The Effects of Local and Systemic Growth Hormone on Germ Cell Morphology and Fertility in Unilateral Experimental Testis Torsion Model: (An Experimental Study on Rats)

Aim of the study: To evaluate the effects of growth hormone (GH) on germ cell population of the contralateral testis of rats subjected to unilateral testicular torsion and orchiectomy after 24 hours.

Material and Method: Forty eight male Wistar Albino rats at age of 3 weeks were divided into 5 groups as follows: Group 1: Sham, Group 2: Control, Group 3: Gelatin, Group 4: GH local and Group 5 GH systemic. In the sham group, the right testis was exposed and sutured with 5/0 silk. In Groups 2-5, right testes were rotated 720 degrees and orchiectomy was done 24 hours later. In Groups 3-4, 1cm2 gelatin films were sutured to the right testes, either unloaded (Group 3), or containing 5μg GH (Group 4). In group 5 GH was administered 0.2 IU/100gr/d for 7 days. After 30 days each male was cohabited with two unexposed female for a maximum of 7 days. Then left testis of each rat was removed for histopathological investigation and DNA flow cytometry. Pregnancy outcomes of each female were recorded. Mean seminiferous tubular diameters (MSTD), and mean testicular biopsy scores (MTBS), of each rat and the percentage of haploid (1n) cells were calculated.

Results: There were no differences between groups regarding mean female offspring numbers were significantly decreased in control and gelatin groups compared with sham, GH local and GH systemic groups. Haploid cell percentage was significantly decreased in contralateral testes of the control and gelatin groups compared with sham and GH local and GH systemic groups. Contralateral MSTD and MTBS and haploid cell percentage were significantly lower in GH systemic group when compared with GH local group.

Conclusion: Unilateral testicular torsion and orchiectomy results in contralateral germ cell damage and decrease in haploid cell percentage. However there is no correlation between these findings and pregnancy outcomes. GH administration resulted in restoration of of germ cell histology in contralateral testes and increased haploid cell percentage after unilateral testicular torsion and following orchiectomy. Our findings suggest that GH treatment may improve fertility after unilateral testicular torsion and orchiectomy.

Tek Taraflı Deneysel Testis Torsiyonu Modelinde Lokal ve Sistemik Büyüme Hormonu Uygulanmasının Germ Hücre Morfolojisi ve Fertilite Üzerine Etkileri (Sıçanlarda Deneysel Çalışma)

Amaç: Tek taraflı testis torsiyonu ve torsiyondan 24 saat sonra orşiektomi yapılmış sıçanların karşı taraf testislerindeki germ hücre popülasyonuna sistemik ve lokal uygulanan büyüme hormonunun (BH) etkilerini değerlendirmek amaçlandı.

Gereç ve Yöntem: Üç haftalık 48 erkek Wistar Albino cinsi sıçanlar 5 gruba ayrıldı: Grup 1, Sham, grup 2 kontrol, grup 3 jelatin, grup 4 lokal BH ve grup 5 sistemik BH. Sham grubunda sağ testis ortaya konularak, 5/0 ipekle tespit edildi. Gruplar 2-5’de sağ testis 720 derece döndürülerek torsiyon oluşturulduktan 24 saat sonra orşiektomi yapıldı. Grup 1, 2ve 5 de sol testis ekpoze edildi. Grup 3- 4 de sol testis üzerine yüksüz (Grup 3) ve 2.5μg ve 5μg BH(Grup 4) yüklü 1cm2 jelatin filmler dikildi. Grup 5’de 0.2IU/100gr/gün dozunda BH subkutiküler olarak yedi gün boyunca sistemik uygulandı. 30 gün sonra her bir erkek 15 gün boyunca iki dişi ile birlikte tutuldu. Ardından sol testis histopatolojik araştırma ve DNA flowsitometri için çıkarıldı. Her bir dişinin gebelik seyri izlenerek, her bir denek başına oluşan yavru sayısı kaydedildi. Histopatolojik değerlendirmede her bir sıçan için ortalama seminifer tübül çapları (OSTÇ) ve ortalama testiküler biyopsi skorları (OTBS) hesaplandı. DNA flowsitometri kullanılarak Haploid (1n) hücrelerin yüzdesi saptandı.

Bulgular: Ortalama dişi yavruları açısından gruplar arasında farklılık yoktu (p>0.05). Karşı taraf OSTÇ ve OTBS sham ve BH lokal ve sistemik grupları ile karşılaştırıldığında kontrol ve jelatin gruplarında belirgin olarak düşük idi (p<0.01). Haploid hücre yüzdesi sham, BH lokal ve BH sistemik grupları ile karşılaştırıldığında kontrol ve jelatin gruplarında belirgin düşmüştü (p<0.01). Karşı taraf OSTÇ, OTBS ve haploid hücre yüzdesi BH lokal grubu ile karşılaştırıldığında BH sistemik grubunda belirgin derecede düşmüştü.

Sonuç: Tek taraflı testis torsiyonunun ve orşiektomi karşı testiste germ hücre hasarı ve haploid hücre yüzdesinde azalma ile sonuçlanmaktadır. Ancak bu durum fertiliteyi belirgin olarak etkilememektedir. GH’ın lokal salınımı karşı taraf testislerde hem germ hücre histolojisinin onarımı ve hem de haploid germ hücre popülasyonunda artış ile sonuçlanmaktadır. Elde edilen bulgular büyüme hormonunun lokal uygulanması ile unilateral testis torsiyonu ve orşiektomi sonrası fertilitenin korunmasında etkili olabileceğini düşündürmektedir.
