A rare form of Throglossal Duct Cyst: Double Thyroglossal Cyst

Thyroglossal cyst is the most common congenital mass in the neck. Cysts can be seen in any region between the base of the tongue and the supra-sternal region. They are painless cysts of soft consistency, which are movable with protrusion of the tongue and swallowing. We reported to double thyroglossal cyst that ıt is very rare.

Case Report: A 6-year-old male patent presented to our clinic with the complaint of swellings in the neck. The first swelling had appeared a year ago and the second swelling had appeared 3 months ago. The swellings had been growing intermittently and there was no pain or erythema. On physical examination of the patient, there were two painless, mobile masses of soft consistency in the midline of neck, the first in the infrahyoid region and the second, which was smaller than the first, in the suprahyoid region (Figure 1). The masses were mobile with protrusion of the tongue. First, neck ultrasonography (US) was performed. On US, there was an intense cystic lesion measuring 14x5.4x19 mm in size in the midline of the neck and superior thyroid isthmus, and there was another intense cystic mass at the superior of this region measuring 15.9x8 mm in size close to the root of the tongue. Complete blood count, biochemical and thyroid function tests were normal. Thyroid scintigraphy was requested in order to eliminate the possibility of ectopic thyroid tissue, on which the thyroid gland was determined to be normal. The patient was operated and the sistrunk procedure was performed. Perforation of the inferior cyst occurred during the operation. The other cyst was removed totally together with the hyoid bone corpus (Figure 2). The patient was discharged on the 3rd postoperative day uneventfully. We did not encounter recurrence during the one year follow-up.

Tartışma: Thyroglossal cyst is the most common congenital mass in the neck. However, double thyroglossal cysts are very rare and our case is the sixth case that has been reported. Imaging techniques are utilized to verify the diagnosis. Ultrasonography is generally used for this reason. However, in double cysts, we suggest that thyroid scintigraphy be used in order to differentiate these cysts from aberrant thyroid tissue.

Tiroglossal duktus kistinin nadir formu: Çift Tiroglossal Kist

Tiroglossal kist en sık görülen konjenital boyun kitlesidir. Kistler dil tabanı ile supra sternal bölge arasında herhangi bir bölgede görülebilir. Ağrısız, yumuşak kıvamlı kistler olup dil çıkarmak ve yutkunmakla hareketlidirler. Biz burada sonderece nadir görülen çift tiroglossal kistlerli hastamızı sunmak istedik.  Olgu sunumu:  Boyunda şişlik şikayeti ile 6 yaşında erkek hasta bize başvurdu. Birinci şişlik 1 yıl önce ve ikinci şişlik ise 3 ay önce meydana gelmiş. Şişlikler aralıklı olarak büyüyormuş, ağrı veya kızarıklık olmamış. Hastanın fizik muayenesinde boyun orta hattında birincisi infrahyoid bölgede olan ve ikincisi birinciden daha küçük suprahyoid bölgede olan ağrısız, mobil ve yumuşak kıvamlı kitle tespit edildi(Şekil 1). Kitleler dilin dışarı çıkartılması ile hareketliydi. Hastaya ilk planda boyun ultrasonografi(US)si yapıldı. US de boyun orta hattında troid istmus superiorunda 14x5.4x19 mm boyutunda yoğun içerikli kistik lezyon ve bu alanın superiorunda dil köküne yakın yerleşimli 15.9x8 mm boyutunda yoğun içerikli kistik kitle tespit edildi. Hastanın hemogram ve biyokimya tetkikleri ve troid fonksiyon testleri normaldi. Hastada ektopik troid dokusu ihtimali nedeni ile troid sintigrafisi istendi ve troid bezi normal olarak tespit edildi. Hasta ameliyata alınıp sistrunk operasyonu uygulandı. Operasyon esnasında inferiordaki kist perfore oldu. Diğer kist ise total olarak hyoid kemik korpusu ile birlikte çıkartıldı(Şekil 2). Hasta postoperatif 3. gün sorunsuz olarak taburcu edildi. Bir yıllık takibimiz esnasında nüks ile karşılaşmadık.

Tartışma: Tiroglossal kist en sık görülen konjenital boyun kitlesidir ancak çift tiroglossal kistler sonderece nadir görülüp bizim vakamız rapor edilen altıncı vakadır.  Görüntüleme tekniklerinden tanıyı doğrulam amacı ile yararlanılır. Bu amaçla sıklıkla ultrasonografi kullanılır. Ancak çift kistli olanlarda aberan troid dokusu ile ayrımın yapılması amacı ile troid sintigrafisinden yararlanmak gerektiğini düşünmekteyiz.
