Pneumovesicoscopic resection of a refluxing redundant ureteral stump

Although there are a limited number of reports in the literature, transvesical endoscopic approach for vesicoureteral reflux (VUR) and diverticulum surgery with carbon dioxide insufflation into the bladder was described. This video shows the surgical technique of pneumovesicoscopic resection of a long and refluxing distal redundant ureteral stump which was left following a nephrectomy. Video presentation: 10 years old boy was admitted to our clinic with a history of posterior urethral valve ablation and right nephrectomy performed in another hospital. Loss of renal function due to VUR was stated as the indication of nephrectomy in his previous records. The patient had frequent urinary tract infections (UTI) in his postoperative follow up. Following the radiological investigations, we suspected from a refluxing stump, and performed cystoscopy. During cystoscopy, a 5 cm long refluxing distal redundant ureteral stump with 1 cm diameter was determined on the right side.  A 14 mm stone in the stump was shown by ultrasound, and this stone was determined as debris of suture and stone and removed during cystoscopy. He did not have UTI following the removal of the debris and excision of the stump was planned. The surgery was done by pneumovesicoscopy. Following a cystoscopy, a 5 mm port was introduced from the bladder dome, and 2 three mm ports were inserted into the lateral sides of the bladder. With a 12 mmHg insufflation pressure, refluxing distal redundant ureteral stump was removed by transvesical endoscopic technique. Muscular and mucosa defect was repaired by 5/0 monofilament suture. The patient was discharged on postoperative 2nd day. Conclusion: Depending on the experience in laparoscopy, pneumovesicoscopic approach could be preferred for intravesical ureter surgery and diverticulum excision in children.   

Pnömovezikoskopi ile reflülü distal üreter güdüğü çıkarılması

Çocuklarda mesane içine karbondioksit insüflasyonu ile transvezikal endoskopik olarak yapılan vezikoüreteral reflü (VUR) ve divertikül cerrahisi literatürde sınırlı sayıda da olsa tanımlanmıştır. Bu video sunumunda nefrektomi sonrası bırakılmış reflülü distal üreter güdüğünün pnömovezikoskopi yöntemi ile çıkarılmasına yönelik teknik detaylar gösterilmektedir.  Video sunumu: On yaşında erkek hastaya, başka bir merkezde posterior üretral valv ve sağ VUR tanıları ile valv ablazyonu ve fonksiyon kaybı nedeni ile sağ böbreğe sağ nefrektomi yapılmış. Sık aralıklarla idrar yolu enfeksiyonu (İYE) geçiren hastanın 2 yıl önce takibimize girmesi sonrası yapılan tetkik ve takiben sistoskopisinde; yaklaşık 5 cm uzunluğunda 1 cm lümen çapında içinde uzun aksı 14 mm olan taşlaşmış sütür artığı ve debris içeren sağ reflülü distal üreter güdüğü (RDÜG) saptandı. Sistoskopi sırasında RDÜG içindeki debris çıkarıldı. Bu girişim sonrası hastada İYE gözlenmedi. Hastanın sık İYE geçirmesi nedeni olarak düşünülen RDÜG için ikinci bir seansta pnömovezikoskopik eksizyon yapılmasına karar verildi. Pnömovezikoskopi işlemi sistoskopi ile başladı. Takiben mesane kubbesinden 5mm’lik bir port, iki adette 3mm’lik lateral mesane duvarlarından transvezikal olarak çalışma portları girildi. 12 mmHg ile oluşturulan mesane içi gaz insüflasyonu sonrası sağ üreter güdüğü transvezikal endoskopik yöntemle eksize edildi. Oluşan mukoza ve kas defekti 5/0 monofilaman sütür ile onarıldı. Hasta postoperatif 2 gün taburcu edildi. Sonuç: Pnömovezikoskopi yöntemi çocuklarda mesane içi üreter cerrahisi ve divertikül eksizyonu gibi girişimler için laparoskopi tecrübesi olan kliniklerde açık cerrahiye alternatif olarak kullanılabilir.
