DISEASES MIMICKING INTUSSUSCEPTION: A DIAGNOSTIC DILEMMA

Introduction: Intussusception is one of the most common abdominal emergencies of early childhood. Symptoms, physical examination and radiologic findings are the key of diagnosis, but some diseases mimicking intussusception can cause unnecessary surgical interventions and morbidity. The aim of this study is to describe the diseases that mimics intussusception.

Method: Seven patients who were initially diagnosed as intussusception with abdominal ultrasonography (US) but the final diagnosis were found as diseases other than intussusception were reviewed retrospectively.

Results: Two patients with ileocolic intussusception were underwent ultrasonography guided reduction by a hydrostatic method but the ultrasonographic findings were persistent. At surgery, only edematous ileocecal valve and mesenteric lymphadenopathy were observed. In three patients with Henoch-Schönlein purpura, initial abdominal US revealed intussusception. The patients with no sign of obstructive symptoms were managed conservatively with the diagnosis of intramural hemorrhage and on follow-up the ultrasonographic findings of intussusception was resolved. One patient with a “target sign” appereance on computed tomography (CT) and US of the abdomen underwent right hemicolectomy due to  a tumor in the cecum. There was no evidence of intussuception. One patient with a cyst in the right lower quadrant accompanying intussusception on CT and US of the abdomen were underwent US guided reduction but the ultrasonographic findings persisted. On exploration, only cecal duplication cyst without intussuception was detected. Segmental resection and anastomosis was performed.

Conclusion: Patients with radiologic findings of intussuception should be evaluated with symptoms and clinical findings before surgical interventions.

İNVAJİNASYONU TAKLİT EDEN HASTALIKLAR: TANISAL SORUN

Giriş: İnvajinasyon erken çocukluk çağının en sık karşılaşılan cerrahi acillerinden biridir. Semptomlar, fizik muayene ve radyolojik bulgular tanıda anahtardır. İnvajinasyonu taklit eden bazı hastalıklar gereksiz cerrahi müdahalelere ve morbiditeye yol açabilmektedir. Bu çalışmanın amacı invajinasyonu taklit eden hastalıkları tanımlamaktır.

Gereç: İlk başta batın ultrasonografi(US)’ sinde invajinasyon tanısı alan fakat sonuç olarak invajinasyondan farklı bir hastalık tanısı alan 7 hasta geriye dönük olarak tarandı.

Bulgular: İleokolik invajinasyon tanısı ile US eşliğinde hidrostatik olarak redüksiyon yapılan 2 hastada US bulguları devam etti. Operasyonda, sadece ödemli ileoçekal valv ve mezenterik lenfadenopati izlendi. Henoch-Schönlein purpuralı 3 hastada ilk US’de invajinasyon raporlandı. Barsak tıkanıklığı semptomları olmayan hastalar intramural kanama tanısı ile konservatif olarak tedavi edildi ve takipte invajinasyonun US bulguları kalmadı. Batın bilgisayarlı tomografi (BT) ve US’sinde “hedef işareti” görünümü olan 1 hastaya çekumdaki kitle nedeni ile sağ hemikolektomi yapıldı. İnvajinasyona rastlanılmadı. Batın BT ve US’sinde sağ alt kadrandaki kiste eşlik eden invajinasyon gözlenen 1 hastada US eşliğinde hidrostatik olarak redüksiyon yapıldı fakat US bulguları devam etti. Operasyonda sadece çekal duplikasyon kisti mevcuttu, invajinasyona rastlanılmadı. Segmenter rezeksiyon anastamoz yapıldı.

Sonuç: Radyolojik olarak invajinasyonu olduğu gösterilen hastalar cerrahi müdahale öncesinde semptom ve klinik bulgularla birlikte değerlendirilmelidir.
