Associated Pathologies in Trauma Patients

Aim:  It is possible to notice some unknown pathology while examining trauma patients in detail. In this study, we aimed to present trauma patients diagnosed with associated pathologies.

Material and Method: Between Jan-2003-2013, 625 patients who had treatment because of trauma in our clinic were collected retrospectively. Abdominal and chest radiography, ultrasonography, computerized tomography and intravenous pyelography were used for evaluation.

Findings: In the study, 52 associated pathologies were determined in 49 patients. 38 patients (77 %) were male, 11 patients (23 %) were female and the average of age was 8 (1-15 years). Associated pathology diagnosed in patients were 19 (36,5 %) urinary tract pathologies (UTP) (4 renal cysts, 5 UPJ obstruction, 2 renal ageneasi, 2 renal calculi, 3 hypospadias, 1 ureterocele, 2 spina bifida occulta), 11 (21,5 %) liver pathologies (4 cyst hydatid, 2 cholelithiasis, 3 hepatomegaly,  2 hemangioma),  9 (17 %) splenic pathologies (3 cyst hydatid, 6 accessory spleen), 8 (15 %) thoracic pathologies (3 cyst hydatid, 4 nodule, 1 Morgagni hernia), 2 (4 %) inguinal hernia and 1 (2 %) rectal polyposis, 1 (2 %)  perianal fistula,  1 (2 %) rib fusion. 31 of them (62.5 %) fell down from height, 13 patients (27 %) had car accidents, 2 (4 %) had sharp object injury, 2 (4 %) had animal kick and 1 of them (2 %) had firearm injury.   

Result: Mostly, UTP pathologies were determined in the study. Cyst hydatid was found in nine patients. This rate was five times more than normal ratio. One sholud be careful about associated pathologies in trauma patients. 

Travma Olgularında Tanımlanan Ek Patolojiler

 

Amaç:  Travma hastaları ayrıntılı incelenirken önceden bilinmeyen bazı patolojileri farketmek olasıdır. Bu çalışmada ek patoloji saptanan travma olgularını sunmayı amaçladık.

Gereç ve Yöntem:  Ocak 2003- 2013 arasında kliniğimizde travma nedeniyle yatırılarak tedavi edilen 625 hasta geriye dönük olarak incelendi. Değerlendirme için ayakta direk karın grafisi (ADKG), akciğer grafisi, ultrasonografi, bilgisayarlı tomografi ve intravenöz pyelografiden faydalanıldı.

Bulgular: Çalışmada 49 hastada  52 ek patoloji saptandı. Hastaların 38’si (%77) erkek, 11’i (% 23) kız,  yaş ortalaması 8 (1-15 yaş) idi.  Hastalarda saptanan ek patlolojiler ; 19 (%36.5) üriner sistem patolojileri (ÜGS) (4 renal kist,5 UPJ darlık, 2 renal agenezi, 2 böbrek taşı, 3 hipospadias, 1 üreterosel, 2 spina bifida okulta), 11 (%21.5) karaciğer patolojileri (4’ü kist hidatik, 2’si kolelitiazis, 3’ü hepatomegali,  2’si hemanjiom),  9 (% 17) dalak patolojisi (3’ü kist hidatik, 6’sı aksesuar dalak), 8 (%15) toraks patolojisi (3’ü kist hidatik, 4’ü nodül,1’i morgagni hernisi), 2 (%4) inguinal herni ve 1(%2) rektal polip, 1(%2)  perianal fistül,  1 (%2) de kot füzyonu idi. Hastaların 31’İ (%62.5) yüksekten düşme, 13’ü (%27) trafik kazası, 2’si (%4) kesici delici alet yaralanması, 2’si (%4) hayvan tepmesi, 1’i (%2) ateşli silah yaralanması idi.

 Sonuç:  Çalışmada en sık ÜGS patolojileri tespit edildi. Dokuz hastada kist hidatik saptandı. Bu oran normal popülasyondan 5 kat fazla idi. Travma hastalarında ek patolojilere karşı dikkatli olmak gerekir.
