğum olarak bildirilir. Tanı hipoglisemik semptomlar ile konur. Çok heterojen bir klinik spektruma sahip olmakla birlikte histolojik olarak fokal ve diffuz iki forma ayrılır. HI halen üzerinde her kliniğin farklı tedavi stratejileri olduğu heterojen ve geniş bir hastalık grubunu oluşturmaktadır. Fokal şeklinde odağın çıkarılması küratif iken, tıbbi tedaviye yanıt vermeyen diffüz olgularda totale yakın pankreatektomi  diabet gelişimine yol açabilmektedir. Aşırı  pankreatektomileri engellemek için tüm pankreas yüzeyinden multipl biyopsiler alınması fokal form tanısını ve bu odakların çıkarılmasını kolaylaştırır. Yaygın ve lokal lezyonların tanınması için daha duyarlı sintigrafik tetkiklerin geliştirilmesi ve cerrahinin küratif olmadığı durumlarda kullanılmak üzere yeni ilaçların kullanıma girmesi prognozu olumlu yönde etkileyecektir.
