A Comparison of Double HIT and HIT Methods with Polyacrylate/polyalcohol Copolymer for the Endoscopic Treatment of Primary Vesicoureteral Reflux

Purpose: The aim is to compare the success rates between double hydrodistention implantation technique (HIT) and single HIT with polyacrylate/polyalcohol copolymer(PPC) for the treatment of primary VUR.

Methods: Patients admitted to our clinic with primary VUR were prospectively randomized into two groups after parental consent was obtained. Patients divided into two groups either received single HIT or Double HIT endoscopic injection. Sex, age, grade of reflux, number of injections, amounts of injected volume and radiologic success rates were recorded and compared retrospectively.

Results: 44 patients underwent endoscopic injection for primary grade III to V VUR. HIT group consisted of 26 patients with 33 ureters (Grade III: 17, Grade IV: 13, Grade V: 3), while double HIT group consisted of 18 patients with 27 ureters (Grade III: 11, Grade IV: 14, Grade V: 2). There were no differences in sex, age and the grades of VUR and volume of injected material in both groups. Successful correction of reflux was achieved in 21/33(63.6%) ureters of 13/26(50%) patients of the HIT group and 25/27(92.6%) ureters of 16/18(88.9%) patients in the double HIT group. The difference in the success of correction of reflux between HIT group and the double HIT group was significant (p<0,05). After injection, all of the patients were checked for novel hydronephrosis with US; none has developed. The only complication was the serious bilateral hydronephrosis due to bilateral ureterovesical junction obstruction occurred in one patient of double HIT group after six months. Bilateral ureteric reimplantations were applied and pathology revealed fibrosis in excised lower ureters.

Conclusion: PPC is an efficient and safe agent for injection, but patients should be followed for long term for early detection of the formation of ureteral fibrosis leading to ureteral obstruction. As the double HIT method leads to a higher success rate, it should be preferred for appropriate primary VUR cases.

Primer Vezikoüreteral Reflünün Endoskopik Tedavisinde “Polyacrylate/polyalcohol Copolymer” ile Yapılan HIT ve Çift HIT Tekniklerinin Karşılaştırılması

Amaç: Çalışmanın amacı, primer vezikoüreteral reflü tedavisindepolyacrylate/polyalcohol copolymer” (PPC) ile yapılan çift “hydrodistention implantation technique” (HIT) ve HIT tekniklerinin karşılaştırılması.

Metot: Bu randomize kontrollü çalışmada, Ocak 2011-Aralık 2011 tarihleri arasında primer VUR nedeniyle kliniğimize başvuran hastalar, aile onamı sonrasında çalışmaya dahil edildi. Hastalar yapılan cerrahi tedavinin tekniğine göre HIT ve çift HIT gruplarına ayrıldı. Hastaların cinsiyet, yaş, reflü derecesi, injekte edilen volüm ve radyolojik başarı verileri retrospektif olarak toplandı ve karşılaştırıldı. 

Sonuçlar: Evre  III-V olan 44 primer vezikoüreteral reflü hastası çalışmaya dahil edildi. HIT grubunda 26 hasta 33 üreter (evre III: 17, evre IV: 13, evre V: 3), çift HIT grubunda 18 hasta 27 üreter (evre III: 11, evre IV: 14, evre V: 2) mevcuttu. İki grup arasında cinsiyet, yaş, reflü derecesi ve kullanılan PPC miktarı açısından anlamlı fark yoktu. Yapılan girişim sonrası HIT grubundaki 13/26 (%50) hastanın 21/33 (%63,6) üreteri, çift HIT grubundaki 16/18 (%88,9) hastanın 25/27 (%92,6) üreteri tedavi edildi. Reflü düzeltilmesinde iki grup arasında istatistiksel olarak anlamlı fark mevcuttu (p<0,05). Postoperatif US kontrollerinde hiçbir hastada yeni gelişen hidronefroz tespit edilmedi. Bir hastada çift HIT tedavisinden 6 ay sonra üreterovezikal darlığa bağlı hidronefroz gelişti. Bilateral üreter reimplantasyonu yapılmak zorunda kalındı. Üreter alt uçlarının patolojik değerlendirilmesinde fibrozis saptandı.

Tartışma: PPC, primer VUR tedavisinde güvenilir ve etkili material olup, üretelyal fibrozis açısından uzun dönem takip edilmesi gereklidir. Çift HIT yöntemi, yüksek başarı oranı sebebiyle uygun hastalarda tercih edilmelidir.
