DIFFERENT COMPOSITIONS OF PARENTERAL NUTRITION SOLUTION DO NOT AFFECT THE INCIDENCE BUT OCCURRENCE TIME OF CHOLESTASIS IN SURGICAL INFANTS

Background/purpose: Parenteral nutrition (PN)-associated cholestasis (PNAC) is one of the most important complications of PN. We aimed to define the precipitating factors for PNAC in infants received two different PN regimens, retrospectively.

Material and method: Two different PN regimens, January 2000- May 2008 (Group 1) and June 2008- May 2012 (Group 2), were evaluated. Patients with and without PNAC (Group 1a-2a) and controls (Group 1b-2b) were evaluated for gender, body weight (BW), gestational age (GA), age at onset of PN, time to PNAC, duration and management of PNAC, composition and caloric content of PN solution and sepsis status. 

Results:  Group 1 (n=109) and Group 2 (n=46), and 1a (n=13), 1b (n=32), 2a (n=6) and 2b (n=8) were constituted. The incidence of PNAC was 14.1% (n=22) and the type of PN regimen didn’t affect the incidence (14.6% vs. 13.0%, p=0.39). GA didn’t differ in groups except lower GA in Group 1b (36.0±2.5weeks) than 1a (36.0±1.8weeks) (p=0.04). Gender and BW didn’t differ (p>0.05). PNAC developed later in Group 2a than 1a (42.0±24.6 vs. 9.0±8.3day, p=0.02). Duration of PNAC didn’t differ (17.7±21.3 vs. 33.2±29.9days, p=0.27). Lipid content was higher in Group 1a than 1b (3.5±0.7 vs. 3.45±0.54g/kg/day, p=0.001) and 2a (2.2±0.4g/kg/day, p=0.01). Lipid dosage was higher in Group 2a than 2b (2.2±0.4 vs. 1.3±0.5g/kg/day, p=0.001). Sepsis was significant co-morbidity in Group 2a (p=0.04). 

Conclusion: Two different regimens resulted in similar PNAC rates. Composition of PN solution affects time to PNAC but not duration of PNAC. Lower lipid concentration is safer through delayed development of PNAC. High caloric and lipid content of PN solution may play role in the development of PNAC. Sepsis usually appears as a concomitant disease. PNAC should be noticed as early as possible and treated promptly.

CERRAHİ İNFANTLARDA PARENTERAL BESLEME KARIŞIMI İÇERİĞİ PARENTERAL BESLEME İLİŞKİLİ KOLESTAZ SIKLIĞINI DEĞİL KOLESTAZ ORTAYA ÇIKIŞ ZAMANINI ETKİLEMEKTEDİR

Giriş ve amaç: Parenteral beslenme (PB) ile ilişkili kolestaz (PBİK), PB’nin en önemli karmaşasıdır. İki farklı PB karışımı ile oluşan PBİK’ı etkileyen faktörler geriye dönük araştırılmıştır.

Gereç ve yöntem: Ocak 2000-Mayıs 2008 (Grup 1) ve Haziran 2008-Mayıs 2012 (Grup 2) dönemlerinde PB uygulanan olgulardan PBİK gelişenler deney (Grup 1a ve 2a), gelişmeyen olgular kontrol (Grup 1b ve 2b) gruplarını oluşturdu. Cinsiyet, vücut ağırlığı, gebelik yaşı, PB başlama yaşı, PB süresi, kolestazın ortaya çıkış zamanı, kolestaz süreci, sağaltımı, lipit, dekstroz ve protein dozu, günlük kalori ve sepsis durumu kaydedildi.

Bulgular: Grup 1’de 109, Grup 2’de 46, Grup 1a’da 13, 1b’de 32, 2a’da 6, 2b’de 8 olgu vardı. Genel PBİK sıklığı %14,1 (n=22), Grup 1a ve 2a’da ise %14,6 ve %13,0 bulundu (p=0,39).  Gebelik yaşı Grup 1b’de (36,0±2,5 hafta) Grup 1a’ya (36,0±1,8 hafta) göre daha düşüktü (p=0,04), diğer gruplarda farksızdı. Cinsiyet ve vücut ağırlığı farksızdı (p>0,05). PBİK Grup 2a’da (42,0±24,6gün) 1a’ya (9,0±8,3gün) göre daha geç gelişti (p=0,02).  Grup 1a (17,7±21,3gün) ve 2a’daki (33,2±29,9) PBİK süreleri benzerdi (p=0,27). Lipit dozu Grup 1a’da (3,5±0,8g/kg/gün), 1b (3,45±0,54 g/kg/gün) ve 2a’ya (2,2±0,4 g/kg/gün) göre yüksekti (p=0,001, p=0,01). Grup 2a’da lipit dozu 2b’ye (1,3±0,5 g/kg/gün) göre yüksekti (0,001). Grup 2a olgularında sepsis birlikteliği önemli bir komorbiditeydi (p=0,04). 

Sonuç: İki farklı karışım ile uygulanan PB’de PBİK sıklığı aynıydı. PB karışımı içindeki lipit ve kalori miktarı PBİK süresini değiştirmemekte, ortaya çıkış zamanını etkilemektedir. Düşük lipit miktarı PBİK gelişimini geciktirebilir ve daha güvenlidir. Sepsis uzun süreli PB verilen olgularda eşlik etmektedir. PBİK erken tanınıp sağaltılmalıdır.
