SPONTANEOUS INTESTINAL PERFORATION (SIP): REPORT OF TWO CASES

INTRODUCTION

Spontaneous intestinal perforation (SIP) is particularly a disease of newborns with low-birth-weight. Although it is frequently misinterpreted as necrotizing enterocolitis (NEC), it is a different condition.  Most commonly the terminal ileum is involved and there is always a punctuate perforation. Correct diagnosis is of vital importance since treatment approach and prognosis are different from that of NEC.

 

CASE REPORT

Case 1: A term male baby with birth weight of 4250 g who hed been delivered in another healthcare facility was referred to our clinic at the age of 6 days due to absence of meconium discharge and

poor overall state. At presentation, he was a visibly sick baby having massive abdominal distension causing respiratory distress. Direct radiography revealed massive pneumoperitoneum and he underwent an emergency operation. There was a punctuate free perforation in the transverse colon. A primary repair with ileostomy was performed and intestinal biopsies were taken. He died at the postoperative 10th hour due to progressive septic shock. On histopathological examination, ganglion cell was noted in all specimens.

Case 2:  The term male baby with birth weight of 3260 g, who was delivered in another center had meconium discharge within the first 24 hours of life. However, he had bilious vomits beginning on the third postnatal day and therefore was transferred to our facility. An upper gastrointestinal series done to exclude malrotation revealed no abnormalities. On follow up, he had no vomiting with regular defecation. Oral nutrition was started. On postnatal 11th day, bilious vomiting recurred and an erythema developed on the right lower quadrant. He was operated on in an emergent basis. There was a punctuate closed jejunal perforation at 40 cm distal to the ligament of Treitz ligament. A 4-cm jejuna segment was resected with primary anastomosis.  He was discharged in good health on the 7th postoperative day.

 

 

 

CONCLUSION

Although SIP is most common in babies with low birth weight and in terminal ileum, this is not a definitive rule. Accordingly, the presented cases are term. In contrary to NEC, there is no gradual clinical course. In addition, here is an isolated, punctuate perforation with intact remaining bowel segments. Therefore, unlike NEC, primary repair is more likely to be preferred. Low birth-weight and delayed diagnosis will increase mortality rate. The reported mortality rate varies from 64% to 90%. Late diagnosis is the primary reason for fatal outcome in the first case presented herein.  Early diagnosis of this entity which occurs much less frequently than NEC, and taking the appropriate surgical measures are possible only with increased awareness of the situation

SPONTAN İNTESTİNAL PERFORASYON (SİP): 2 OLGU SUNUMU

GİRİŞ

Spontan intestinal  perforasyon (SİP) özellikle düşük doğum ağırlıklı yenidoğanlarda görülür. Sıklıkla nekrotizan enteroklit (NEK) ile karıştırılmasına karşın, farklı bir hastalıktır.  SİP’de çoğunlukla terminal ileumda ancak her zaman noktasal perforasyon vardır. Tedavi yaklaşımı ve prognoz NEK’den farklı olduğu için doğru tanınması önemlidir

 

OLGU SUNUMU

Olgu 1: Dış merkezde term ve 4250 gram doğan erkek bebek mekonyum çıkışının olmaması ve genel durum bozukluğu nedeniyle postnatal 6. gününde yönlendirildi. Başvuruda genel durumu kötüydü, karın distandüydü ve distansiyon nedeniyle solunum sıkıntısı vardı. Hastanın direkt radyografisinde pnömoperitonyum saptanması üzerine ameliyat edildi. Transvers kolonda noktasal serbest perforasyon saptandı. Primer onarım yapılarak çeşitli seviyelerden biyopsi alındı ve ileostomi açıldı. Hasta ameliyat sonrası 10. saatte ilerleyen septik şok bulgularıyla eksitus oldu. Histopatolojik incelemede alınan tüm örneklerde gangliyon hücresi mevcuttu.

Olgu 2:  Dış merkezde term 3260 gram doğan erkek bebeğin ilk 24 saatte mekonyum çıkışı olduğu öğrenildi. Ancak postnatal 3. günde başlayan safralı kusmaları nedeniyle 4. günde kliniğimize yönlendirildi. Malrotasyona yönelik yapılan kontrastlı çalışmada özellik saptanmadı. Takipte hastanın safralı getirisi kayboldu, oral beslenmeye geçildi, düzenli gayta çıkışı izlendi. Postnatal 11. gününde tekrar safralı kusması oldu ve karın sağ alt kadranda hiperemi gelişti. Hasta acil ameliyata alındı.  Treitz ligamanının 40 cm distalinde, jejunumda noktasal kapalı perforasyon saptandı.4 cm’lik jejunal rezeksiyon ve anastomoz yapıldı. Ameliyat sonrası 7. günde sorunsuz şekilde taburcu edildi.

 

SONUÇ

SİP genellikle düşük doğum tartılı bebeklerde ve terminal ileumda görülürse de, bu bir kural değildir. Nitekim burada sunulan olgular term bebeklerdir. NEK’den farklı olarak, aşamalı bir klinik seyir yoktur. Ayrıca, izole, noktasal bir perforasyon söz konusudur ve kalan barsaklar salimdir. Bu nedenle, yine NEK’den farklı olarak, primer onarım tercih edilebilir. Bebeğin doğum tartısının düşük olması ve tanıda geç kalınması mortaliteyi artırır. Bildirilen mortalite oranı % 64-90 arasında değişmektedir. Burada sunulan ilk olguda tanıda geç kalınması nedeniyle mortalite görülmüştür.  NEK’e göre görülme insidansı daha düşük olan bu antitenin erken tanınması ve uygun cerrahi girişim yapılabilmesi farkındalığın artmasıyla mümkün olacaktır
