Last Year Experiences on Foreign Bodies in the Esophageal Web

Aim: Evaluation of hospitalized foreign body in the esophageal web diagnoses in our clinic. Our experiences throughout the last year

Materials and Methods: The medical records of all hospitalized and treated patients diagnosed with foreign body in the esophageal web between January 2013 and January 2014 in our clinic were examined retrospectively. The files of these patients were evaluated in terms of age, sex, type of foreign body, foreign body localization, treatment, symptoms, and complications.

Findings: 8 male (62%) and 5 female (38%) in total 13 patients were treated endoscopically in our hospital during the last year for foreign bodies in the esophageal web. The average age of the patients was 4.8 years (6 months-12 years). Foreign body was located in the 1st narrow section in 10 patients and in 6 (60%) it was money. In two cases, in the 2nd narrow section residual food was the foreign body in the esophageal web. Out of these two cases, residual food was in the 2nd narrow section due to operated esophageal atresia. In the other one, due to swallowing of corrosive material secondary residual food was in the 2nd narrow section. There were only 2 patients, aged 5 years and more, with foreign body in was in the 2nd narrow section. There was only one patient with a foreign body was in the 3rd narrow section. The foreign body was a disc battery stuck to the mucosa leading to burns in semısurculer mucosa. General symptoms were restlessness, inability to swallow, and pain in the forefront.

Conclusion: Early diagnosis of foreign bodies in the esophageal web is of utmost importance due to the possibility of serious and life-threatening complications. As in our cases, population of children under the age of five is within the risky age group. Among the foreign bodies, money is the most evident one; yet, repetitive esophageal stenosis due to residual food because of operated esophagus or corrosive esophagitis are also present. Underlying diseases should be investigated and thus foreign body removal should be made well prepared. If the foreign body in the esophageal web is a disc battery, it should be removed at an early stage as it may lead to severe burns and perforation.

Çocuklarda Özefagus yabancı cisimleri son 1 yıllık deneyimlerimiz

Amaç: Kliniğimizde özefagus yabancı cismi tanısıyla yatmış olan olguların değerlendirilmesi.

Gereç ve Yöntem: Ocak 2013 ile Ocak 2014 tarihleri arasında kliniğimizde özefagus yabancı cismi tanısıyla yatarak tedavi gören olguların hastane kayıtları incelendi. Bu olguların dosyaları yaş, cinsiyet, yabancı cisim tipi, yabancı cismin lokalizasyonu, uygulanan tedavi, semptom ve komplikasyonlar açısından değerlendirildi.

Bulgular: 8 Erkek(%62), 5 Kız(%38) toplam 13 hasta, hastanemizde son 1 yıl içinde endoskopik olarak yabancı cisim nedeniyle tedavi edildi. Ortalama yaş: 4.8yaş  (6ay-12yaş)idi. 1.darlıkda yabancı cisim 10 hastada olup 6’sında (%60’da ) bu paraydı. 2. darlık’da gıda artığı olup, bu iki olgunun birinde opere özefagus atrezi darlığı nedeniyle gıda artığı vardı, diğerinde ise koraziv madde içimi sonrası oluşan darlığa sekonder gıda artığı vardı.  2. darlıkda yabancı cismi olan 2 hasta vardı, yaşları 5 yaş ve üzeriydi. 3.darlıkda 1 hasta olup, disk pil yabancı cismi mukozaya yapışmıştı, semisürküler mukozada yanık mevcuttu. Genel semptom olarak huzursuzluk yutamama, ağrı ön plandaydı.

Sonuç: Özefagus yabancı cisimlerinin erken tanısı, tedavisi ciddi ve hayatı tehdit edebilecek komplikasyonlardan dolayı çok önemlidir. Olgularımızda olduğu gibi beş yaş altı çocuk popülasyonu riskli yaş grubundadır. Yabancı cisimlerden para en sık görülüp, opere özefagus yada koraziv özefajite bağlı darlıklarda gıda artığına bağlı mükerrer tıkanmalarla karşımıza çıkmaktadır.  Altta yatan hastalık olup olmadığı araştırılmalı yabancı cisim çıkarılmasına bu şekilde hazırlıklı girilmelidir. Özofagustaki yabancı cisim disk pil ise, erken dönemde çıkarılmalıdır. Çünkü ciddi yanık ve perforasyon yapabilir.
